This visit was conducted with Jessica Warren, Lead Teacher. Currently this center operates with a Notice of Compliance. The restrictions include daytime care only, children 2 ½ years in rooms with a direct exit and uses corporal punishment. The facility is owned and operated by The Increase Learning Center Inc. The Secretary of State website was checked and the corporation is listed as current and active. Twelve (12) children were present. The children were observed completing routine tasks, eating snack and playing indoors. Meals are provided by the parents. All indoor and outdoor areas were monitored. Fire drill reports were monitored during today's visit. The last fire drill was conducted on April 21, 2026. It took the children one (1) minute to evacuate. The last emergency drill, a lockdown drill, was conducted on February 26, 2026. The emergency drills must be conducted every three (3) months. The staff and children’s files were monitored in accordance with the Division of Child Development and Early Education policies and procedures. Ms. Warren’s criminal background check expired in March 2026. I checked the ABCMS system and there is not a current one listed in the system. Ms. Warren has fifteen (15) days to complete and receive a qualified criminal background check. If she does not receive one on the fifteenth day, she cannot return to the facility until she has received a qualifying background check. The lead teachers stated that no children enrolled have emergency medication. The facility does not provide routine transportation. The following violations were observed during today's visit. Violation Number Comment Rule 1044 Prior to the expiration date of the qualification letter, the child care provider did not complete and submit required forms to complete a criminal background check (a qualification letter is valid for a maximum of five years for the date of issuance). A staff member's qualification letter expired March 2026. G.S. 110-90.2(b) & .2703(n)&(o) 1419 All commercially manufactured equipment and furnishings were not assembled and installed according to procedures specified by the manufacturer and/or the manufacturer's instructions were not kept on file or electronically accessible, if available. In space 2, there were four (4) children sitting in high chairs. None of the children were buckled into the chairs . .0601(b) Compliance letter: You must correct the violation found during today's visit immediately. Please send me two (2) letters verifying compliance. The letters must be received by May 27, 2026. Please include in your letter each violation number and how you corrected each violation and what plan will be implemented to prevent these violations from occurring again. This visit was conducted with Jessica Warren, Lead Teacher. Currently this center operates with a Notice of Compliance. The restrictions include daytime care only, children 2 ½ years in rooms with a direct exit, and uses corporal punishment. This facility is owned and operated by The Increase Learning Inc. The Secretary of State website was checked and the corporation is listed as current and active. Sixteen (16) children were present during today’s visit. I observed the children arriving, playing indoors, completing routine tasks and finishing morning snack. The facility had a fire inspection completed on November 5, 2025. I received a copy today. The sanitation inspection was completed on February 28, 2025. The facility received a superior rating with no demerits. The emergency drill log was monitored during today's visit. The last fire drill recorded was completed on October 30, 2025. The evacuation took one (1) minute. The last emergency drill, a shelter in place drill, was conducted on October 30, 2025. There are no new staff. The staff have current qualifying letters. They do not provide routine transportation for children enrolled this school year. The following violations were cited today. Violation Number Comment Rule 540 An individual written feeding plan was not provided by child's parent or health care provider or was not followed and posted. (omit posting for centers located in a residence) There was not a feeding plan for a fourteen-month-old child. 10A NCAC 09 .0902(a) 840 All corrosive agents, pesticides, bleaches, detergents, cleansers, polishes, any product which is under pressure in an aerosol dispenser, and any substance which may be hazardous to a child if ingested, inhaled, or handled were not stored in a locked room or cabinet. A hand sanitizer that must be kept out of reach of children was left on a table within reach of children. .2820(b) Technical Assistance: I left a copy of the feeding plan with Ms. Kellum today. Any child less than fifteen (15) months must have a feeding plan posted in the classrooms. Compliance: You must correct the violation found during today's visit immediately. Please send me a two (2) letters verifying compliance. The letters must be received by December 8, 2025. Please include in your letter each violation number and how you corrected each violation and what plan will be implemented to prevent these violations from occurring again. This visit was conducted with Wanda Kellum, Lead Teacher. Currently this center operates with a Notice of Compliance. The restrictions include daytime care only, children 2 ½ years in rooms with a direct exit, and uses corporal punishment. The sanitation inspection was completed on February 28, 2025, with a “superior” classification with no demerits. The fire inspection was completed on October 10, 2024. I received a copy during a previous visit. The facility is owned and operated by The Increase Learning Inc. The Secretary of State website was checked prior to today’s visit and the corporation was listed as current and active. Fourteen (14) children were present. The children were observed finishing a morning snack, completing routine tasks and participating in group time. All meals are provided by parents/guardians. All indoor and outdoor areas were monitored. Fire drill reports and playground inspections were monitored during today's visit. The last fire drill was conducted on May 6, 2025. It took the children one (1) minute to evacuate. The last emergency drill, a shelter in place drill, was conducted on April 30, 2025. The emergency drills must be conducted every three (3) months. The staff and children’s files were monitored in accordance with the Division of Child Development and Early Education policies and procedures. The teachers in both classrooms stated that no children enrolled have emergency medication. The facility does not provide transportation. The following violation was observed during today's visit. Violation Number Comment Rule 721 All equipment and furnishings were not in good repair. The top pole of the fence, in the far corner near the bush, has come loose from the post. G.S. 110-91(6); .0601(b) Consultation: On the outdoor area I observed four mushrooms varying in size. I could not determine what type of mushroom. They were not disturbed. Please create a plan for how these mushrooms will be identified and treated, if needed. Please submit the plan to me with the compliance letter. Compliance letter: You must correct the violation found during today's visit immediately. Please send me two (2) letters verifying compliance. The letters must be received by June 5, 2025.
