This visit was conducted with Betty Blackwell, Owner/Director. Currently this center operates with a Five (5) Star license. The restrictions include first and second shift and meets all enhanced requirements and reduced ratios. The facility is owned and operated by Little Angels Child Development Center Inc. The secretary of state website was checked prior to today’s visit and the corporation is listed as current and active. Twenty-one (21) children were present. The children were observed playing indoors and outdoors, completing routine tasks, and eating lunch. All indoor and outdoor areas were monitored. The sanitation inspection was completed on October 3, 2025. The facility received a superior with three (3) demerits. The fire inspection was completed on July 16, 2025. I received a copy today. Fire drill reports were monitored during today's visit. The last fire drill was conducted on September 30, 2025. It took the children one (1) minute to evacuate. The last emergency drill, a shelter in place drill, was conducted on August 29, 2025. The emergency drills must be conducted every three (3) months. The staff and children’s files were monitored in accordance with the Division of Child Development and Early Education policies and procedures. Ms. Blackwell stated that no children enrolled have emergency medication. Ms. Blackwell stated that the facility does not provide transportation. There are twenty-three NC Pre-K children enrolled at the facility. The NC Pre-K requirements were monitored during today’s visit. The following violation was observed during today's visit. Violation Number Comment Rule 807 A safe indoor and outdoor environment was not provided for the children. On the outdoor play environment there was a loose tree limb hanging off of the roof of the wooden stage. 10A NCAC 09 .0601(a) 1769 The health assessment did not include a dental screening. A child's health assessment stated that the child had an upcoming dental appointment but there was no dental screening on file. .3005 (a)(5) 1783 The teacher assistant does not meet the education requirements and/or documentation was not available for review of the progress toward the required standard. The teacher assistant has only completed seven (7) ongoing training hours instead of the required fifteen (15) hours. .3013 (1)(2)(a-b) Rated License Information: Today we discussed the QRIS modernization plan. I completed a template documenting the pathway you plan to pursue, identifying needs, documenting a timeline for completion, resources, next steps, etc. Over the next few months, I’ll be checking in with you, ahead of your next visit to check on progress and determine if additional assistance is needed with WORKS, NCRLAP/ERS, accreditation, etc. Technical Assistance: NC Pre-K teacher assistants must hold an Associate’s Degree, making progress toward having an Associate’s Degree or hold a CDA. They must also complete fifteen (15) trainings hours annually regardless of education level completed. Compliance letter: You must correct the violation found during today's visit immediately. Please send me two (2) letters verifying compliance. The letters must be received by October 28, 2025. Please include in your letter each violation number and how you corrected each violation and what plan will be implemented to prevent these violations from occurring again. Please include the name of your center and ID number, date, and the title of the person who signs the letter. Please send the letter to: Sally Allen, PO Box 962, Eden, NC 27289. If you state in your letter that corrections have been made when they have not, it will be considered falsification of information. Failure to correct these violations in a timely manner may result in an administrative action. If more time is needed to correct any of the violations, you must send a request for an extension as soon as possible. This visit was conducted by Sally Allen, Child Care Consultant, with Betty Blackwell, Director. Currently this center operates with a Five Star license. Restrictions include first and second shift, meets all enhanced requirements and reduced ratio. This facility is owned and operated by Little Angels Child Development Center Inc. The Secretary of State website was checked prior to today’s visit and the corporation was listed as current and active. The sanitation inspection was completed May 3, 2024, with a "Superior" classification with no demerits. The fire inspection was completed on July 18, 2024. A copy was given to me today. Upon arrival staff/child ratio was observed and in compliance as indicated on the attached enrollment worksheet. Children were adequately supervised during today’s visit. All indoor and outdoor areas were observed during today's visit. Twenty-one (21) children were present during today’s visit. The children were observed playing indoors, completing routine tasks, participating in group time and playing outdoors. Fire drill reports and playground inspections were monitored during today's visit. The last fire drill was completed September 30, 2024. It took the children one (1) minute to evacuate. The last emergency drill, a shelter in place drill, was completed on August 30, 2024. Children’s and staff files were reviewed according to the policy set by the Division of Child Development and Early Education. Hazardous supplies were kept in locked storage. Ms. Blackwell stated that children enrolled have emergency medication. Ms. Blackwell stated that they do not provide transportation. The NC Pre-K requirements were monitored today. The facility currently has twenty-two (22) NC Pre-K children. The following violations were observed during today's visit. Violation Number Comment Rule 721 All equipment and furnishings were not in good repair. The fence located at the outdoor building has a large gap at the top measuring three and a half inches. The fencing is able to be pulled away from the pole. G.S. 110-91(6); .0601(b) 1898 Staff did not complete the health and safety training within one year of employment. One (1) staff did not complete all the health and safety trainings. She was missing medication administration. .1102(a) 1899 Health and safety training topics were not included as part of on-going training within five years of completing the previous health and safety training topics. A teacher completed health and safety trainings in June 2019 and has not completed any health and safety training since. .1103(b) Technical Assistance: 1-The health and safety trainings are required to be completed within the first year of employment, with the exception of Recognizing and Responding to Suspicions of Child Maltreatment, CPR and First Aid training. These three (3) trainings must be completed within ninety (90) days of employment. Following the initial completion of the health and safety trainings they must be completed within the next five years of employment or prior to expiration (CPR and First Aid). There is a training log available on the Division’s website to document the completion of the trainings. All topics must be covered. The Recognizing and Responding to Suspicions of Child Maltreatment training can be found at https://www.preventchildabusenc.org/. Consultation: 1-Child Care administrators can now view and edit their facility’s staff roster in ABCMS, the DCDEE’s criminal background check system. Provider Access to ABCMS allows administrators to: •See the real-time background check status of staff members •Run a printable report of the staff roster to assist with compliance visits •See new background check applicants and add to staff roster To get started, complete the ABCMS Child Care Provider Portal Training in Moodle. 2-NC Pre-K teacher assistants must have fifteen (15) training hours annually. Rated License The hold harmless has been extended. You will receive an email regarding the rated license process and when it will resume.
