This visit was conducted with Ms. Alyson Walker, Director. This program currently operates with a Notice of Compliance. Restrictions include daytime care only, building 1 capacity 125, no cooking allowed in space 3 and 4 only, building 3 capacity 60, children may not play on ark outside fenced playground, children under 2 ½ years old in rooms with direct exits only. I have observed indoor and outdoor areas today. There are no new staff since my last visit. I monitored staff/child ratios and space capacity today, as is indicated on the attached worksheet. I observed proper supervision of children. During the visit, the children were playing indoors and outdoors, having circle time and playing in activity centers. Today, the following items were monitored: • Supervision • Capacity • CPR / First Aid • SIDS training • Criminal Background Checks • General health • Administration of medication • Storage of hazardous products • Storage of medication • General safety • General license requirements • Program records • License posted • Fire safety • Sanitation Limited monitoring of staff files was conducted in accordance with DCDEE procedures for the purpose of reviewing CPR and First Aid, EPR, SIDS training, Health and Safety trainings and Criminal Background Checks. The storage and administration of medications was monitored. Medication authorization was monitored. The storage of hazardous items was monitored. A fire drill was documented on 09-06-2024. A shelter in place drill was documented on 05-16-2024. An outdoor inspection was documented on 10-07-2024. The following violations were documented during this visit: Violation Number Comment Rule 812 Electrical outlets and power strips, not in use, which were located in space used by children did not have safety outlets or were not covered with safety plugs unless located behind furniture or equipment that cannot be moved by a child. There was one outlet uncovered in the hallway. 10A NCAC 09 .0604(c) 859 Monthly playground inspections were not completed and/or they were not completed by an individual trained in playground safety requirements. There was no monthly playground check completed for September 2024. .0605(q) 1850 Signage was not posted regarding the smoking and tobacco restriction at the entrance of the center and/or in vehicles used to transport children. There was no "No Smoking" sign at the entrance of the preschool. .0604(i) Additional monitoring visits may be conducted during the year to monitor compliance with applicable childcare requirements. I recommended that Ms. Walker do another shelter in place drill for this school year. Ms. Walker reports that this site does not transport children at all. I have reviewed this visit summary with Ms. Walker at the conclusion of the visit and a copy was given to her. This visit was conducted with Ms. Alyson Walker, Director. This program currently operates with a Notice of Compliance. The NC Secretary of State website was reviewed on 05-13-2024. The Regulatory System is currently down. Programs are required to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. I have observed indoor and outdoor areas today. I monitored staff/child ratios and space capacity today, as is indicated on the attached worksheet. I observed proper supervision of children. During the visit I observed children playing indoors and outdoors. The last sanitation inspection was conducted on 1-30-2024. The last fire inspection was conducted on 4-30-2024. A fire drill was documented on 4-30-2024. A shelter in place drill was documented on 03-11-2024. An outdoor inspection was documented on 05-06-2024. I observed program records and posted items today. I observed staff and children’s records today according to DCDEE procedures. I observed the storage of hazardous items and cleaning supplies. I observed both the storage of medications and the administration of medication paperwork. This program does not provide transportation. Today the following violations were documented: Violation Number Comment Rule 607 Separate lavatories with hot and cold running water, soap, and individual sanitary towels or other approved hand-drying devices were not available for staff in food preparation areas or diaper changing areas. One bathroom used by two classrooms of children did not have paper towel. 15A NCAC 18A .2818(b) & (d) 812 Electrical outlets and power strips, not in use, which were located in space used by children did not have safety outlets or were not covered with safety plugs unless located behind furniture or equipment that cannot be moved by a child. Twenty electrical outlets within reach of children were not covered. 10A NCAC 09 .0604(c) 1879 Prescribed medicines, that are pharmaceutical samples, was not stored in the manufacturers original packaging, was not labeled with the child's name, and/or written instructions did not include the required information. One emergency medicine was not in the original packaging with the child’s name. 10A NCAC 09 .0803(2)(b)(i-v) Additional monitoring visits may be conducted during the year to monitor compliance with applicable childcare requirements. I have asked Ms. Walker to send me written documentation by May 27, 2024 indicating that the violations that were documented today have been corrected. I have asked her to include the center name and license number in her documentation, each violation number and how each was corrected, and to both sign and date the document. The document may be mailed or emailed to me at the address below. I have reminded Ms. Walker that if the documentation indicates that corrections have been made when they have not, this may be considered falsification of information and an Administrative Action may be recommended. In addition, repeated violations, continued non-compliance, or failure to submit the compliance letter prior to the date indicated in this visit summary could lead to the recommendation of an Administrative Action. Consultation – I have suggested that very large plastic bags that are used to hold sleeping items be out of reach of children when not in use. I have reviewed this visit summary with Ms. Walker at the conclusion of the visit and a copy was given to her.
