Lydia Neyer, operator, assisted me with the visit. The facility was issued a temporary license on January 30, 2026. The restriction on the license is daytime care only. During today’s visit, the following items were monitored: •Supervision •Capacity •Adequate Approved/Space •Storage of Hazardous Substances •Storage of Medication •General License Requirements •Staff Records (Not Applicable- No New Staff) •Program Records •License Posted •Applicable Rated License Requirements Children in care were engaged in free choice play after returning inside from outdoor play. Children were served lunch during the visit that consisted of broccoli and chicken casserole with a breaded topping, apple slices and goldfish crackers. Restroom, diapering, and handwashing were completed for all children and children were laid down for rest. The following violations were documented during today’s visit: Violation Number Comment Rule 714 Monthly check for hazards on the outdoor play area was not completed using a form supplied by the Division. Outdoor checks have not been documented for the months of April or May 2026. 10A NCAC .1721(e)(5)(A-F) 921 Operator did not maintain accurate daily attendance records including documentation of arrival and departure for all children in care, including the operator's own preschool children. Daily arrival and departure times have not been documented daily for children in care for the months of April and May 2026. .1721(e)(6) 1853 The operator did not conduct a monthly fire drill. Fire drills have not been conducted for the months of April or May 2026. .1719(a)(15) & .1721( e)(2) 1948 Sleeping infants, ages 12 months and younger were not visually checked every 15 minutes. Safe sleep checks have not been documented as required each day, every fifteen minutes for one infant. .1724(a)(7) Technical Assistance Provided During Today’s Visit: Completing fire drills and outdoor inspections and documenting each of these shows you are in compliance with ensuring a safe environment for children in your care and are teaching the children during fire drills what to do in an emergency. You should write a note on your calendar, or set an alarm notification to remind you to complete fire drills and outdoor checks monthly. The infant enrolled in the family child care home is your own child and I understand that this makes it difficult to remember to complete safe sleep checks every fifteen minutes. Even though you are checking on your infant and using a monitor, you must show compliance with safe sleep safety by documenting the checks just as you would for any other enrolled infant in your care. It is necessary that be able to account for children each day by having an accurate attendance and documentation of their arrival and departure times. If parents do not complete the sign-in and out form with times, you may do this to ensure it is complete. All violations must be corrected immediately. You must submit documentation no later than June 10, 2026, to Ms. Childress at tammy.childress@dhhs.nc.gov. The documentation should contain the following information: -Date you are submitting the documentation -Name of the Facility and ID# -Date of Today’s Visit -Each Item# of the violation(s) Cited -Date when each Item# was Corrected -Description of how each Item# was corrected -Sign the documentation Rated License Assessment Review: You did not wish to submit an Application for Assessment for a Rated License for a FCCH and have chosen to be issued a One Star License once your Temporary License expires on July 30, 2026. We discussed options for increasing your star rating level in the future, including completing early childhood coursework and using the T.E.A.C.H. Early Childhood Project for scholarship assistance. To find out more about T.E.A.C.H. Early Childhood Project visit the website for Child Care Services Association at www.childcareservices.org or contact Child Care Resource Center at (336)345-4900 (childcareresourcecenter.org) for training. Capacity Options: You have 5 children currently enrolled, ages 7 months to 4 years old and are licensed for a capacity of 8 children. While North Carolina General Statue 110-1(7)(b) now allows a capacity of 10 children as listed below, your zoning letter from Rockingham County only allows for 8 children to be in care (5 children ages 0-5 and 3 school age children). I contacted zoning prior to today’s visit to confirm that they allow 8 children to be in care rather than 10 allowed by statute. I left a message for someone to return my call. If I am able to speak with anyone confirming that 10 children can be in care, I will notify you and we can change your capacity to 10 children on your final star rated license. Consultation Provided During Today’s Visit: Criminal Background Check Provider Portal •As stated in G.S. 110-90.2 & .2703(r) child care operators are to notify the Division of any new child care providers working who were hired or moved into the child care facility within five business days. For Family Child Homes, this includes the hiring of yourself and also includes adding household members over the age of fifteen years old into your facility roster, which is discussed below. •The process of notifying the Division has changed and is now captured in ABCMS. This change has been in effect since February 2024. Effective immediately, you will need to obtain a Business NCID and complete Provider Portal training in Moodle at https://www.dcdee.moodle.nc.gov/course/view.php?id=119. No action is needed on your part if you have completed the reference training and are currently using the ABCMS Provider Portal to update information regarding new hires or residents. •Once the training has been completed and access has been given, you must verify your facility roster to ensure current staff are noted on the roster. This information should be updated in ABCMS on an ongoing basis as staff members are hired and when their employment is terminated. This satisfies the requirement to notify the Division of new child care providers working who were hired or moved into the child care facility within five business days. The compliance of this rule will be monitored during your next visit. Please note, the hard copy of the Change of Information form will no longer be needed or accepted. The compliance history is calculated using the percentage of points you earn during visits conducted over an 18-month time period. It is important that you maintain compliance history no lower than 75%. Failure to maintain the minimal 75% compliance history will affect your star rating and result in an Administrative Action. Emergency Preparedness and Response Plan: You must complete the Emergency Preparedness and Response for Childcare training within one year of licensure (by January 30, 2027) and within four months of completing the training you must develop and Emergency Preparedness and Response Plan for your program. You must print the plan from the template provided to you at the training, and review the plan annually. You will print off the first page of the plan each year noting the date you reviewed it. Name of Operation: Lyd's Kids Facility ID: 79000341 Consultant: TAMMY CHILDRESS Operation Type: Family CC Home Case Number: Visit Date: 3/31/2026 Number Present: 3 Completed Date: 3/31/2026 Age: From 0 To 4 Total Minutes: 90 Time In: 02:00 PM Time Out: 03:30 PM Time In: Time Out: List to Use: Family CC Home Type Of Visit: Temp Time Period Announced/Unannounced: Unannounced The purpose of today’s unannounced visit was to monitor your program for compliance with all applicable child care requirements during the first temporary time period visit. You, Lydia Neyer, assisted me with the visit. This facility was issued a temporary license on January 30, 2026, with restrictions of daytime care only. During this visit, we walked through the home and observed the indoor and outdoor environment and completed a full assessment of the licensed space. One child was asleep, one child was being held by Ms. Neyer, and one child was engaged in free choice activities. Program records, including fire drills and monthly outdoor area inspections were complete and current. There have been no changes to the operator’s records since licensure. The following violations were observed today and must be corrected immediately. Violation Number Comment Rule 706 Corrosive agents, pesticides, bleaches, detergents, cleansers, polishes, and products under pressure in an aerosol dispenser and any substance that may be hazardous to a child if ingested, inhaled, or handled were not kept in locked storage when children were in care. Two bottles of aerosol sunscreen were on a shelf in the dining room where children receive care. Hairspray and cologne/perfume in aerosol containers were on a shelf in the bathroom used by children. .1719 (a)(7) 910 Copy of each child's health assessment, including the operator's own preschool child(ren), was not on file within 30 days of enrollment. A child enrolled in care on February 6, 2026 did not have a health assessment on file. GS 110-91(1); 10A NCAC 09.1721(a)(1) Child Care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. Any violation(s) documented may impact the compliance history score. The violation(s) documented must be corrected immediately. On or before April 9, 2026, I must receive a written, dated, and signed compliance letter that describes accurately and in detail, how and when the violations were corrected. Please be aware, any information submitted by you is considered legal documentation. If it is determined the information provided in the letter is not true, this may be considered falsification of information. If sufficient information is not received by the due date, a follow-up visit may be completed. Email the information to me at tammy.childress@dhhs.nc.gov. The compliance letter must be sent from the email address registered with the DCDEE (this serves as your signature) and the following information must be included: name, position, facility name, and facility ID number. Technical Assistance Provided During Today’s Visit: Any substances such as sunscreen, hair spray, cooking spray, perfume/colognes that are in aerosol dispensers must be kept in locked storage at all times (key, combination, magnetic, or electronic). The aerosol containers have multiple warnings other than keep out of reach of children. All enrolled children must have their medical assessment on file within 30 days of enrollment. Please use the child file checklist to ensure that all documents are in children’s file within the required timeframes. Consultation Provided During Today’s Visit: Rated License Discussion- On March 19, 2026, I emailed you information regarding the three pathway options to obtain a 2–5-star license and the Application for Assessment for a Rated License for a FCCH. I asked you to review this information prior to our visit. During today’s visit we reviewed QRIS Modernization rules and processes, “QRIS Pathway to the Stars, Where Quality Meets Choice” for Family Child Care Home operators. We reviewed each of the Pathway options and the requirements within each option for star ratings. You will take additional time to review this information, and if you feel that you would like to apply for a 2-5 Star Rated License prior to the expiration of your Temporary License, you will need to submit to me a Rated License Application no later than May 1, 2026, with the pathway option you have selected indicated. As a newly licensed operator, I encourage you to contact the Rockingham Partnership for Children at (336)342-9676 for assistance in preparing for a star rated license. You should visit the North Carolina Rated License Assessment Project (NCRLAP) website at ncrlap.org and review information for the Family Child Care Environment Rating Scale 3 (FCCERS-3) and the “FCCH Self-Study process.” The FCCERS-3 and the self-study are required for Pathway 1- Program Assessment for Family Child Care Homes (FCCH). Documents can be accessed there for you to use to complete the self-study (in the “FCCH Self-Study” blue box option) if you choose this pathway. In addition, you will need to obtain a copy of the spiral bound, green booklet entitled “Family Child Care Environment Rating Scale Third Edition.” You may order a copy online by google searching the name of the book or ask your local smart start office if there is a copy of the booklet that you can use for the self-study. The NCRLAP website also has excellent resources and training that will be helpful for you. We reviewed education requirements for each star rating. We discussed that your Temporary License expires on July 30, 2026, and that this may allow you time to complete EDU 119 or other EDU coursework to meet the education standards. We discussed the T.E.A.C.H. Early Childhood® Project for comprehensive scholarships to help pay the cost of tuition and books. To find out more about the project and scholarship information call 919.967.3272 or visit the website www.childcareservices.org. All community colleges provide EDU coursework that you can complete. I recommend checking with various community colleges (Forsyth Tech, Johnston Community College, Haywood Community College, etc.) and explore an on-line option for EDU 119 and look carefully as to the length of the course. We also discussed purchasing and using curriculum required at higher star ratings. At 2-3 Star Ratings the curriculum does not have to be from the approved curriculum list on the DCDEE website. You can learn more about the Early Learning Matters, free curriculum, visit the website https://elmcurriculum.org/. Once you select a curriculum, you will need to receive training on using the curriculum and receive a training certificate. On-Going Professional Development In addition to the required Health and Safety Trainings, you can access professional development training on Moodle under Early Childhood Professional Development in the categories of Child Care and Development Fund (CCDF), Orientation and Regulatory. Use your individual NCID username and password to access the Child Development Module. If you experience problems with Moodle, contact dcdee_moodle_support@dhhs.nc.gov for assistance. In addition to gaining introductory knowledge about child development, early educators learn about the many resources and professional agencies available in North Carolina to help with promoting a responsive and engaging learning environment to support all children served. Upon completion, you will receive one (1) contact hour credit (CHC). You stated today that you had completed the Health and Safety Trainings, and we will review your certificates during the next visit. You have completed lead water testing and the requirements for lead-based paint and asbestos testing. You have completed well water testing. Lead water testing must be repeated every 3 years, and the well water testing must be completed every 2 years. On March 19, 2026, and March 25, 2026 I emailed you information for the required ABCMS Provider Portal training and the requirement to create a facility roster. You must complete the training and enter the roster during this temporary time period to avoid a violation of child care requirements found in Child Care Rule 10A NCAC 09 .2703(r). You stated today that you had completed these steps. At the completion of the visit, I was unable to enter and save the visit summary into the Regulatory System. A handwritten visit summary was completed and the computer generated visit summary will be emailed to you once I can access the system. You, Lydia Neyer, assisted me with the visit. This facility was issued a temporary license on January 30, 2026, with restrictions of daytime care only. During this visit, we walked through the home and observed the indoor and outdoor environment and completed a full assessment of the licensed space. One child was asleep, one child was being held by Ms. Neyer, and one child was engaged in free choice activities. Program records, including fire drills and monthly outdoor area inspections were complete and current. There have been no changes to the operator’s records since licensure. The following violations were observed today and must be corrected immediately. Violation Number Comment Rule 706 Corrosive agents, pesticides, bleaches, detergents, cleansers, polishes, and products under pressure in an aerosol dispenser and any substance that may be hazardous to a child if ingested, inhaled, or handled were not kept in locked storage when children were in care. Two bottles of aerosol sunscreen were on a shelf in the dining room where children receive care. Hairspray and cologne/perfume in aerosol containers were on a shelf in the bathroom used by children. .1719 (a)(7) 910 Copy of each child's health assessment, including the operator's own preschool child(ren), was not on file within 30 days of enrollment. A child enrolled in care on February 6, 2026 did not have a health assessment on file. On or before April 9, 2026, I must receive a written, dated, and signed compliance letter that describes accurately and in detail, how and when the violations were corrected. We reviewed each of the Pathway options and the requirements within each option for star ratings. You will take additional time to review this information, and if you feel that you would like to apply for a 2-5 Star Rated License prior to the expiration of your Temporary License, you will need to submit to me a Rated License Application no later than May 1, 2026, with the pathway option you have selected indicated. As a newly licensed operator, I encourage you to contact the Rockingham Partnership for Children at (336)342-9676 for assistance in preparing for a star rated license.
