This family child care home currently operates with a Two Star License issued July 8, 2022. The last annual compliance visit was completed on 9/17/2025. Ruth Godsey, the owner/operator, was present and available to assist. Five (5) children were present during my visit. The children were observed in toileting and handwashing routines, eating lunch, free choice play, and a teacher directed art activity. Lunch consisted of peanut butter and jelly sandwiches, peas, cheese slices, pears, and milk. The caregiver interacted with the children in a positive manner. A walkthrough of the home was completed today. Fire drills, shelter in place/lock down drills, and playground inspections were all monitored and in compliance. You stated you do not provide transportation. Your husband, T. Godsey, and two underage children also reside in the home also. Required criminal background qualification letters were observed and current. If there are changes in your household, please contact me and if anyone moves into your residence, please ensure they complete the Criminal Background Check (CBC) process and have a valid qualification letter on file prior to moving into the Family Child Care Home (FCCH). I used the Annual Compliance Monitoring Checklist for Family Child Care Homes and the Family Child Care Home Item Number Listing (DCD 0357) as a basic monitoring tool to assess compliance with all applicable child care requirements pertinent to this facility. Please remember the entire home is licensed for child care and must meet the applicable child care requirements. Although you designated space to be used for child care in a specified section of your home, the license is issued for the entire location. Any monitoring visit conducted may include spaces other than those you designated for child care. A computer-generated visit summary was reviewed and left with you at the conclusion of the visit. The following violations of child care requirements were documented. Violation Number Comment Rule 1405 Authorization for up to 12 months to apply over-the-counter medications, topical ointment, topical teething ointment or gel, insect repellents, fluoridated toothpaste, lotions, creams, and powders; such as sunscreen, diapering creams, baby lotion, and baby powder, to a child, when needed, was not in writing and did not contain information required by rule .1720(b)(7). Aquaphor ointment for T. Brooks did not have a permission to administer form on record for review. 10A NCAC 09 .1720(c)(4) 1920 Indoor and outdoor area was not checked daily for debris, vandalism, broken equipment, and animal waste. Animal waste was observed in the outdoor play area. .1719(a)(17) 2031 Operator did not provide a copy of the shaken baby syndrome and abusive head trauma policy to parents at time of enrollment, and / or within fourteen days of a changes to the policy. Shaken Baby and Abusive Head Trauma policy was not on record for J. Coleman. .1726(b)&(c) Child Care Licensing Requirements are established to ensure a safe, healthy, and developmentally appropriate environment for all child care facilities. Therefore, it is important for you to be knowledgeable of all licensing requirements that apply to your facility. Child Care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. It is your responsibility to maintain compliance with the applicable laws and rules; always whether they have been monitored or specifically discussed with you in the past. The best way for you to ensure that you are meeting all requirements is to periodically review the childcare laws and rules on the Division of Child Development and Early Education (DCDEE) website. Any violation(s) documented may impact the compliance history score. The violation(s) documented must be corrected immediately. On or before July 13, 2026, I must receive a written, dated, and signed compliance letter that describes accurately and in detail how and when the violations were corrected. Please be aware any information submitted by you is accurate documentation. If any information provided in the letter is not accurate, this may be considered falsification of information. If sufficient information is not received by the due date, a follow-up visit will be conducted. COMPLIANCE HISTORY Prior to today's visit, this facility has maintained an 18-month compliance score of 100%. TECHNICAL ASSISTANCE/ CONSULTATION SAFETY AND SUPERVISION Animal waste was observed in the outdoor play area. CHILDREN’S RECORDS Shaken Baby and Abusive Head Trauma policy was not on record for J. Coleman. MEDICATIONS Aquaphor ointment for T. Brooks did not have a permission to administer form on record for review. REMINDERS The facility’s EPR should be reviewed yearly before the previous one expires. QRIS/STAR RATED LICENSE During the visit conducted on 2/24/2026, we discussed the different pathway options for the new QRIS system. You decided to complete Pathway 1-Program Assessment Pathway for FCCH. However, on 5/19/2026, an email was received from you stating you would like to decrease the two-star rating to a one-star rating. Upon receiving the email, a conversation took place via phone. You stated it would not affect your facility to decrease in stars because you currently have all private paying families. We discussed that a one-star facility is not eligible to receive subsidy payments for as long as they have a one-star rating. You stated that you understood. The facility’s license will be processed as a one-star, pending approval by the Raleigh office.
