This visit was conducted with Jennifer Lee-Stacey, Program Coordinator. Currently this center operates with a Three (3) star license. Restrictions include daytime care only, school age children only, reduced group size by one in each group from the seven point level as required in Rule .2818(c), meets enhanced space and children are not allowed to play on stationary equipment. Rockingham County Schools operates this program. Staff/child ratio was observed and in compliance as indicated on the attached enrollment worksheet. Children were adequately supervised. Indoor and outdoor areas were observed during today's visit. Ms. Lee-Stacey stated that no children enrolled have emergency medication. Twenty-four (24) children were present during today’s visit. The children were playing indoors and outdoors. Fire drill reports and playground inspections were monitored during today's visit. The last fire drill was conducted on May 12, 2026. It took the children one minute and nine seconds to evacuate. The last emergency drill, a shelter in place drill was conducted on March 16, 2026. Staff and children’s files were reviewed today following the Division of Child Development and Early Education procedures. They do not provide routine transportation but they do participate in field trips. The following violations were observed during today's visit. Violation Number Comment Rule 1048 All staff did not successfully complete certification in First Aid appropriate to the age of children in care. Verification of staff completion of First Aid training from an approved training organization was not in the staff file. A new staff, hired February 2026, has a First Aid training through an unapproved organization. .1102(c) 1049 All staff did not successfully complete certification in CPR training appropriate to the age of the children in care. Verification of staff completion of the CPR course from an approved training organization was not in the staff file. A staff member, hired in February 2026, has a CPR training from an unapproved organization. .1102(d) 1805 A child care operator did not notify the Division of any new child care providers, as defined in G.S. 110-90.2(a)(2), who were hired or moved into the child care facility within five business days. Two staff, one hired in January and another hired in February, are not on the staff roster. G.S. 110-90.2 & .2703(r) Rated License Information: The facility has chosen Pathway 2, Classroom and Instructional Quality Pathway. We reviewed the CQI for the facility and the staff today. Ms. Lee-Stacey stated that she will begin working on these forms. They are planning to apply in the fall. Consultation: 1-For all new staff they have the first year of employment to complete the health and safety trainings, with the exception of CPR, First Aid and recognizing and responding to suspicions of child maltreatment, which must be completed within ninety (90) days of employment. For all current staff, the health and safety trainings topics must be completed within five years. There is a training log that the staff can use to document the year that the health and safety training has been completed. Compliance: You must correct the violation found during today's visit immediately. You can mail two (2) letters or email the letter verifying compliance by July 1, 2026. Please include in your letter each violation number and how you corrected each violation and what plan will be implemented to prevent these violations from occurring again. This visit was conducted with Jennifer Stacey, Program Coordinator. Currently this center operates with a Three (3) Star license. The restrictions include daytime care only, school age children only, reduced group size by one in each group from the seven point level as required in Rule .2818(c), enhanced space and children are not allowed to play on stationary equipment. The sanitation inspection was completed on April 24, 2025, with a “superior” classification with two (2) demerits. The fire inspection was completed on September 30, 2024. I received the fire inspection today. The facility is owned and operated by Rockingham County Schools. Twenty-one (21) children were present. The children were observed playing indoors, completing routine tasks, eating morning snack and participating in a fire drill. All indoor and outdoor areas were monitored. Fire drill reports and playground inspections were monitored during today's visit. The last fire drill was conducted on July 8, 2025. It took the children one (1) minute and thirty (30) seconds to evacuate. The last emergency drill, a shelter in place drill, was conducted on March 31, 2025. The emergency drills must be conducted every three (3) months. The staff and children’s files were monitored in accordance with the Division of Child Development and Early Education policies and procedures. Ms. Stacey stated that no children enrolled have emergency medication. The facility does provide transportation for off premise activities. They use the public school activity buses. The following violations were observed during today's visit. Violation Number Comment Rule 1043 All staff records, except financial records, were not made available for review. A new employee, hired May 19, 2025, was not included on the public school staff record off site verification form. G.S. 110-91( 9) 1449 Within the first two weeks of assuming responsibilty for supervising a group of children, staff did not complete at least 6 hours of training on topics outlined in this rule. There was not a record of orientation being completed. .2510(i)(1)(A-D) 1811 Shelter-in-place or lockdown drills were not practiced every three months and/or drill record was incomplete. The last emergency drill was completed March 31, 2025. A drill should have been completed in June. .0604(u);.0302(d)(8) Compliance letter: You must correct the violation found during today's visit immediately. This visit was conducted by Sally Allen, Child Care Consultant, with Leah Ward, Program Specialist. The allegations are as follows: There are concerns regarding supervision. There are concerns regarding safe environment. During today’s visit, the allegations were discussed with you. You had the opportunity to respond. Information shared during this visit are documented in this visit summary. Your program currently operates with a Three Star License. Restrictions include daytime care only, meets enhanced space, school age children only, and reduced in each group from the seven point level as required in Rule .2818(c) and children are not allowed to play on stationary equipment. Today, the following items, in addition to the allegations, were monitored: • Supervision • Staff / Child Ratio • Adequate / Approved Space • License Posted • Permit Restrictions Allegation #1: There are concerns regarding supervision. Observation and/or Interviews #1: I interviewed the program specialist, Leah Ward; the program coordinator, Jennifer Stacey and three (3) group leaders today. I also reviewed video of the incident that occurred on January 23, 2025. On the video two (2) children were seen pushing each other. After several seconds of pushing they stopped and began playing. They began throwing balls. Then the same two (2) children were seen fighting each other. During both interactions a group leader is seen sitting on the stage. She does not get up or intervene. Another group leader is seen coming into the gym. She walks over to the group leader sitting on the stage and talks to her. She also sits down on the stage with the first group leader. After she sits, she looks over and sees the two (2) children fighting. The children had now been engaged in this behavior for four to five minutes. The group leader gets up and tries to break up the children fighting. She falls down trying to intervene. The other group leader sitting on the stage gets up after the group leader falls down. During the interviews it was stated that one of the group leaders calls on the walkie talkie to get assistance. On the video the program specialist is seen coming into the gym and breaking up the two (2) children fighting. In the interviews it is said that the program coordinator took the two (2) children into the office to calm down. A different group leader was in the office with her as she spoke to the two (2) children. Conclusion #1: Based on my observation and information gathered during the interviews, the allegation regarding supervision is substantiated. Allegation #2: There are concerns regarding safe environment. Observation and/or interviews #2: I interviewed the program specialist, program coordinator, and three (3) group leaders. A video of an incident that occurred on January 23, 2025, was reviewed. Conclusions #2: There was not enough information to confirm the allegation. The following violation was cited during today’s visit. Violation Number Comment Rule 303 Children were not adequately supervised at all times. It was observed that during an incident on January 23, 2025 staff did not interact with the children while moving about the indoor area nor were they aware of the children's activities as two (2) children were fighting. .1801(a)(1-5) Comments Section – Child Care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. Any violation(s) documented may impact the compliance history score. There are concerns regarding inappropriate discipline. During today’s visit, the allegations were discussed with you. You had the opportunity to respond. Today, the following items, in addition to the allegations, were monitored: • Supervision • Staff / Child Ratio • Adequate / Approved Space • License Posted • Permit Restrictions Allegation #1: There are concerns regarding supervision. Observation and/or Interviews #1: I interviewed the interim principal, Ms. Casto; the program specialist, Leah Ward; and the program coordinator, Jennifer Stacey today. I also reviewed video of the incident that occurred on December 9, 2024. As seen on the video the children were coming inside for outdoor play. The program coordinator was standing in the doorway as two (2) children were observed running to the outdoor area to retrieve balls. Another child was seen going to the bench and picking up a jacket or sweatshirt. He was seen throwing the article of clothing back down on the bench. The program coordinator is seen speaking to the child at the bench. There is no audio. At 4:01pm the child sits on the bench. There are no other people outdoors with the child. The program coordinator shuts the door and walks into the cafeteria with her back to the door leading to the outdoor. At 4:02pm the child walks to the door from outside and the program coordinator walks to the door from the inside. Before opening the door, the program coordinator says something to the child. Again, there is no audio. She stated that she asked the child if he was ready to come inside. The child turns his back to the door and grabs his head. The program coordinator opens the door while the child’s back is to the door. The child walks away from the door toward the bench and the program coordinator closes the door again and walks away to talk to a group leader in the cafeteria. At 4:03 pm the child runs toward the door and the program coordinator lets him inside. The door stays locked at all times. Conclusion #1: Based on my observation and information gathered during the interviews, the allegation regarding supervision is substantiated. Allegation #2: There are concerns regarding inappropriate discipline. Observation and/or interviews #2: The program coordinator was observed leaving the child unattended outside. When the child came back to the door to be let inside the program coordinator is seen saying something to him. There is no audio to know what is said to the child. It is stated that she asked him if he was ready to come inside. When he turned away he grabbed his hair and then began to walk away. The program coordinator closed the door again and turned with her back to the door, leaving him unattended again. The program coordinator asked a group leader to contact the principal for help with the child. Conclusions #2: Based on observations and interviews, the allegation regarding inappropriate discipline is unsubstantiated. The following violation was cited during today’s visit.
