This visit was also used to monitor compliance with applicable childcare requirements located in Child Care Rule Section .3000. This visit was conducted with Julie Matejko, Lead Teacher. Your program currently operates with a Five (5) Star License. Restrictions include daytime care only, meets all enhanced requirements and reduced ratios and Certified Developmental Day. The license was observed, and the restrictions were found in compliance. The facility is owned and operated by Rockingham County Schools. Indoor and outdoor areas were observed. Staff/child ratio and space capacity were monitored as indicated on the attached worksheet. Supervision was adequate during the visit. I observed staff/child interactions today. I observed children playing indoors, participating in group time, completing routine tasks and eating lunch. I observed materials and activities that were available to the children in the classrooms. The last annual compliance visit was conducted on May 7, 2025. A sanitation inspection was completed March 4, 2026 with a superior classification. The last fire inspection was conducted September 22, 2025. Program records and required postings were monitored. A fire drill was conducted on April 4, 2026. A shelter in place drill was documented on March 13, 2026. Staff files and children’s records were monitored per DCDEE procedures. Nutrition requirements were monitored per DCDEE procedures. Storage of hazardous items was monitored today. No medications were observed nor reported. You stated that you provide routine transportation. I observed the off-site transportation and records verification forms. The developmental day requirements and NC Pre-K requirements were monitored for compliance. The following violation was cited during today’s visit: Violation Number Comment Rule 705 Equipment and furnishings were not sturdy, stable and free of hazards. The blinds on the door were bent in toward the classroom creating a protrusion at the child's eye level. .0601(c) Child Care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. Any violation(s) documented may impact the compliance history score. You must correct the violations cited during today's visit immediately and send me documentation verifying compliance on or before May 6, 2026. The following information must be included in your signed and dated compliance letter: • the name of your center • facility ID number • date • title of the person who signs the letter • each violation number(s) • describes accurately and in detail how and when you corrected each violation You may email the letter to sally.allen@dhhs.nc.gov or mail two copies of the letter to: Sally Allen P.O. This visit was conducted with Julie Matejko, Lead Teacher. Currently this center operates with a Five Star license. Restrictions include daytime care only, meets all enhanced requirements and reduced ratio, Certified Developmental Day. This facility is owned and operated by Rockingham County Schools. The sanitation inspection was completed February 6, 2025. The facility received a superior rating with two (2) demerits. The fire inspection was completed on September 4, 2024. A copy was received during a routine unannounced visit. Upon arrival staff/child ratio was observed and in compliance as indicated on the attached enrollment worksheet. Children were adequately supervised during today’s visit. All indoor and outdoor areas were observed during today's visit. Twelve (12) children were present during today’s visit. The children were observed playing indoors, completing routine tasks and participating in group time. Fire drill reports and playground inspections were monitored during today's visit. The last fire drill was completed April 2, 2025. It took the children twenty-three (23) seconds to evacuate. The last emergency drill, a lockdown drill, was completed on April 4, 2025. Emergency drills must be conducted every three (3) months. Children’s and staff files were reviewed according to the policy set by the Division of Child Development and Early Education. Ms. Matejko stated that a child enrolled has emergency medication. Ms. Matejko stated that they do provide transportation. They use the public school bus. The NC Pre-K and Developmental Day requirements were monitored today. The facility currently has eleven (11) NC Pre-K children. The following violations were observed during today's visit. Violation Number Comment Rule 841 Medications including prescription and non-prescription items were not stored in a locked cabinet or other locked container. There was a bottle of Benadryl unlocked. 15A NCAC 18A .2820(d) 1757 A valid qualification letter was not on file and available to review at the facility. The therapist working with children alone did not have a letter on file. She is qualified but did not have the letter on file. G.S. 110-90.2(b) & (d) & .2703(e) Consultation: 1-The health and safety trainings are required to be completed within the first year of employment, with the exception of Recognizing and Responding to Suspicions of Child Maltreatment, CPR and First Aid training. These three (3) trainings must be completed within ninety (90) days of employment. Following the initial completion of the health and safety trainings they must be completed within the next five years of employment or prior to expiration (CPR and First Aid). There is a training log available on the Division’s website to document the completion of the trainings. All topics must be covered. The Recognizing and Responding to Suspicions of Child Maltreatment training can be found at https://www.preventchildabusenc.org/. 2-Child Care administrators can now view and edit their facility’s staff roster in ABCMS, the DCDEE’s criminal background check system. Provider Access to ABCMS allows administrators to: •See the real-time background check status of staff members •Run a printable report of the staff roster to assist with compliance visits •See new background check applicants and add to staff roster To get started, complete the ABCMS Child Care Provider Portal Training in Moodle. The process of notifying the Division has changed and is now captured in ABCMS. This change has been in effect since February 2024. Effective immediately, you will need to obtain a Business NCID and complete Provider Portal training in Moodle at https://www.dcdee.moodle.nc.gov/course/view.php?id=119. No action is needed on your part if you have completed the reference training and are currently using the ABCMS Provider Portal to update information regarding new hires or residents. Once the training has been completed and access has been given, you must verify your facility roster to ensure current staff are noted on the roster. This information should be updated in ABCMS in an ongoing basis as staff members are hired and when their employment is terminated. This satisfies the requirement to notify the Division of new child care providers working who were hired or moved into the child care facility within five business days. The compliance of this rule will be monitored during your next visit. Please note, the hard copy of the Change of Information form will no longer be needed or accepted. This visit was conducted by Sally Allen, Child Care Consultant, with Julie Matejko, Lead Teacher. Currently this center operates with a Five Star license, issued August 7, 2023, earning seven (7) points in staff education, seven (7) points in program standards and one (1) quality point. Restrictions include daytime care only, meets all enhanced requirements and reduced ratios and Certified Developmental Day. This facility is owned and operated by Rockingham County Schools. The last annual compliance visit was conducted September 28, 2022. The facility closes for summer. The sanitation inspection was completed June 2, 2023, with a "Superior" classification with no demerits. The fire inspection was completed on September 13, 2023. The facility is approved for daytime care only. I received a copy today. Upon arrival staff/child ratio was observed and in compliance as indicated on the attached enrollment worksheet. Children were adequately supervised during today’s visit. All indoor and outdoor areas were observed during today's visit. Eleven (11) children were present during today’s visit. The children were observed arriving, playing in activity areas indoors, completing routine tasks and participating in group time activity. There is a new arrival policy for the school. The parents walk the children to the main school entrance where the teachers wait to accept the children. The teachers wait until there are six (6) children and then one teacher walks those children to the classroom while the other teachers wait for the other children to arrive. If children are late arriving then the parents walk the children to the classroom. The teachers stated that at departure time they walk the children back to the entrance and wait for the parents to arrive. Fire drill reports and playground inspections were monitored during today's visit. A fire drill was conducted today. It took the children twenty-four (24) seconds to evacuate. The last emergency drill, a shelter in place drill, was completed on June 2, 2023. The next drill will need to be conducted in this month. Children’s and staff files were reviewed according to the policy set by the Division of Child Development and Early Education. Hazardous supplies were kept in locked storage. Ms. Matejko stated that no children enrolled have emergency medications. Ms. Matejko stated that they do not provide routine transportation. The NC Pre-K requirements were monitored today. The facility currently has eleven (11) NC Pre-K children. The requirements for Developmental Day were monitored and found in compliance today. The following violation was observed during today's visit. Violation Number Comment Rule 721 All equipment and furnishings were not in good repair. The stationary piece of equipment has cracks in the surfacing on the platform base. G.S. 110-91(6); .0601(b) Technical assistance and other topics discussed during today's visit: 1-The health and safety trainings are required to be completed within the first year of employment for all new staff and then they must begin their ongoing training hours annually. The health and safety trainings are also required to be completed within the next five years of employment/completion of trainings previously. There is a training log available on the Division’s website to document the completion of the trainings. All topics must be covered. 2-There is one (1) new staff that began employment on August 17, 2023. She has until November 2023 to complete the Recognizing and Responding to Suspicions of Child Maltreatment training. You can access this training at the website www.preventchildabusenc.org. Compliance: You must correct the violation found during today's visit immediately. Please send me two (2) letters verifying compliance. The letters must be received by October 3, 2023. Please include in your letter each violation number and how you corrected each violation and what plan will be implemented to prevent these violations from occurring again. Please include the name of your center and ID number, date, and the title of the person who signs the letter. Please send the letter to: Sally Allen, PO Box 1305, Eden, NC 27289. If you state in your letter that corrections have been made when they have not, it will be considered falsification of information.
