This visit was conducted with C. Johnson, teacher. Ms. Anthony, Administrator, was off property today. Ms. Brenda Roten, Head Start, Director, was called to come out and sign the visit summary today. Your program currently operates with a FIVE (5) Star license issued 5/2/19. Restrictions include Daytime care only; meets enhanced ratios minus one; meets enhanced space. The NC Secretary of State website was reviewed on 5/12/25 and Blue Ridge Opportunity Commission, Inc. was listed as current and active. The program’s compliance history was 98% prior to today’s visit. All programs are required to maintain at least 75% compliance. Today, the following items were monitored: • Supervision • Staff / Child Ratio • CPR / First Aid • Special Training • CBC Qualification • ITS – SIDS • Emergency Medical Care Plan • Administration of Medication • Storage of Hazardous Products • Storage of Medication • General Safety • Discipline • Adequate / Approved Space • Program Records • License Posted • Permit Restrictions The license was observed, and the restrictions were found in compliance. A walk-through of the facility was completed today. Indoor and outdoor areas were observed. Staff/child ratio and space capacity were monitored as indicated on the attached worksheet. Supervision was adequate during the visit. I observed staff/child interactions today. I observed children having free choice center time and individual time with teachers. No new staff were reported at this program. No files for new staff were reviewed. No medications were observed nor reported. Storage hazardous items was monitored today. A playground inspection was recorded today 5/14/25. A fire drill was recorded 3/18/25 at 1:40 pm with two (2) minutes, one (1) second to evacuate. There was no documentation of a fire drill for April 2025. A lock down emergency drill for the facility was recorded 12/4/24 at 11:50 am with one (1) minute, forty-two (42) seconds to lockdown. There was no documentation for an emergency drill within three months of the last emergency drill in December 2024. The most recent fire inspection was on 10/18/24. A copy of this fire inspection was received on the last annual compliance visit on 12/4/24. I observed a sanitation inspection was last conducted on 3/18/25 earning a Superior classification with seven (7) demerits. You stated that you do provide transportation in the morning and afternoon for Head Start children. However, during today’s visit you stated that the minibus is in the shop for service. The minibus was not on the property today. The minibus was last used on the morning of Tuesday, May 13, 2025. The following violations were cited during today’s visit: Violation Number Comment Rule 805 Fire drills were not practiced monthly and/or the drill record was incomplete. There was no documentation of a fire drill for April 2025. .0604(t); .0302(d)(5) 1811 Shelter-in-place or lockdown drills were not practiced every three months and/or drill record was incomplete. There was no documentation of an emergency drill within three (3) months of December 2024 when the last drill was conducted. .0604(u);.0302(d)(8) Child Care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. Any violation(s) documented may impact the compliance history score. You must correct the violations cited during today's visit immediately and send me documentation verifying compliance on or before Wednesday, May 28, 2025. Technical assistance discussed during today’s visit: • There was no documentation for a fire drill for the month of April 2025. See Childcare Rule 10A NCAC 09 .0604(t) & .0302(d)(5). We discussed that it is important to record each drill as well as it is extremely important for the children to know how to react and where to go in the event of a real fire. • There was no documentation of an emergency drill within three (3) months of the last emergency drill in December of 2024. See Childcare Rule 10A NCAC 09 .0604(u) & .0302(d)(8). We discussed that it is important to document each emergency drill for your records. We discussed how important it is for children to practice emergency drills to know where to go and what to do in the event of a real emergency. Additional Information/Consultation: • Make plans now to join the virtual Springtime Center Director’s meeting on May 28, 2025, from 1:00-2:00 pm. The link was sent to you in an email from me. You will receive 1 hour of training credit. • You must notify your consultant 30 days prior to changing the ownership status of your facility. • You are still in Hold & Harmless. • Visit NCRLAP website and read the information on Connecting with the Three’s! ECERS-3 and ITERS-3 which began on 2/1/25. • The QRIS is in the process of being implemented. You can go to the DCDEE homepage and go to the “What’s New?” tab, then scroll down to QRIS modernization to read about Updates. • The process of notifying the Division has changed and is now captured in ABCMS. This change has been in effect since February 2024. Effective immediately, you will need to obtain a Business NCID and complete Provider Portal training in Moodle at https://www.dcdee.moodle.nc.gov/course/view.php?id=119. No action is needed on your part if you have completed the reference training and are currently using the ABCMS Provider Portal to update information regarding new hires or residents. Once the training has been completed and access has been given, you must verify your facility roster to ensure current staff are noted on the roster. This information should be updated in ABCMS on an ongoing basis as staff members are hired and when their employment is terminated. This satisfies the requirement to notify the Division of new childcare providers working who were hired or moved into the childcare facility within five business days. The compliance of this rule will be monitored during your next visit. Please note, the hard copy of the Change of Information form will no longer be needed or accepted. Ms. Anthony has completed the ABCMS training and has all staff in the portal. • Public schools and licensed childcare facilities in North Carolina are eligible to participate in the funding mechanisms to cover the costs of testing for lead in water and inspecting for asbestos and lead-based paint hazards beyond May 1, 2024, deadline listed in 10A NCAC 41C .1002(a). Due to the recent impacts of Hurricane Helene and other natural disasters, this deadline has been revised statewide, and facilities that have not yet enrolled should complete the online process by May 31, 2025. The Clean Classrooms for Carolina Kids Program provides free facility-wide testing and inspections without any out-of-pocket costs. Due to the volume of testing and inspections, completion of the online enrollment process in the program is considered temporary compliance with the testing and inspection requirement and will allow facilities to participate in the funding mechanisms. Required water testing for childcare centers under rule 15A NCAC 18A .2816 is not affected by the new May 31, 2025, deadline. Enroll today at https://www.cleanwaterforcarolinakids.org/enroll. This visit was conducted by Karen Young, Childcare Consultant, with Amber Sparks Anthony, Director. Your program currently operates with a license. Restrictions include Daytime care only; meets enhanced ratios minus one; meets enhanced space. The license was observed, and the restrictions were found in compliance. The NC Secretary of State website was reviewed on 12/11/23 and Blue Ridge Opportunity Commission, Inc. was listed as current and active. The program’s compliance history was 97% prior to today’s visit. All programs are required to maintain at least 75% compliance. A walk-through of the facility was completed today. Indoor and outdoor areas were observed. There were no violations cited on the indoor or outdoor environment today. Staff/child ratio and space capacity were monitored as indicated on the attached worksheet. Supervision was adequate during the visit. I observed positive interactions today. I observed children having free choice center time, circle time, and outdoor gross motor play. I observed developmentally appropriate materials and activities were available to the children in the classrooms. Your program was also monitored for compliance with implementing an approved curriculum as required for all four- and five-star licensed facilities where four-year-old children are enrolled. Your facility uses Creative Curriculum with Teaching Strategies Gold and the developmental domains were listed on the lesson plans. The last annual compliance visit was conducted on 1/11/23. A sanitation inspection was completed 10/4/23 with a Superior classification and seven (7) demerits. The last fire inspection was conducted 10/10/23. Program records and required postings were monitored. A fire drill was conducted on 12/11/23 at 11:25 am with one(1) minute, two (2) seconds to evacuate thirteen (13) children and four (4) adults. A shelter in place drill was documented on 11/6/23 at 1:50 pm with three (3) minutes, one (1) second to shelter-in-place. An outdoor inspection was documented on 11/29/23 by D. Stringer with no issues notes on the playground inspection checklist. Staff files and children’s records were monitored per DCDEE procedures. I monitored two (2) staff files today, including one(1) new staff. I monitored two (2) children’s files. There were no violations cited on the staff or children’s files today. Nutrition requirements were monitored per DCDEE procedures. Storage of hazardous items was monitored today. I observed a can of Phenomenal Citrus Hospital Disinfectant Deodorant aerosol spray unlocked on the driver’s side door of the mini bus. No medications and no allergies were observed nor reported. You stated that you do provide transportation in the am & pm for the Head Start & NC Pre-K children. During today’s visit, I monitored emergency information, permission to transport forms for all children being transported, and the bus log. I observed first aid kits and fire extinguishers on the vehicle. I monitored one (1) vehicle today. I observed a 2009 GMC Thomas private mini bus, with tag number CEF 4615 . This vehicle was clean, in good repair, and had working seatbelts and eleven (11) child restraint car seats installed. The mini bus had adequate tire tread and was current on inspection and insurance. Your next inspection date on this vehicle is due 7/1/24 for Selective Insurance Company of the Southeast and the license tag expires 12/21/24. I observed the monthly bus transportation checklist log as well as the children’s bus on/off log. I received a copy of both driver’s licenses today. The following violation was cited during today’s visit: Violation Number Comment Rule 840 All corrosive agents, pesticides, bleaches, detergents, cleansers, polishes, any product which is under pressure in an aerosol dispenser, and any substance which may be hazardous to a child if ingested, inhaled, or handled were not stored in a locked room or cabinet. I observed an areosol spray container of Phenomenal Citrus Hospital Disinfectant Deodorant unlocked on the driver's side door in the mini bus. .2820(b) Child Care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent.
