Name of Operation: ANGIE'S DAY CARE Facility ID: 42000255 Consultant: MARQUITTA HOLLOMAN Operation Type: Family CC Home Case Number: Visit Date: 9/3/2025 Number Present: 6 Completed Date: 9/3/2025 Age: From 0 To 5 Total Minutes: 150 Time In: 09:30 AM Time Out: 12:00 PM Time In: Time Out: List to Use: Family CC Home Type Of Visit: Annual Comp Full Announced/Unannounced: Unannounced The purpose of my unannounced visit was to monitor this program for compliance with applicable child care requirements during an annual compliance visit. This family child care home currently operates with a Two Star License issued April 9, 2018. The last annual compliance visit was completed on October 8, 2024. Angela Shearin, owner/operator was present and available to assist. Six (6) children, ranging in ages zero (0) year to five (5) years, were present during my visit. The children were observed in free choice play, and toileting and handwashing routines.. The caregiver interacted with the children in a positive manner. This family child care home is licensed to serve (8) children. Children are enrolled on the first, shift. A walkthrough of the home was completed today. Fire drills, shelter in place/lock down drills, and playground inspections were all monitored and in compliance. You stated you do not provide transportation. I used the Annual Compliance Monitoring Checklist for Family Child Care Homes and the Family Child Care Home Item Number Listing (DCD 0357) as a basic monitoring tool to assess compliance with all applicable child care requirements pertinent to this facility. Please remember the entire home is licensed for child care and must meet the applicable child care requirements. Although you designated space to be used for child care in a specified section of your home, the license is issued for the entire location. Any monitoring visit conducted may include spaces other than those you designated for child care. A computer-generated visit summary was reviewed and left with you at the conclusion of the visit. The following violations of child care requirements were documented. Violation Number Comment Rule 508 Operator did not successfully complete a first aid course as referenced in Rule.1702(b)(2) First aid training was not renewed on or before the expiration of the certification. First aid training expired 8/21/2025 .1703(a)(2) 511 Operator did not successfully complete a CPR course as referenced in Rule.1702(b)(2) CPR training was not renewed on or before the expiration of the certification. .1703(a)(3) 2019 Operator did not complete Recognizing and Responding to Suspicions of Child Maltreatment training within the required time frame. Recognizing and Responding to Suspicions of Child Maltreatment training was not retaken within the required five year timeframe. .1703(a)(5) Child Care Licensing Requirements are established to ensure a safe, healthy, and developmentally appropriate environment for all child care facilities. Therefore, it is important for you to be knowledgeable of all licensing requirements that apply to your facility. Child Care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. It is your responsibility to maintain compliance with the applicable laws and rules; always whether they have been monitored or specifically discussed with you in the past. The best way for you to ensure that you are meeting all requirements is to periodically review the childcare laws and rules on the Division of Child Development and Early Education (DCDEE) website. Any violation(s) documented may impact the compliance history score. The violation(s) documented must be corrected immediately. On or before , I must receive by September 17, 2025 a written, dated, and signed compliance letter that describes accurately and in detail, how and when the violations were corrected. Please be aware any information submitted by you is accurate documentation. If any information provided in the letter is not accurate, this may be considered falsification of information. If sufficient information is not received by the due date, a follow-up visit will be conducted. OWNER STATUS/CONTACT INFORMATION You, A. Shearin, are listed as the Sole Proprietor on the Family Child Care Home’s License and you stated during today’s visit, this ownership remains the same. Prior to changing the facility name and/or ownership (i.e., forming a corporation), please contact me to discuss procedures. Failure to comply may affect your Family Child Care Home License. You verified the mailing address, phone number, and email address listed on the facility profile are correct. Please contact me if changes occur to ensure information is updated in the system. COMPLIANCE HISTORY Prior to today's visit, this facility has maintained an 18-month compliance score of 99%. TECHNICAL ASSISTANCE/ CONSULTATION REQUIRED TRAININGS CPR and First Aid training for the provider expires 8/21/2025. There was no documentation of updated training information. You stated you are registered to complete the CPR and First Aid training 9/23/2025. Documentation of the updated Recognizing and Responding to suspicions of Child Maltreatment training was not on file for review. The last training on file expired 7/14/2023. The training can be completed on the Prevent Child Abuse website at https://preventchildabusenc.org/resource-hub/recognizing-responding-to-child-maltreatment/. RESOURCES The Division of Child Development and Early Education's (DCDEE) website is https://ncchildcare.ncdhhs.gov/. Visit this website for the most up-to-date child care forms, resources related to child care, and new child care rules/laws. PROVIDER TRAINING Recognizing and Responding to Child Maltreatment training is required every five years to remain in compliance with child care rules. STAR RATED LICENSE Hold harmless has been extended until the new QRIS is implemented (SB 425). Providers in Cohort 1 are not required to go any further with a rated license assessment unless they want to, and providers in Cohort 2 do not need to start their preparation year unless they want to. This should be the providers’ choice only. We discussed the pathway options and I encouraged you to look at the differences on the pathways to determine which one may fit your program better.
