Name of Operation: LIFE GIVERS CHILD CARE & AFTERSCHOOL PROGRAM Facility ID: 64000317 Consultant: APRIL FLEMING Operation Type: Center Case Number: Visit Date: 4/10/2025 Number Present: 0 Completed Date: 4/10/2025 Age: From 0 To 0 Total Minutes: 200 Time In: 12:30 PM Time Out: 03:50 PM Time In: Time Out: List to Use: Center Type Of Visit: Annual Comp Full Announced/Unannounced: Unannounced The purpose of my unannounced annual compliance visit was to monitor this program for compliance with applicable child care requirements. Currently, this facility operates with a Notice of Compliance effective February 13, 2015. The center's last annual compliance visit was completed on May 14, 2024. A checklist was used to note the requirements I monitored during the visit. The last sanitation inspection was completed on April 3, 2025, earning a provisional classification and “10” demerits. The last satisfactory fire inspection was completed on January 30, 2025. Sarah Watts, Administrator, was present and available to assist. Currently, there are no children enrolled. Your facility will have six months to enroll children. Once the children are enrolled, you should contact me within twenty four hours. A no enrollment letter was left with you during today's visit. Fire drills and lockdown/shelter in place, were current. Playground inspections were also current. This facility is owned by Salvation and Praise Tabernacle, Inc. The corporation was researched in the NC Secretary of State database and found to have a current/active status. The following violations of child care requirements were documented during the visit. Violation Number Comment Rule 104 Center has not passed required sanitation inspection and received an approved or superior rating. The facility received a provisional sanitation inspection. 10A NCAC 09 .0304(b) The following violations must be corrected immediately. The letter must describe, in detail, specifically how each violation has been corrected and any supporting documents. You can mail or email me with verification of compliance regarding the violations that were documented. If you choose to mail, two signed and dated copies must be sent and received by April 24, 2025. Please be aware that any written information submitted by you regarding correction of the violations documented during today's visit is legal documentation. It is important that all information submitted be accurate and truthful. Please be aware that if it is determined that information submitted was inaccurate or willfully falsified, Administrative Action, including Revocation of the facility's license, could be issued. Therefore, it is important for you to be knowledgeable of all the licensing requirements that apply to your type of program. It is your responsibility to maintain compliance with all the applicable laws and rules always if they have been monitored or specifically discussed with you in the past. The best way for you to make sure that you meet all requirements is to periodically review the child care law and rules. Compliance History: Prior to today's visit, this facility maintained an 18-month compliance score of 99%. Technical Assistance/ Consultation CHILDREN AND STAFF RECORDS: No children are enrolled currently. Ten percent (10%) or more of existent staff files were monitored. Staff records reviewed were in compliance. You should review the staff and training worksheet and monitor staff files that were not reviewed to ensure compliance for the same information. SANITATION: An approved or a superior rating for sanitation inspections should be maintained. An inspection was conducted on April 3, 2025, earning a provisional rating. Demerits documented were verified in compliance. Once re-inspection is completed, please submit the inspection. CLEAN WATER FOR US KIDS: The Clean Water for Carolina Kids program is expanding to the Clean Classrooms for Carolina Kids program. This is a legislatively mandated effort to address lead and asbestos hazards in North Carolina public schools, licensed child care centers, and licensed family child care homes. Participation allows facilities to meet all rule requirements for identified lead and asbestos hazards, as required by law. In the Clean Water for Carolina Kids facility data, status for lead in water is “analysis complete”. For lead base paint and asbestos, enrollment has started but there are currently no results. You stated you have been in contact with the city to verify the dates the building was built. To ensure compliance, you should follow up with the city and complete the process. PROVIDER DOCUMENTS/FORMS: You should visit the website often to ensure you’re using all updated forms. All requirements must be included if you choose to use your own forms electronically or hard copy. Child care rules were updated effective November 1, 2024. You can view MOODLE training for the Child Care Rule roll out. Currently, this facility operates with a Notice of Compliance effective February 13, 2015. The center's last annual compliance visit was completed on June 7, 2023. The last sanitation inspection was completed on October 18, 2023, earning a superior classification and “8” demerits. The last satisfactory fire inspection was completed on January 12, 2023. Sarah Watts, Administrator/Provider was present and available to assist. One (1) child, three (3) years of age was present during the visit. The child was observed eating lunch. Staff interacted with the child in a positive manner. Fire drills, lockdown/shelter in place and playground inspections were current. This facility is owned by Salvation and Praise Tabernacle, Inc. The corporation was researched in the NC Secretary of State database and found to have a current/active status. The following violation of child care requirements were documented. Operator did not submit the original approved report to DCDEE within one week of the inspection visit on a form provided by the Division. To maintain compliance, you should contact the fire marshal to ensure inspection is scheduled prior to previous inspection date. SANITATION TRAINING ANNOUNCEMENT: Children’s Environmental Health has newly readopted child care sanitation rules effective July 1, 2023. You should review new sanitation rules on our website to ensure compliance. EARLY CHILDHOOD EQUIVALENCY EXAM: The North Carolina Early Childhood Equivalency Exam is a new pathway option comparable to a NC Early Childhood Credential. Applicants who achieve a score of 80% or higher on the NC Early Childhood Equivalency Exam earn the new North Carolina Early Childhood Equivalency Certificate. This certificate is equivalent to a North Carolina Early Childhood Credential and will qualify the individual to be a lead teacher in an early childhood classroom. To take the exam, go to DCDEE Moodle and select Early Childhood Professional Development/Child Care and Development Fund (CCDF)/North Carolina Early Childhood Equivalency Exam.
