The allegation was discussed with Administrators, K. Huff and J. Stevens. Ms. Stevens provided the following information regarding an incident that occurred on 5/16/25. Ms. Stevens stated that on 5/16/25, H. Burris, Assistant Director, in charge of all infant/toddler classrooms came to the office and shared with her that a parent stated her child was fed another child's bottle. Ms. Stevens stated that the lead teacher for space #14, M. Russell, left for the day at 1:45pm. She contacted M. Russell to discuss the concerns of the parent. Ms. Stevens stated that M. Russell was not aware of a child being fed another child's bottle. Ms. Stevens then spoke with the other teacher present in the classroom, A. Laton, who was also unaware of a child being fed another child's bottle. Ms. Stevens contacted the child's parent to discuss the concerns. Ms. Stevens also contacted the parents of another child enrolled in classroom space #14 to confirm that child was sent home with the correct number of empty and full bottles. This parent confirmed that all bottles were accounted for. She thought that one of this child's bottles may have been fed to the child in question. Ms. Stevens could not confirm this to be accurate. Ms. Stevens showed me the information in the Procare app that the facility uses to track feedings, naps, bathroom routines, etc. Ms. Stevens stated that the child is still enrolled at the facility and the parent has not communicated anything further regarding this issue with administrative staff. Ms. Stevens stated that she has spoken with all staff who work in classroom spaces where bottle feeding occurs reminding them of the importance of ensuring that they are feeding bottles only to the child whose name is on the bottle. During the visit, I spoke with H. Burris, who stated that on 5/16/25, a parent of a child enrolled in space #14 approached her with concerns that her child was fed another child's bottle that day. H. Burris immediately went to the office to share the parent's concerns with J. Stevens. I interviewed M. Russell during today’s visit. She stated that on 5/16/25, prior to leaving work at 1:45pm, she fed the child in question two(2) of the three (3) bottles brought to child care that day. I interviewed A. Laton during today's visit. She stated that she fed the child a bottle that afternoon. A copy of the log from Procare for this child was received by me, Michell Kocan, Child Care Consultant, today. Based upon my review of the the documentation from the Procare app it was determined that the allegation that a child care requirement regarding nutrition for infants was not in compliance. This allegation has been confirmed. The following violations of child requirements were cited during the visit. Violation Number Comment Rule 1887 Each infant was not served only bottles labeled with their individual name. A child enrolled in space #14 was fed another child's bottle on 5/16/25. .0902(d) The violation documented must be corrected immediately. To verify correction of the violations you shall submit a signed letter to me explaining how you have corrected the violations described above. I met with Jenna Stevens, Director, and we conducted a walk-through of the facility. The program operates with a five-star license issued March 1, 2019. Restriction: Daytime care only; Meets enhanced space. This program operates two (2) NC Pre-K classrooms. The facility had a 93% compliance history prior to today's visit. The NC Secretary of State website was viewed before today’s visit and Richfield Child Development Center Inc. was active and current. There are 239 children currently enrolled and 130 were present during today's visit. The children were engaged in the following activities: free-play in activity areas, outdoor free-play, teacher-directed activities, bathroom/handwashing routines, lunch and nap. The staff and training worksheets were received prior to this visit. One (1) new staff members has been hired since the last monitoring visit was conducted. I monitored the file of the new staff and three (3) existing staff files for 10% of staff files. I monitored twenty-five (25) children's records during the visit for 10% of children's records. Five (5) of the twenty-five (25f children's records monitored were those for NC Pre-K children. The following items were posted for easy reference: license, Summary of NC Child Care Law and Rules, safe arrival and departure procedures, emergency medical care plan, daily schedules, current activity plans, first aid posters, menu, applicable staff/child ratio charts, tobacco free signage, and evacuation routes. The annual fire inspection was conducted 8/13/2024 and the facility was found to be satisfactory and approved for daytime care only. A sanitation inspection was conducted 3/26/25 with a score of 0 demerits for a Superior classification. The last documented fire drill was conducted on 3/19/25. A shelter-in-place drill was conducted on 3/31/25. The monthly playground inspections have been completed as required. NC Pre-K: This facility has two (2) NC Pre-K classrooms. The NC Pre-K program operates from 8:15a.m. until 3:15p.m. The facility uses the ‘Creative Curriculum’ for the NC Pre-K program, as well as the 'Brigance Early Childhood Screen' and the 'Teaching Strategies Gold' (TS Gold) developmental screening tools. The NC Pre-K Program Site Monitoring Tool was reviewed today and found to be complete and accurate. The initial 'Brigance Screen' was completed for each child within the required time frame. The TS Gold developmental screenings have been completed multiple times during the school year for all children. Parents participate in Family Engagement activities on-site that occur bi-monthly, monthly take home projects, and other activities. Newsletters are sent home monthly, and parents receive daily communication via the Pro Care app. Parent/teacher conferences are held three (3) times per year. Two (2) conferences have been held thus far for the school year. Progress reports are shared with parents during the parent/teacher conferences. The environment rating scale assessment was conducted November 30, 2018, and the program received a score of 5.86. I monitored one (1) van for compliance with transportation requirements. All transportation requirements were in compliance. The following items were monitored today: supervision, staff/child ratio, staff, health, safety, program records, management, and transportation. There was one (1) violation of child care requirements observed today regarding medication. Violation Number Comment Rule 843 A drug or medicine was administered after its expiration date. The facility operates with a five-star license. Upon arrival I spoke with Kelley Huff, Director, we discussed the purpose of today's visit. A walkthrough of the indoor spaces used by children was conducted and monitored for the following: supervision, discipline, staff/child ratios, group size, licensed capacity, and permit restrictions. I monitored staff records to ensure all staff have current CPR and First Aid certification, ITS-SIDS certification and that all criminal background qualifying letters are valid. The following items were posted for easy reference: license, NC Summary of Law, First Aid poster, evacuation routes, safe arrival and departure procedures, classroom schedules, activity plans, menu, staff/child ratio charts, emergency telephone numbers, tobacco free policy and emergency medical care plan. The 120 children present during today's visit were observed engaged in tummy time, napping, and completing bathroom/handwashing routines. The following violations of childcare requirements were observed during the visit . Violation Number Comment Rule 524 When children 3 years and older were in care, screen time was not used to stimulate a developmental domain; was not limited to 30 minutes a day and no more than a total of two and a half hours per week, per child; and/or was not documented on a cumulative log or the activity plan that is available for review. There is a concern that a staff member used inappropriate discipline. The allegation was discussed with the Administrator, K. Huff. Ms. Huff provided the following information regarding an incident that occurred on 7/26/24. Ms. Huff stated that she was informed by fellow administrator, J. Huff, that a staff member popped their own child who is enrolled in care at the facility on the back. Ms. Huff stated that the child was receiving care in space #10a/b when the incident occurred. She stated that the staff person reported having popped their child to fellow administrator, J. Huff. K. Huff stated during today’s visit that K. Beeker is still employed at the facility, but she arrives in the evenings when no children are present on the premises to perform housekeeping duties. I interviewed J. Huff during today’s visit. She stated that upon arrival for work on 7/26/24, K. Beeker, approached her and stated that while delivering breakfast to the classrooms that morning when she entered the classroom where her child was being cared for the child was ripping up a book and she popped the child on the back. J. Huff stated that she immediately told K. Beeker to take a break. J. Huff shared the information she received with K. Huff, owner/administrator. K. Beeker was later sent home for the remainder of the day. J. Huff stated that K. Beeker is still employed at the facility and performs housekeeping duties in the evenings after the facility has closed. I interviewed staff member H. Medlin, who was caring for the group of children in space 10a/b at the time of the incident. She stated that on 7/26/24, she was assisting the group of two and three-year-old children in her care clean up toys in preparation for breakfast when she heard a “smacking” sound.
