This visit was conducted with Patricia Bray, Teacher. Currently this center operates with a Five Star license. Restrictions include daytime care only, meets all enhanced requirements and reduced ratio and a Certified Developmental Day. This facility is owned and operated by Rockingham County Schools. The sanitation inspection was completed September 16, 2025. The facility received a superior rating with two (2) demerits. The last fire inspection on file was completed on September 19, 2025. Upon arrival staff/child ratio was observed and in compliance as indicated on the attached enrollment worksheet. Children were adequately supervised during today’s visit. All indoor and outdoor areas were observed during today's visit. Eleven (11) children were present during today’s visit. The children were observed playing indoors and completing routine tasks. Fire drill reports and playground inspections were monitored during today's visit. The last fire drill was completed February 25, 2026. It took the children twenty (20) seconds to evacuate. The last emergency drill, a lockdown drill, was completed on November 19, 2025. Emergency drills must be conducted every three (3) months. Children’s and staff files were reviewed according to the policy set by the Division of Child Development and Early Education. The substitute present today does not have a criminal record check. Amy Rose, preschool coordinator, was contacted due to the principal and lead teacher being on a school field trip. A therapist that has a current criminal background check will come to the school within an hour to meet the staff/child ratio. The substitute must leave as soon as the therapist arrives. Only substitutes with a criminal background check will be allowed to meet staff/child ratio for the rest of the week. Ms. Rose will contact me to inform me of who will be subbing the rest of the week. Ms. Bray stated that no child enrolled has emergency medication. Ms. Bray stated that they do not provide routine transportation. The NC Pre-K requirements were monitored today. The facility currently has nine (9) NC Pre-K children. The following violations were observed during today's visit. Violation Number Comment Rule 1041 Prior to employment a Criminal Background Check was not completed. There was a substitute present that does not have a criminal background check. G.S. 110-90.2(b) 1805 A child care operator did not notify the Division of any new child care providers, as defined in G.S. 110-90.2(a)(2), who were hired or moved into the child care facility within five business days. The principal is not listed on the staff roster in the ABCMS system. G.S. 110-90.2 & .2703(r) Technical Assistance: Only employees that have a criminal background check can be counted in the staff/child ratio or be left alone with the children. It may be good practice to have other individuals in the school to have a current criminal background check completed in case of emergencies or case of staff absences. Rated License Information: This facility completed a rated license assessment on April 24, 2025. They will be due for a rated license reassessment in April 2027. Compliance: You must correct the violation found during today's visit immediately. Please send me two (2) letters verifying compliance. The letters must be received by April 1, 2026. Please include in your letter each violation number and how you corrected each violation and what plan will be implemented to prevent these violations from occurring again. This visit was conducted by Sally Allen, Child Care Consultant, with Ashley Bloss, Lead Teacher. Currently this center operates with a Five Star license. Restrictions include daytime care only, meets all enhanced requirements and reduced ratio and Certified Developmental Day. This facility is owned and operated by Rockingham County Schools. The sanitation inspection was completed February 25, 2025, with a "Superior" classification with no demerits. The fire inspection was completed on September 19, 2024. I received a copy during a routine unannounced visit. Upon arrival staff/child ratio was observed and in compliance as indicated on the attached enrollment worksheet. Children were adequately supervised during today’s visit. All indoor and outdoor areas were observed during today's visit. Fifteen (15) children were present during today’s visit. The children were observed playing indoors, completing routine tasks and playing outdoors. Fire drill reports and playground inspections were monitored during today's visit. The last fire drill was completed March 26, 2025. It took the children twenty (20) seconds to evacuate. The last emergency drill, a lockdown drill, was completed on February 21, 2025. Children’s and staff files were reviewed according to the policy set by the Division of Child Development and Early Education. Hazardous supplies were kept in locked storage. Ms. Bloss stated that a child enrolled has emergency medication. Ms. Bloss stated that they do not provide transportation. The NC Pre-K and Developmental Day requirements were monitored today. The facility currently has fifteen (15) NC Pre-K children. The following violations were observed during today's visit. Violation Number Comment Rule 847 Parent's medication authorization did not include required information. A child's permission to administer expired March 2025. 10A NCAC 09 .0803(4)(6-9) 1048 All staff did not successfully complete certification in First Aid appropriate to the age of children in care. Verification of staff completion of First Aid training from an approved training organization was not in the staff file. There was no record of a staff member completing first aid training. .1102(c) 1897 The child care administrator and all staff did not complete the Recognizing and Responding to Suspicions of Child Maltreatment training within 90 days of employment. There was no record of a staff member completing the training. .1102(g) Technical Assistance: 1-The health and safety trainings are required to be completed within the first year of employment, with the exception of Recognizing and Responding to Suspicions of Child Maltreatment, CPR and First Aid training. These three (3) trainings must be completed within ninety (90) days of employment. Following the initial completion of the health and safety trainings they must be completed within the next five years of employment or prior to expiration (CPR and First Aid). There is a training log available on the Division’s website to document the completion of the trainings. All topics must be covered. The Recognizing and Responding to Suspicions of Child Maltreatment training can be found at https://www.preventchildabusenc.org/. Consultation: 1-Child Care administrators can now view and edit their facility’s staff roster in ABCMS, the DCDEE’s criminal background check system. Provider Access to ABCMS allows administrators to: •See the real-time background check status of staff members •Run a printable report of the staff roster to assist with compliance visits •See new background check applicants and add to staff roster To get started, complete the ABCMS Child Care Provider Portal Training in Moodle. The process of notifying the Division has changed and is now captured in ABCMS. This change has been in effect since February 2024. Effective immediately, you will need to obtain a Business NCID and complete Provider Portal training in Moodle at https://www.dcdee.moodle.nc.gov/course/view.php?id=119. No action is needed on your part if you have completed the reference training and are currently using the ABCMS Provider Portal to update information regarding new hires or residents. Once the training has been completed and access has been given, you must verify your facility roster to ensure current staff are noted on the roster. This information should be updated in ABCMS in an ongoing basis as staff members are hired and when their employment is terminated. This satisfies the requirement to notify the Division of new child care providers working who were hired or moved into the child care facility within five business days. The compliance of this rule will be monitored during your next visit. Please note, the hard copy of the Change of Information form will no longer be needed or accepted. Should you need assistance please contact the Criminal Background Check Unit at (919) 814-8401 and someone will assist you. 2-NC Pre-K teacher assistants must have fifteen (15) training hours annually. Rated License The facility voluntarily applied for a rated license reassessment. They had an environment rating scale completed on June 4, 2024. There was a change in the teacher assistant. Program Standards: The facility meets enhanced requirements and reduced ratios. They received a score of 6.35 on the ECERS-R completed on June 4, 2024. There were only three (3) indicators that scored less than five (5) on the scale. The facility qualifies for seven (7) points in program standards. Staff Education: The principal meets seven (7) points with a principal license. The lead teacher meets seven (7) points with a BK license and years of experience. The teacher meets seven (7) points with an Associate Degree and years of experience. The facility qualifies for seven (7) points in staff education. Quality point: The facility meets the quality point by having staff benefits and parent involvement. The facility qualifies for a Five (5) Star license earned with seven (7) points in program standards, seven (7) points in staff education and one (1) quality point. Once the violations cited today are corrected a rated license will be issued. Compliance: You must correct the violation found during today's visit immediately. Please send me two (2) letters verifying compliance. The letters must be received by April 24, 2025.
