Name of Operation: Lighthouse Learning Center Facility ID: 77000206 Consultant: RASHEKA WHEELER Operation Type: Center Case Number: Visit Date: 6/24/2026 Number Present: 12 Completed Date: 6/24/2026 Age: From 0 To 5 Total Minutes: 150 Time In: 10:00 AM Time Out: 12:30 PM Time In: Time Out: List to Use: Center Type Of Visit: Temp Time Period Announced/Unannounced: Unannounced The purpose of my unannounced visit was to monitor compliance with the applicable childcare requirements for the first temporary visit. I met Christy Jordan, Owner /Director, upon arrival and discussed this visit as the center’s first temporary time period visit. We conducted a walk-through of the facility. Currently this center operates with a Temporary License, issued on May 19, 2026. The restrictions include first shift care; Restricted age 0-12 years of age and children less than 2 ½ years in age are in rooms with direct exits. The facility licensing capacity is 96. The NC Secretary of State website was viewed before the visit and Lighthouse Learning Center, LLC active/current January 22, 2026. If any changes to the corporation need to be made or you decide to sell your business, then you must notify your consultant at least thirty days prior to any changes occurring. The license cannot be bought, sold, subleased, transferred to another person or location or inherited. Indoor Learning Environment: As we began the walk-through of the facility, Ms. Jordan and I spoke about enrollment, marketing, and continual working on getting children enrolled at the center. There is a total of 25 children enrolled on first shift. A total of 12 children were observed present. Outdoor Learning Environment: There are three (3) designated spaces for children to utilize for gross motor play. All three (3) spaces were in compliance with child care laws/rules and regulations during today's visit. A fire drill took place on 6/19/2026. The last playground inspection was documented on 6/19/2026. Sanitation Inspection took place on 4/13/26 and received a Superior Status with 0 Demerits. Transportation: This program currently does participate in Transportation. Records: Three children’s records were reviewed during today’s visit, with one violation found. All staff records were reviewed prior to today’s visit during the pre-licensing visit. Nutrition: This program offers breakfast, lunch and afternoon snacks. Lunch consisted of, BBQ Meatball, Mashed Potatoes, Sweet Peas, Peaches with milk. Weapons: Your facility is in compliance with Child Care Requirements regarding firearms. The following four (4) violations were cited during today’s visit. Violation Number Comment Rule 125 Daily records of arrival and departure times for children at the center were not maintained as children arrive and depart and/or were not made available for review. In space #6, a classroom with children present, did not have all children signed in with an arrival time during the visit. 10A NCAC 09 .0302(d)(4) 415 A current schedule was not posted for each group of children for reference. A current schedule was not made available during today's visit for space #1. GS 110-91(12);.0508(a) 1308 Application did not include the names of individuals to whom the center may release the child, as authorized by the person who signs the application. One child's file, did not complete or include any persons that the center may contact in the case of an emergency on the child's application. .0801(a)(7) 1311 Emergency medical care information was not on file in the center on the child's first day of attendance and/or was not updated as changes occurred or at least annually for each child. One child's file, enrolled in June 2026, did not have a completed emergency medical history on file. .0802(c) Compliance History: The next two Temporary Time Period visits will be unannounced. During those visits a full assessment of the center's ability to maintain compliance with all applicable Child Care Requirements will be conducted. Please note that any violations cited today or during future visits will negatively impact your compliance history. North Carolina General Statute 110-90(4)(d) requires all child care facilities to maintain a compliance history of at least seventy-five percent (75%) for the past 18 months or during the length of time the facility has operated, whichever is less. Failure to do so may result in the issuance of a provisional license or other administrative action against the facility's license. As discussed, at the end of your temporary license period you must be at least a three (3) star facility in order to continue to provide care for children who receive subsidies. WORKS: If you haven’t done so as of today, please make sure your staff establish an NCID for entering their information in the DCDEE WORKS system. Once all staff have completed entering their information in the DCDEE WORKS system, then mail the official transcripts to the Workforce and let me know when they are mailed. Refer to the information on the Division’s website for detailed instructions regarding how to use DCDEE WORKS. To help child care providers meet the new training requirements, the Division of Child has free Moodle online training modules. Click on the Course Catalog tab at the top of the page on the website to login to your training. Technical Assistance: A friendly reminder, the weather during the Summer may produce increasing rainy days in which may cause the grass to grow more frequently. Please make sure that you are considering this each week, making sure the grass is reasonable and safe for children. We slso spoke about aerosol cans of sunscreen. I stated all aerosol cans are required to be locked. Ms. Jordan stated that Sanitation (EHS) no aerosol cans should be in a licensed child care program because it may cause combustion or fire. I stated our rule regarding aerosol cans. I highly recommend that you spend time reviewing the ncchildcare.dhhs.gov website, visit QRIS Modernization information regarding the Rated License Assessment, Education Requirements for (Administrator, Lead Teacher, Teacher, Group Leader). NCRLAP.org website for additional information regarding interactions, self-study, and required materials. It is important for licensed providers to be knowledgeable of all of the licensing requirements that apply to their type of program. Child care consultants can make unannounced visits at any time. It's the provider's responsibility to be in compliance with all of the applicable laws and rules. RESPONSE: You should correct the violation(s) immediately.
