V. Mcleod, Administrator assisted me with the visit. Twenty-four (24) children ranging in ages zero (0) years old to five (5) years old. The children were observed participating in morning activities, engaged in free-choice play and interacting with caregivers. Limited monitoring of the child care requirements was completed today. The following child care requirements were monitored: Staff/child ratio/group sizes, supervision, medication storage, storage of hazardous products, staff records, program records, adequate approved space and permit restrictions. Two (2) violations of child care requirements were documented. At the conclusion of the visit, a computer-generated visit summary was reviewed and left with you. Violation Number Comment Rule 1882 Medication authorization, giving the caregiver standing authorization did not meet the specifications in rule. A tube of Aquaphor ointment was stored, in the room for infants/toddlers, for M. Scott, without a medication authorization form. Powell within 90 days of hire. .1102(g) Child Care Licensing Requirements are established to ensure a safe, healthy, and developmentally appropriate environment for all child care facilities. Therefore, it is important for you to be knowledgeable of all licensing requirements that apply for your facility. Child Care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. This program currently operates with a Three-Star license, issued November 20, 2023, earning two (2) points in the education component, six (6) points in the program standards component and one (1) quality point. The last sanitation inspection was completed April 1, 2025 with a “Superior” classification. The last fire inspection was conducted October 22, 2024 with a “Satisfactory” result and approved for daytime care only. You, V. McLeod, Administrator, was available and assisted with the visit. Twenty-nine (29) children were in attendance. The children were observed in free play activities, teacher directed activities, outdoor play, and eating lunch. Lunch consisted of fish nuggets, corn, pineapple, and milk. All staff interacted with children in a positive, nurturing manner. I used the Annual Compliance Monitoring Checklist for Child Care Centers and the Child Care Center Item Number Listing as a basic monitoring tool to assess compliance with all applicable child care requirements pertinent to this facility. The staff and training worksheet was emailed to me prior to the visit. Ten percent of staff records and ten percent of children’s records were monitored. All children and program records were in compliance. Two (2) violations were documented for staff record keeping. A computer-generated visit summary was reviewed and left with you at the conclusion of the visit. The following violations of child care requirements were documented today. Note, other Pathways to the Profession of Early Childhood Teacher still include: · EDU 119 – Introduction to Early Childhood Education: This 12-week course introduces the foundations of culturally responsive, equitable and inclusive early childhood education. Ms. McLeod spoke with LaShonda Wood, facility contact, during today’s visit and notified you of my presence and the reason for today’s visit. I reviewed today’s visit summary with Ms. McLeod. I spoke with Ms. Wood via phone call today to review specifics of today’s visit and Rated License information. All rooms currently being used for child care were monitored today. You are using a total of four spaces for child care. A total of nine (9) children were present in Room 1 as I was conducting observations. When asked about the ages of the children, it was discussed that one of the children is two (2) years old today and was grouped with the children in Room 1 today. The youngest child present in Room 1 today was an infant; therefore, four (4) infants, four (4) one-year olds, and a two (2) year old was observed present and grouped together in Room 1. When asked, both staff present in Room 1 stated the child turned two (2) years of age today. Ms. McLeod stated she was aware a child did turn two (2) years old today, but she mistakenly had not moved the child to the next age group. When we reviewed this, Ms. McLeod moved the child to the next age group into Room 2. As we reviewed, this was documented as a violation today and was also verified as corrected during the visit. We reviewed that based on the violation, an unannounced follow up visit will be conducted in the near future to ensure the applicable grouping of children is maintained. The children were observed remaining in the areas where they typically attend. The attendance sheets were reviewed for each group of children. As children have been moved up to other age groups, you have been documenting and updating the attendance sheets to reflect the child(ren) were “moved”. The children in various groups today were observed finishing lunch, completing daily routines such as hand washing and toileting. The children rested/slept on their assigned covered cots during the visit. The infants and toddler aged children were eating and being fed. When asked, no staff have been hired since the last monitoring visit conducted April 16, 2024. Continue to use the Staff Records Checklist as a guide to ensure all required information is completed and on file as required per applicable NC Child Care rules. Required center program records were also monitored today. As a reminder, ensure both a fire drill and an outdoor playground inspection are completed each month and document the drill and inspection on the applicable forms. Overall attendance and enrollment were collected today. The attached space, capacity and enrollment worksheet reflects the enrollment and attendance information along with the staff that were present and caring for children today. The attached worksheet reflects the children present in particular rooms before the child that was two (2) years of age was moved from Room 1 to Room 2. The following violation was documented and verified as corrected today. Violation Number Comment Rule 316 Children under one year of age were not kept separate from children two years and older. A two (2) year old child was grouped together with eight (8) other children in Room 1 today. The violation documented today may affect this facility’s current Compliance History score. As previously reviewed, all facilities must maintain at least a 75% compliance history score over an eighteen-month time frame to be in good standing with DCDEE. If a facility’s compliance history falls below 75% over an eighteen-month time frame, some type of administrative action may be issued. CORRECTION OF THE VIOLATION: The violation documented today was verified as corrected during the visit; however, email me a compliance letter addressing how you plan to ensure the grouping of children is maintained, specifically when a child has a birthday. Email or mail the compliance letter to me within the next two weeks, on or before May 14, 2024. As stated, I will also be conducting an unannounced follow up visit to ensure the violation has been corrected as well. Technical Assistance/Consultation/Reminders Specific to Today’s Visit: STAFF/CHILD RATIOS: This facility is currently meeting enhanced ratios, and this is listed as a restriction on the child care license. The applicable staff/child ratios were posted on the wall in each group of children today. We reviewed grouping children and that when a child turns two years of age, the child has to be moved to the next age group if infants are present. Four (4) Infants, four (4) one-year olds, and a two (2) year old were present grouped together in Room 1 today. When reviewed, Ms. McLeod moved the child that was two (2) years of age to Room 2 to correct the violation. This child’s birthday was today. We have reviewed staff/child ratios and the grouping of children during previous visits. Ms. McLeod stated she made a mistake today by not moving the two (2) year old today. We discussed reminding staff in the applicable rooms to notify Ms. McLeod or whoever is in charge of when children have birthdays to ensure the children are moved and grouped appropriately, based on applicable NC Child Care rules. SAFE SLEEP REQUIREMENTS: We reviewed purchasing a thermometer to be used in Room 1, where infants are cared for to ensure the temperature is not exceeded based on NC Child Care rules. We discussed using a baby gate that will be secure at the entrance door to Room 1 so that the door can remain open to keep the temperature of the room cooler. The air circulates throughout the facility but when the door leading into Room 1 is closed, the room temperature appears to be affected. The thermostat registered 71 degrees Fahrenheit on the wall outside of Room 1 and the air was set to 69 degrees Fahrenheit. RATED LICENSE: We reviewed the current procedures for a facility that has completed a change of ownership and how this affects the facility’s rated license. We reviewed the rated license information today and Ms. McLeod signed the Program Standards Form and Initial Application for Assessment For a Two Through Five Star Rated License. Based on current procedures, this facility will transition to a star rated license based on the points earned from the previous facility prior to this change of ownership. If staff education increases in the future and you choose to be reassessed, contact your assigned Child Care Consultant, regarding this information. You stated several staff members are planning to enroll to complete EDU 119 and additional Early Childhood coursework from the local Community College this summer. We reviewed that after staff complete coursework to ensure they submit their official transcripts to the Workforce Education Unit at DCDEE for education evaluation. You stated you are working to increase points in Education Standards so you can meet alternate enhanced requirements in the future. As we reviewed, based on recent violations documented pertaining to a complaint, some type of administrative action may be warranted. I am working on this information and will be in contact with you regarding further action. You submitted information to me via email April 22, 2024, regarding some training that will be conducted with staff and verification of a recent staff meeting you conducted. This information will be further reviewed, and I will contact you regarding a possible administrative action. If an administrative action is warranted, I will be in contact with you regarding processing the rated license for this facility. As of today, this facility will earn the below in each component of the rated license based on the previous facility’s information. This facility will be due for a rated license reassessment beginning July 1, 2025, through June 30, 2026, based on Cohort 2. PROGRAM STANDARDS: This facility will be continuing to meet both enhanced space and enhanced ratios. This facility is currently meeting applicable minimum and enhanced requirements. The previous facility completed the Environment Rating Scales and earned 5.27 on the ITERS-R and a 5.36 on the ECERS-R. The average of these two scores was 5.32. Based on this information, this facility is eligible to earn 6 points in Program Standards. EDUCATION STANDARDS: Based on the staff education and experience from the previous facility, this facility is eligible to earn 2 points in Education Standards. QUALITY POINT: When asked, this facility continues to use Creative Curriculum as a developmentally appropriate curriculum listed under the Programmatic Options. 6+2+1 = 9 points A total of 9 points appears to earn this facility a Three Star License pending an administrative action. The complaint report was received March 18, 2024. Today’s visit was completed in conjunction with a second temporary time period visit. Two visit summaries were competed for each type of visit. Upon arrival, owner, L.Wood, was not present; however, the facility administrator, V.McLeod, was present and available for consultation. I asked that Ms. McLeod contact you via phone to notify her of my visit. Ms. McLeod contacted you and I spoke with you via phone today. Ms. Wood and Ms. McLeod were given the opportunity to state her perception of the allegations and offer any pertinent information regarding the allegations. Twenty-six children, ages zero through five-years, were present during today's visit. The center serves children ages zero through twelve-years during first shift child care. Overall enrollment and attendance were collected and documented with the temporary time period visit. Applicable child care requirements for a complaint visit were monitored today in addition to requirements for a second temporary time period visit. Children were observed playing with materials, listening to assigned staff read a book, coloring and making Easter cards, and interacting with staff. Upon arrival, the toddler aged children, two (2) children, were grouped together with children in Room 2, that were all two (2) years of age. The toddler aged children were taken to their assigned space in Room 1 and remained in Room 1 for the remainder of the visit. Children were also observed participating in bathroom and hand washing routines. The children in all groups were supervised by the staff members today. Findings - ALLEGATION ONE: There are concerns regarding supervision. You and all staff present today were interviewed and given an opportunity to share information regarding the allegation. Observations were also conducted in all four rooms to monitor supervision. I also observed staff walking with children to the bathroom and assisting them as needed with routines. Staff were positioned in areas where adequate supervision was maintained. Ms. Wood stated during our phone conversation that she was not aware of any incidents where any child(ren) left their assigned group unsupervised. Ms. Wood stated no one has discussed this with her, staff and/or parents. Ms. Wood stated she uses Team Reach, an app that allows staff and parents to communicate and send messages to one another. Ms. Wood stated Team Reach allows staff and or parents to send a private message to someone, if needed. Ms. Wood also stated she is present at the facility each day in the mornings from around 7:30AM to 8:15AM, prior to leaving to go to work. She further stated she has not observed a child not being supervised or a child leaving their assigned group unsupervised. When asked about a camera/video system, Ms. Wood stated she has the capability to watch live video footage but the program she uses does not have sound and does not record footage. I was not able to watch any live feed today, but Ms. Wood stated while she was working today, she could view each classroom and other areas of the center from her computer. During interviews today, it was reported to me that a one (1) year old child was found by a staff member along in the bathroom. A staff member reported appropriate staff were notified and the child was taken back to Room 1, where the child routinely is present. It was also reported that staff, names not provided, have left children that were two (2) years of age, unsupervised in Room 2 at various times. When I asked for specific dates and/or times, a staff member stated exact dates and times were not known and could not be provided. Based on a specific interview with a staff member, this allegation was determined to be substantiated. ALLEGATION TWO: There are concerns with nurture and care. Ms. Wood and applicable staff were interviewed today regarding concerns with nurture and care of the children. Ms. Wood stated no one (parent or staff member) has shared any concerns with her regarding inappropriate nurture and care of children. Time out and redirection are used with the children. The facility’s discipline policy is the sample discipline policy that can be found from the Division of Child Development and Early Education’s website under the “Provider Documents and Forms” tab. Parents of all children enrolled sign the facility’s discipline policy at enrollment. Signed discipline policies were observed on file today in the children’s records that were reviewed. Observations between the staff and children were completed today. Staff were observed interacting with the children on their level, assisting them with routines such as assisting with the bathroom and handwashing, and during meal times. Staff were seated at tables with children and assisting them with materials and coloring Easter cards during observations as well. Staff were observed talking to children during the visit and at no time were staff heard yelling at children. During today’s visit, I did not observe any staff inappropriately handling children such as snatching or grabbing a child. As reviewed above, the live feed video system does not record actual footage and no sound is heard per Ms. Wood. Based on this information, no video footage could be reviewed. During interviews conducted today, it was reported that children have been “yelled” at and have been snatched by their arms by staff. When asked about the ages of the children and names of staff that has been observed yelling or snatching children, a staff member provided the names of three current staff members. Observations were conducted specifically with the named staff today. I did not observe any children being yelled at or snatched by the three specific staff or any staff present during today’s visit. As interviews were conducted with other staff present, no one stated they have observed any staff yelling or snatching children. Based on a specific interview with a staff member, this allegation was determined to be substantiated. The violations documented are listed below: Violation Number Comment Rule 303 Children were not adequately supervised at all times. It was reported that a staff member found a one (1) year old alone in the bathroom located outside and to the right of Room 1. It was reported that staff left children, all two (2) years of age, unsupervised in Room 2. .1801(a)(1-5) 902 Each child was not attended to in a nurturing and appropriate manner, or in keeping with the child's developmental needs. It was reported that children were yelled at and were snatched by their arms by staff.
