Currently this facility operates with a Four-Star License, issued July 6, 2018. The last annual compliance visit was conducted on August 13, 2024. The last sanitation inspection was completed on February 5, 2025, earning a superior classification and fifteen (15) demerits. The last satisfactory fire inspection was completed on July 26, 2024. D. Anthony, Assistant Administrator, was present and available to assist. Two (2) children were present during today’s visit. The children were observed napping. Staff monitored children to ensure safety while napping. This program is owned and operated by Chosen Generation Child Care Center. This center was researched in the NC Secretary of State database and had a current/active status. The following violations of child care requirements were documented. Violation Number Comment Rule 1044 Prior to the expiration date of the qualification letter, the child care provider did not complete and submit required forms to complete a criminal background check (a qualification letter is valid for a maximum of five years for the date of issuance). Prior to the expiration date of the qualification letter, a staff member did not complete a submit required forms to complete the criminal background check. G.S. 110-90.2(b) & .2703(n)&(o) 1065 Child care providers scheduled to work in the infant room, did not complete ITS-SIDS training within two months of employment or did not complete the training every three years. Child care administrators did not complete the ITS-SIDS training within 90 days of employment and every three years thereafter. Childcare administrators did not complete the ITS-SIDS training every three years. .1102(f) 1757 A valid qualification letter was not on file and available to review at the facility. TECHNICAL ASSISTANCE/CONSULTATION: CRIMINAL BACKGROUND CHECK/CRIMNIAL BACKGROUND APPLICATIONS: A staff member, Deon Anthony criminal background qualification expired on November 14, 2024. This is a violation of child care requirements and should be corrected within two weeks. Once qualification letter is obtained, please notify me. A copy of the qualification letter should be submitted with the compliance letter. Staff member should complete the process immediately to maintain to ensure compliance. A New feature links criminal background applications to facilities. North Carolina child care administrators can now view and edit their facility’s staff roster in ABCMS, the DCDEE’s criminal background check system. The application should be implemented and used. You have completed the training. The roster was reviewed. I informed you that all staff should be on the roster. CLEAN WATER FOR US KIDS: The Clean Water for Carolina Kids program is expanding to the Clean Classrooms for Carolina Kids program. This is a legislatively mandated effort to address lead and asbestos hazards in North Carolina public schools, licensed child care centers, and licensed family child care homes. Participation allows facilities to meet all rule requirements for identified lead and asbestos hazards, as required by law. In review of the Clean Water for Carolina Kids facility data, the facility status for lead in water results is “analysis complete”. For lead base paint the status is “mitigation pending” and asbestos, the status is “section complete”. Please ensure that all steps are taken to have a completed status. STAR RATED LICENSE (SRL): Hold harmless has been extended until the new QRIS is implemented (SB 425). ECERS-3, ITERS-3, & FCCERS-3: The 3’s are now being implemented.
