Name of Operation: Covington Childcare Center Facility ID: 79000337 Consultant: TAMMY CHILDRESS Operation Type: Center Case Number: Visit Date: 6/30/2026 Number Present: 5 Completed Date: 6/30/2026 Age: From 3 To 5 Total Minutes: 125 Time In: 09:40 AM Time Out: 11:45 AM Time In: Time Out: List to Use: Center Type Of Visit: Routine Unannounced Announced/Unannounced: Unannounced The purpose of today’s visit was to monitor your program for compliance with applicable child care requirements for a routine unannounced visit. Misty Dove, Lead Teacher, and Nicole Garrison, Administrator, assisted me with the visit. The program currently operates with a Notice of Compliance effective December 18, 2025, with restrictions of daytime care only and no children in care under three years of age. The program’s compliance history was one hundred percent prior to today’s visit. The NC Secretary of State website was reviewed today and the owner, Covington Wesleyan Church, was listed as current- active. Today, the following items were monitored: •Supervision- In Compliance •Staff / Child Ratio- In Compliance (1:5- Ages 3-5 years old) •CPR / First Aid – In Compliance •CBC Qualification- In Compliance •Emergency Medical Care Plan- Posted •Administration of Medication- There are no enrolled children who are administered medication at the facility. •Storage of Hazardous Products- No hazardous products were observed •Storage of Medication- There are no medications on site •General Safety- In Compliance •Discipline- In Compliance- Observed Verbal Re-Direction •Adequate / Approved Space- In Compliance •Program Records- Observed •Permit Restrictions- In Compliance A walk-through of the facility was completed today; all indoor and outdoor areas were monitored. Children present today were participating in group time, free choice play with classroom materials, and personal care routines. Ms. Dove was interacting with and meeting the needs for each of the children. A guest arrived during the visit to do a science activity with the children. The following violations were documented today: Violation Number Comment Rule 805 Fire drills were not practiced monthly and/or the drill record was incomplete. Fire drills were not documented for the months of May and June 2026. .0604(t); .0302(d)(5) 1890 Each staff member did not have the required medical report, proof of tuberculosis test or screening and/or completed health questionnaire in a medical file, maintained separately from the staff member's individual personnel file. A new staff member hired June 15, 2026, did not have a medical report or Tb screening or test results on file. .0701(d) Child care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. Any violation(s) documented may impact the compliance history score. The violation(s) documented must be corrected immediately. On or before July 14, 2026, I must receive a written, dated, and signed compliance letter that describes accurately and in detail, how and when the violations were corrected. Please be aware any information submitted by you is considered legal documentation. If it is determined the information provided in the letter is not true, this may be considered falsification of information. If sufficient information is not received by the due date, a follow-up visit may be completed. Email the information to me at tammy.childress@dhhs.nc.gov. This email must be sent from the email address on file with DCDEE (this serves as your signature) and the following information must be included: name, position, facility name, and facility ID number. Technical assistance: Fire Drills and lockdown drills were posted. A fire drill has not been documented for the months of May or June 2026. Ms. Garrison stated that the drills were completed but had not been documented. It may be helpful to designate a staff member to double-check that the information is entered on the form when drills are completed. You might also consider completing a scheduled monthly review of all posted documents, including the drill record, to ensure that all documents are current. A staff member’s medical and tb test results were not on file. These documents must be on file prior to employment to ensure that the staff person is free of active Tb and is emotionally and physically fit to care for children. Consultation Provided During Today’s Visit: There has been a name change for the corporate owner, effective June 3, 2026, and a review of all required documents was completed with Ms. Garrison and the pastor during today’s visit. Emergency Preparedness and Response in Child Care Training; You must complete Emergency Preparedness and Response in Child Care Training no later than December 17, 2026. Contact the Rockingham County Partnership for Children at (336)342-9676 to schedule this training. Once the training is completed you will have four months to create a specific Emergency Response Plan for your program. You will receive guidance in accessing a template to create your plan during the training. You must review the completed EPR plan with all staff members and then again review annually. The date of the initial and then the date of the annual plan review should be entered on Staff and Training Worksheets for each staff member, and I would recommend a statement with the review date and signature for all staff to be on file. ABCMS Provider Portal and Facility Roster: You are working to complete a center staff roster. You must enter all employees into the ABCMS (Automated Background Check Management System) provider portal and verify your facility roster to ensure current staff are noted on the roster. This information should be updated in ABCMS on an ongoing basis as staff members are hired and when their employment is terminated. You will need to access ABCMS every six months to complete the verification process; this ensures the facility’s ABCMS portal includes only current employees who are in the facility’s ABCMS portal. Lead in Water testing was completed on 9/25/2025 and must be repeated every three years. You must re-test no later than 9/25/2028 through Clean Classrooms for Carolina Kids, https://www.cleanwaterforuskids.org/en/carolina/. At the completion of the visit, this visit summary was reviewed with Ms. Garrison. A copy will be emailed to you. Ms. Garrison had other scheduled church obligations following the visit.
