Currently this facility operates with a Five-Star License, issued August 20, 2019. The last annual compliance visit was conducted on May 6, 2026. The last sanitation inspection was completed on April 20, 2026, earning a superior classification and fourteen (14) demerits. The last satisfactory fire inspection was completed on August 21, 2025, and was approved for day time care. Ann Dickens, Lead Teacher, and Kimera Tunstall, Assistant Director, assisted me during today’s visit. Forty-two (42) children were present. Children were observed during free choice play, outdoor play, daily routines and preparing for lunch. Staff interacted with children in a positive manner. B. Yarborough Director was not present during the visit. A follow-up visit was conducted due to violation of staff child ratio requirements and unavailable records during the annual compliance visit on May 6, 2026. The compliance letter was received on May 19, 2026. Staff child ratio and other violations documented on May 6, 2026, were verified in compliance today. This program is owned by the Church of The Good Shepherd. This is considered an individual owner. The following violations of child care requirements were documented during the visit. Violation Number Comment Rule 104 Center has not passed required sanitation inspection and received an approved or superior rating. A provisional inspection was received on April 16, 2026. However, a superior reinspection was received on April 20, 2026. 10A NCAC 09 .0304(b) 1041 Prior to employment a Criminal Background Check was not completed. Prior to employment, a criminal background was not completed for a staff member. G.S. 110-90.2(b) 1045 New staff, who had contact with children, did not receive at least 16 hrs. orientation within first 6 weeks. A staff member starting in January 2026 orientation documentation was incomplete. .1101(a) 1757 A valid qualification letter was not on file and available to review at the facility. A valid qualification letter was not available for review for T. Arrington. G.S. 110-90.2(b) & (d) & .2703(e) 1811 Shelter-in-place or lockdown drills were not practiced every three months and/or drill record was incomplete. The last drill was in February 2026, however the drill prior to that was in October 2025. .0604(u);.0302(d)(8) 1824 The trained staff did not review the EPR Plan annually or when information in the plan changed to ensure all information was current. Documentation of the review was not maintained on file. All staff did not review the EPR plan annually. .0607(f) 1897 The child care administrator and all staff did not complete the Recognizing and Responding to Suspicions of Child Maltreatment training within 90 days of employment. A staff member did not complete the training within 90 days of employment. .1102(g) The following violations must be corrected immediately. TECHNICAL ASSISTANCE/CONSULTATION: STAFF: New staff and ten percent (10%) or more staff records were monitored. Staff records reviewed were not in compliance. To ensure all required documents are on file, administration should continue using the staff checklist as a guide. Also, use a calendar as a tracking method for due dates. CRIMINAL BACKGROUND APPLICATIONS: A New feature links criminal background applications to facilities. The training has been completed and the roster was generated. Please remember, this information should be updated in ABCMS on an ongoing basis as staff members are hired and when their employment is terminated. This satisfies the requirement to notify the Division of new child care providers working who were hired or moved into the child care facility within five business days. Please note, the hard copy of the Change of Information form will no longer be needed or accepted. Should you need assistance please contact the Criminal Background Check Unit at 1-800-859-0829 and someone will assist you. Torrey Arrington did not have a valid qualification on file for review. Technical assistance was provided during the visit. Mr. Arrington was notified that a new application should be completed to link his fingerprints that were submitted to complete the process. Bernnadette Yarborough, Director, accompanied me during a walk-through of the facility. During the visit, I discussed the allegations with Ms. Yarborough and two (2) and additional staff members. Limited monitoring of child care requirements occurred during today’s visit. The following violations were confirmed during today’s visit. Violation Number Comment Rule 303 Children were not adequately supervised at all times. On August 12, 2025, a staff member walked out of the classroom, leaving four one-year-old children in a classroom unattended. One child followed the staff member out into the hallway while two additional children crawled to the doorway in an attempt to do the same, before another staff member escorted all three children back into the classroom. .1801(a)(1-5) 902 Each child was not attended to in a nurturing and appropriate manner, or in keeping with the child's developmental needs. On August 12, 2025, a staff member in the classroom for children ages 0-12 months, displayed verbally and physically aggressive behavior toward another staff member, in the presence of five (5) children. G.S. 110-91(10) 904 A child was handled in a rough way, including shaking, pushing, shoving, pinching, slapping, biting, kicking, or spanking. TECHNICAL ASSISTANCE AND CONSULTATION: OUTDOOR ENVIRONMENT: All equipment indoor and outdoor should be in good repair. Broken toys were observed on the playground. This was corrected during the visit when a staff member removed the toys. To maintain compliance, continue completing playground inspection and I encourage staff to check the equipment/toys daily before children access the playground. SANITATION TRAINING ANNOUNCEMENT: Children’s Environmental Health has newly readopted child care sanitation rules effective July 1, 2023. You should review new sanitation rules on our website to ensure compliance. PROVIDER DOCUMENTS/FORMS: You should visit the website often to ensure you’re using all updated forms.
