Currently, this facility operates with a Three-Star license issued on May 18, 2017. The center's last annual compliance visit was completed on October 31, 2024. The last sanitation inspection was completed on August 15, 2024, earning a “Superior” classification. There was a sanitation inspection conducted on April 2, 2025, and yielded a “Superior” classification. A copy of the most recent inspection was obtained during the visit. The last satisfactory fire inspection was completed on January 3, 2025. S. Patterson Administrator, was present and available to assist. Thirty-four (34) children were present during my visit. Children were observed in free choice play, teacher directed activities, story time, toileting and handwashing routines, and during lunch. Lunch consisted of meatballs, green beans, peaches, and milk. All staff interacted with children in a positive manner. This facility does not provide transportation. The center is owned and operated by Kingdom Kids Daycare Center, LLC. This LLC was researched in the North Carolina Secretary of State Corporation Database and found to have a Current/Active Status. I used the Annual Compliance Monitoring Checklist for Child Care Centers and the Child Care Center Item Number Listing as a basic monitoring tool to assess compliance with all applicable child care requirements pertinent to this facility. Ten percent of staff records, ten percent of children’s records, and all program records were monitored. All children’s records and program records were all in compliance. One (1) violation was documented for staff record keeping/training. One (1) violation was documented for the outdoor play area. Any violations observed were discussed with you and documented on the computer-generated visit summary left with you at the conclusion of the visit. The following violations were documented today. It is your responsibility to maintain compliance with the applicable laws and rules; always whether they have been monitored or specifically discussed with you in the past. Currently, this facility operates with a Three-Star license issued on May 18, 2017. The center's last annual compliance visit was completed on December 18, 2023. A one page visit summary was left with you during the visit due to printing issues. A computer generated visit summary and enrollment form was emailed to the provider during the visit. A checklist was used to note the requirements I monitored during the visit. The last sanitation inspection was completed on August 15, 2024, earning a “Superior” classification and “3” demerits. The last satisfactory fire inspection was completed on January 3, 2024. M. Revis, Owner/Operator and T. Richardson, Administrator, were present and available to assist. Fifty (50) children were present during my visit. Children were observed in free choice play, teacher directed activities, story time, during lunch and infant feeding. All staff interacted with children in a positive manner. This facility does not provide transportation. The center is owned and operated by Kingdom Kids Daycare Center, LLC. This LLC was researched in the North Carolina Secretary of State Corporation Database and found to have a Current/Active Status. The following violations of child care requirements were documented. Violation Number Comment Rule 842 A drug or medication was administered without written authorization and/or instructions from a child's parent or authorized health professional. Authorization to administer a EPI pen was not on file for review. 10A NCAC 09 .0803(1)(a & b) 844 Prescribed medicine was not in original labeled container or accompanied by signed and dated written instructions from prescribing physician or health care professional. A prescribed EPI pen was not in the original labeled container. .0803(2)(a) 1812 The center did not complete an EPR Plan within four months of completing the EPR in Child Care training and/or the Plan was not completed on a template provided by the Division. The EPR plan was not completed and updated on the template in the Risk Management Portal. .0607(c) 1825 All staff did not review the center's EPR Plan during orientation and/or on an annual basis with the trained staff. Documentation of the review was not maintained on file. Nine staff members did not review the EPR plan with trained staff annually. .0607(f) 1834 Application did not have a medical action plan attached for any child with health care needs such as allergies, asthma, or other chronic conditions that require specialized health services. A child with health care needs did not have a medical action plan available for review. .0801(b) The above violations must be corrected immediately. Compliance History: Prior to today's visit, this facility has maintained an 18-month compliance score of 95%. CONSULTATION/TECHNICAL ASSISTANCE CHILDREN AND STAFF RECORDS: Ten percent (10%) or more of children’s records were monitored. Children records reviewed, were in compliance. Other children’s files not reviewed should be monitored to ensure compliance with the same information. All new staff and ten percent (10%) or more of existing staff files were monitored. Staff records reviewed were in compliance. Other staff files not reviewed should be monitored to ensure compliance with the same information. EPR Plan: The EPR plan was reviewed by staff who completed the training. All the other staff who haven’t completed the training, the plan was not reviewed with them. Also, the plan has not been completed on the template in the Risk Management Portal provided by the Division. The EPR plan was not available for review. MEDICATION: All medication should be in its original container with the prescription information. A medical action plan and permission to administer medication form should also be filed for children with life-threatening medical conditions. One child who requires an EPI pen did not have permission to administer form, nor a medical action plan on file for review. The epi also was not in the original container. CLEAN WATER FOR US KIDS: The Clean Water for Carolina Kids program is expanding to the Clean Classrooms for Carolina Kids program. This is a legislatively mandated effort to address lead and asbestos hazards in North Carolina public schools, licensed child care centers, and licensed family child care homes. Participation allows facilities to meet all rule requirements for identified lead and asbestos hazards, as required by law. Currently operating child care facilities are required to test lead in paint and asbestos once only unless the consultant notes a concern during a visit and refers to the program to environmental health. All existing licensed programs must complete the application summary for their program regarding lead paint and asbestos by November 1, 2024. The application is on the https://www.cleanwaterforuskids.org/en/carolina/ website. Testing for lead in water was completed and results were verified in the clean water for US kids facility data. However, results for lead base paint and asbestos have not been completed. Please be sure you are enrolled, and you are meeting requirements for lead base paint and asbestos. STAR RATED LICENSE (SRL): Hold harmless has been extended until the new QRIS is implemented (SB 425). This means that no child care facilities in cohort one will have a rated license assessment unless they voluntarily want to. Also, cohort two do not need to start a preparation year. Currently, this facility operates with a Three-Star license issued on May 18, 2017. The center's last annual compliance visit was completed on February 17, 2022. The last sanitation inspection was completed on May 24, 2022, earning a “Superior” classification and “5” demerits. The last satisfactory fire inspection was completed on January 3, 2024. M. Reavis, Owner/Operator wasn't present. S. Patterson, Assistant Director was present and available to assist. Forty-three (43) children age ranging one (1) to five (5) years old were present. Fire Inspection: Fire inspection report was not submitted to DCDEE within one week of the inspection date. You submitted the fire inspection during the visit. DCDEE WORKS: New staff that has not had their education evaluated will need to create a WORKS account by accessing our website under the provider tab, then click DCDEE WORKS. Once account is created, staff should upload or submit original transcripts or certificates to the division for evaluation of their education. Existing staff who has completed additional college course work should also upload and or submit new transcripts. Staff should also apply for what position they want to be evaluated for. NEW CRIMINAL BACKGROUND CHECK LEGISLATIVE CHANGES: The 2022-2023 Appropriations Bill (HB 103) signed by Governor Cooper on July 11, 2022, made two important changes to North Carolina General Statute 110-90-2 regarding child care Criminal Background Checks. • The period for background check qualification renewals was revised from three years to five years. • The provisional qualification period was reduced to forty-five days. The Division intends to implement these changes as soon as possible. An individual’s current Qualification Letter will be extended 2 years for the total of 5 years. New Qualification Letters will not be generated. Individuals with a Provisional Qualification Letter will receive a Qualification Letter on the 45th day. Letters are generated in the Criminal Background Check portal. HEALTH AND SAFETY TRAININGS: Health and safety trainings are due every five (5) years. If any staff completed trainings in 2017, trainings will be due this year, 2022. I encourage staff to be intentional and start completing trainings. Health and safety trainings are now on MOODLE. There are a couple of ways for a student to access the health and safety training. • The enrollment process and completion of course materials tend to work best with a windows laptop or desktop using Firefox, but other devices and Google Chrome should also work. • From the Moodle home page, students need to log-in with their NCID and password. Veronica Lawrence, Assistant Director, accompanied me during a walk-through of the facility. During the visit, I discussed the allegations with Sandra Patterson, Director, and two additional staff members. Limited monitoring of child care requirements occurred during today’s visit. Violation Number Comment Rule 303 Children were not adequately supervised at all times. On January 24, 2024, staff members were not aware that a two-year-old child was involved in an incident on the playground that resulted in the child being injured until informed by another child on the playground. .1801(a)(1-5) 852 Incident reports were not completed each time a child was injured, it did not include all the information required in rule, it was not signed by the parent and/or it was not maintained in the child's file. A staff member failed to complete an incident report or notify a child's parent when a child was involved in an incident on the playground and then displayed signs of possible injury. .0802 (e) 873 Center staff did not follow the EMC plan. The last sanitation inspection was completed on May 24, 2022, earning a “Superior” classification and “5” demerits. The last satisfactory fire inspection was completed on January 9, 2023. M. Reavis, Owner/Operator wasn't present. S. Patterson, Assistant Director was present and available to assist. Sixty-six (66) children age ranging zero (0) to nine (9) years old were present. Children were observed in free choice play, teacher directed activities, lunch time, story time, and infant feeding. All staff interacted with children in a positive manner.
