This visit was conducted with Linda Bullock, Lead Teacher. Currently this center operates with a Five Star license. Restrictions include daytime care only and meets all enhanced requirements and reduced ratio. This facility is owned and operated by Rockingham County Schools. The sanitation inspection was completed January 22, 2026. The facility received a superior rating with no demerits. The last fire inspection on file was completed on September 22, 2025. A copy was received during a previous visit. Upon arrival staff/child ratio was observed and in compliance as indicated on the attached enrollment worksheet. Children were adequately supervised during today’s visit. All indoor and outdoor areas were observed during today's visit. Sixteen (16) children were present during today’s visit. The children were observed playing indoors, completing routine tasks and playing outdoors. Fire drill reports and playground inspections were monitored during today's visit. The last fire drill was completed April 1, 2026. It took the children one (1) minute and twenty (20) seconds to evacuate. The last emergency drill, a shelter in place drill, was completed on March 10, 2026. Emergency drills must be conducted every three (3) months. Children’s and staff files were reviewed according to the policy set by the Division of Child Development and Early Education. Ms. Bullock stated that a child enrolled has emergency medication. They do not provide routine transportation. The NC Pre-K requirements were monitored today. The facility currently has eleven (11) NC Pre-K children. The following violation was observed during today's visit. Violation Number Comment Rule 1805 A child care operator did not notify the Division of any new child care providers, as defined in G.S. 110-90.2(a)(2), who were hired or moved into the child care facility within five business days. Three (3) staff are not listed on the staff roster in the ABCMS system. G.S. 110-90.2 & .2703(r) Rated License Information: The facility must submit the application, staff/child ratio sheet, and program assessment form to me with the compliance letter. The facility meets the standards for Pathway 3, Accreditation/Head Start. Once it is determined that the violation has been corrected a Five Star Rated license will be issued. Consultation: Due to the facility serving NC Pre-K children to meet NC Pre-K requirements the teacher assistant must complete fifteen (15) ongoing training hours each year. Compliance: You must correct the violation found during today's visit immediately. Please send me two (2) letters verifying compliance. The letters must be received by April 29, 2026. This visit was conducted by Sally Allen, Child Care Consultant with Linda Bullock, Lead Teacher. This is the first annual compliance visit to the facility. Currently this center operates with a Five (5) Star license. Restrictions include daytime care only and meets all enhanced requirements and reduced ratios. Rockingham County Public Schools is the operator of the facility. The sanitation inspection was completed February 8, 2024, with a "Superior" classification with no demerits. The fire inspection was completed on September 15, 2023. I received a copy during another visit. Upon arrival staff/child ratio was observed and in compliance as indicated on the attached enrollment worksheet. Children were adequately supervised during today’s visit. All indoor areas were observed during today's visit. Due to inclement weather the outdoor area was not observed today. Eighteen (18) children were present during today’s visit. The children were observed playing indoors, completing routine tasks, and interacting with teachers. Fire drill reports and playground inspections were monitored during today's visit. The last fire drill was completed April 30, 2024. It took the children one (1) minute to evacuate. The last emergency drill, a shelter-in-place, was conducted March 7, 2024. Children’s files were reviewed following DCDEE procedures. The staff files were reviewed during a temporary time period visit conducted December 4, 2023. Ms. Bullock stated that children enrolled have emergency medication. Hazardous supplies were kept in locked storage. Ms. Bullock stated that they do not provide routine transportation but have participated in off premise activities. They use the public school activity bus. There are seven (7) NC Pre-K children enrolled. The NC Pre-K requirements were monitored today. The following violation was observed during today's visit. Violation Number Comment Rule 476 Staff did not use the list of participating children to check attendance when leaving the center, periodically when the children were involved in the activity, before leaving the activity to return to the center, and/or upon return to the center. Attendance was not taken during the required times at the off premise activity that occurred on April 18, 2024. .1005(b)(6) Consultation: 1-The health and safety trainings are required to be completed within the first year of employment, with the exception of Recognizing and Responding to Suspicions of Child Maltreatment, CPR and First Aid training. These three (3) trainings must be completed within ninety (90) days of employment. CPR must be appropriate for the ages of children in care. Following the initial completion of the health and safety trainings they must be completed within the next five years of employment or prior to expiration (CPR and First Aid). There is a training log available on the Division’s website to document the completion of the trainings. All topics must be covered. 2-10A NCAC 09 .3013 (a) All teacher assistants shall have a high school diploma or its equivalent and shall either: (1) hold a Child Development Associate (CDA) credential; or (2) hold or be working toward an Associate Degree or higher in birth-throughkindergarten, child development, early childhood education, or an early childhood education related field. Teacher assistants working toward the Associate Degree shall make progress by completing a minimum of six semester hours per year. (b) All teacher assistants shall complete a minimum of 15 hours of annual in-service professional development. A combination of college coursework, Continuing Education Units (CEUs), or clock hours may be used to complete the requirements of this Rule. The Temporary License was issued August 31, 2023, and this was the second visit conducted. This visit was conducted by Sally Allen, Child Care Consultant with Linda Bullock, Lead teacher. This is a one (1) classroom head start program located in an elementary school. All indoor areas were monitored for meeting all minimum licensing requirements. Staff/child ratios were observed and were in compliance. Children were adequately supervised during today’s visit. There were materials and equipment arranged in activity areas. The required posted items, such as, activity plans and daily schedules were observed. Twelve (12) children were present today. The children were observed playing outdoors, completing routine tasks, listening to a story as they wait and eating lunch. First aid kit, medication and hazardous supply storage were monitored. An epi-pen expired November 2023. The outdoor learning environment was monitored for all pertinent safety and equipment related requirements. The black plastic border was flush to the fence making the fence height less than four feet. The border has been moved away from the fence. I reviewed two (2) NC Pre-K children’s files and all staff files. The staff have a current qualifying letter. All staff today have current CPR and First Aid trainings. NC Pre-K The NC Pre-K classroom has seven (7) NC Pre-K children enrolled. There was a DIAL assessment on file for the children. For the two (2) files reviewed there was a health assessment on file. The facility uses Creative Curriculum and Teaching Strategies for the ongoing assessments. Attendance is taken daily. If children miss more than three days, they contact the parent and document absences and reasons. Family engagement is met by conducting home visits, having parent/teacher conferences, having parent volunteer opportunities, having parent meetings, and providing community resources to the parents. There was a fire drill listed that occurred on November 9, 2023. It was recorded that it took the children forty-five (45) seconds to evacuate. The emergency drill, a shelter in place, was completed on October 24, 2023. The fire inspection was completed on September 15, 2023. The facility is approved for daytime care only. A sanitation inspection was completed on September 28, 2023. The facility received a “superior” rating with no demerits. I cited the following violations: Violation Number Comment Rule 843 A drug or medicine was administered after its expiration date. An epi-pen expired November 2023. 10A NCAC 09 .0803(1)(d) 1232 Each employee's personnel file did not contain an annual staff evaluation and a staff development plan.
