This visit was conducted with Bisceglari Wilson, Owner/Lead Teacher. Currently this center operates with a Five Star license. Restrictions include daytime care only, children in care on ground level only, children may not use lobby elevator, no children under three (3) and meets all enhanced requirements and reduced ratio. This facility is owned and operated by Bee’s Discovery and Learning Centre LLC. The Secretary of State website was checked and the corporation is listed as current and active. The sanitation inspection was completed October 20, 2025. The facility received a superior rating with two (2) demerits. The fire inspection was completed on June 19, 2025. I received a copy today. Upon arrival staff/child ratio was observed and in compliance as indicated on the attached enrollment worksheet. Children were adequately supervised during today’s visit. All indoor and outdoor areas were observed during today's visit. Nine (9) children were present during today’s visit. The children were observed eating breakfast, playing indoors, completing routine tasks and participating in group time. Fire drill reports and playground inspections were monitored during today's visit. The last fire drill was completed November 26, 2025. It took the children three (3) minutes to evacuate. The last emergency drill, a lockdown drill, was completed on October 30, 2025. Emergency drills must be conducted every three (3) months. Children’s and staff files were reviewed according to the policy set by the Division of Child Development and Early Education. Ms. Wilson stated that a child enrolled has emergency medication. Ms. Wilson stated that they do not provide routine transportation. The NC Pre-K requirements were monitored today. The facility currently has six (6) NC Pre-K children. The following violations were observed during today's visit. Violation Number Comment Rule 849 Leftover medicines were not returned to the parent after the course of treatment was completed, after authorization was withdrawn or after authorization had expired and/or medication was not discarded within 72 hours of completion of treatment or withdrawal of authorization. A permission to administer for an inhaler expired September 2025. .0803(12) 1768 The health assessment did not include a hearing screening. A child's assessment was missing the hearing screening. .3005 (a)(4) 1769 The health assessment did not include a dental screening. Two (2) children's health assessments were missing a dental screening. .3005 (a)(5) 1805 A child care operator did not notify the Division of any new child care providers, as defined in G.S. 110-90.2(a)(2), who were hired or moved into the child care facility within five business days. One staff was not listed on the staff roster in the ABCMS system. G.S. 110-90.2 & .2703(r) 1835 The medical action plan was not updated on an annual basis or when changes to the plan were made by the child's parent or health care professional. A medical action plan dated June 2024 was not updated annually. .0801(b) Rated License Information: The facility is not due for a rated license reassessment until June 2027. We briefly discusses the pathway to the stars today by reviewing the three (3) pathways. Compliance: You must correct the violation found during today's visit immediately. Please send me two (2) letters verifying compliance. The letters must be received by December 17, 2025. Please include in your letter each violation number and how you corrected each violation and what plan will be implemented to prevent these violations from occurring again. Please include the name of your center and ID number, date, and the title of the person who signs the letter. Please send the letter to: Sally Allen, PO Box 962, Eden, NC 27289. If you state in your letter that corrections have been made when they have not, it will be considered falsification of information. The visit was conducted with Bisceglari Wilson, Owner/Director, and Annette Galloway, Director, by Tammy Childress, Lead Child Care Consultant. Ms. Wilson and Ms. Galloway are also acting as Lead Teachers at this time. Currently this center operates with a Temporary License, issued on December 1, 2023, to operate first shift. The facility is licensed to provide care to children ages three to twelve-years-old and is approved to operate one classroom within the building with an occupancy of seventy-six. Restrictions are daytime care only, children in care on ground level only, children may not use lobby elevator, and no children under 3. The Secretary of State website was checked today, and the owner, Milestone Learning Centre LLC, is current on all required documents and active on the NC Business Registry. If any changes to the corporation need to be made or you decide to sale your business, then you must notify your consultant at least thirty days prior to any changes occurring. The license cannot be bought, sold, subleased, transferred to another person or location, or inherited. All designated indoor and outdoor spaces for child care were monitored, all staff files, all program records and documents required to be posted were monitored during today’s visit. A percentage of children’s files were reviewed during the last visit. The facility does not provide transportation. There have been no changes to the playground area since the pre-licensing and first temporary time period visits. Meals are prepared at the child care facility and children eat all meals in their classroom. The playground is located to the right of the building and is surfaced with wood mulch and grass, is enclosed by four-foot chain linked fencing and has no stationary equipment. During today’s visit children present were observed to be engaged in group and free choice activities, personal care routines, lunch, and nap. The following violations were cited during today’s visit: Violation Number Comment Rule 508 Special diet or food allergy information was not posted where they can be seen in food preparation and eating areas. Enrolled children's food allergies were not posted in the kitchen. .0901(g) 525 Menus for all meals and snacks were not planned at least 1 week ahead and dated. The posted menu was for April 2, 2024, only. 10A NCAC 09 .0901(b) 608 Children did not wash their hands upon arrival at the center, after each visit to the toilet, before eating, before and after water activity play, after outside play, and after handling animals or animal cages. Children did not wash hands after returning to the classroom from the playground. 15A NCAC 18A .2803(c) 841 Medications including prescription and non-prescription items were not stored in a locked cabinet or other locked container. Benadryl for one enrolled child was not in locked storage. 15A NCAC 18A .2820(d) 1033 On or before the first day of work, all staff, including the director and individuals who volunteer more than once per week did not provide results indicating that they were free of active TB and/or TB test or screening was older than 12 months. One staff member hired January 2, 2024, did not gave Tb test results on file. .0701(a) All violations must be corrected immediately. You must submit documentation on how each violation was corrected to Ms. Childress by April 16, 2024. Tuberculin (TB) Test or Screening The results indicating the individual is free of active tuberculosis shall On or before first day of work. 10A NCAC 09 .0901 GENERAL NUTRITION REQUIREMENTS (b)When food is prepared by or provided by the center, menus for nutritious meals and snacks shall be planned at least one week in advance. At least one dated copy of the current week's menu shall be posted where it can be seen by parents and food preparation staff when food is prepared or provided by the center. A variety of food shall be included in meals and snacks. Any substitution shall be of comparable food value and shall be recorded on the menu prior to the meal or snack being served. (g) Children's special diets or food allergies shall be posted where they can be seen in the food preparation area and in the child's eating area. 15A NCAC 18A .2820 STORAGE (c) Medications including prescription and non-prescription items shall be stored in a locked cabinet or other locked container and shall not be stored above food. Designated emergency medications shall be kept inaccessible to children but are not required to be in locked storage. Non-prescription diaper creams and sunscreen shall be kept inaccessible to children when not in use but are not required to be in locked storage. 15A NCAC 18A .2803 HANDWASHING (c) Child care center employees shall ensure that children wash their hands as follows: (1) upon arrival at the child care center; (2) after each diaper change or visit to the toilet; (3) before eating meals or snacks; (4) before and after water play; (5) after being outdoors; and (6) after handling animals or animal cages. One enrolled child had a Medical Action Plan (MAP) signed by a parent and doctor indicating the use of Benadryl for food allergies, but the mother has not provided the facility with this medication. Today we discussed that since the information for the medication is on the MAP, the mother should either provide the medication or ask the doctor to complete another form. Your compliance history affects the star rating. It is imperative that staff maintain compliance with child care rules and requirements at all times. Violations of child care requirements affect the facility’s compliance history. You must maintain a minimum compliance history of seventy-five (75) percent. During today’s visit we again discussed the Rated License Assessment Process and the Environment Rating Scale Assessment. We reviewed the program and education components for a star rated license, how points are earned, and the Environment Rating Scale Assessment. The application for a Star Rated License Assessment and Request for an Environment Rating Scale Assessment was completed during today’s visit and the ERS Assessment will be requested tomorrow, April 3, 2024. Continue to prepare for the ERS Assessment by reviewing the Early Childhood Environment Rating Scale (revised edition) booklets, visiting the North Carolina Rated License Assessment Project website at www.ncrlap.org and reviewing resource guides/additional notes/ informational videos and participate in the free online training on topics such as activity areas, indoor and outdoor materials, daily schedules, sanitation, and interactions. Instructions for the DCDEE WORKS system is located in the DCDEE website at https://ncchildcare.ncdhhs.gov. Once the Facility Summary Report with the individual and overall assessments scores is received and all staff education is verified through WORKS a final visit will be conducted to determine the Star Rated License that will be issued. Health and Safety Training All staff must complete Health and Safety Trainings within one year of hire and must be repeated every five years with training certificates on file for review. These trainings can be accessed on the DCDEE website at https://ncchildcare.ncdhhs.gov under the Provider tab from the home screen, select Training and Professional Development, and finally DCDEE Moodle. If you need assistance in locating the trainings, please contact me. It is important to remember that the child care administrator and all staff members must complete the specific Health and Safety Training called “Recognizing and Responding to Suspicions of Child Maltreatment”, within 90 days of employment. This training can be accessed through Moodle or directly at https://www.preventchildabusenc.org/services/trainings-and-professional-development/rrcourse. Orientation For New Staff/ New Staff Files/ CPR/FA and Trainings Health and safety training shall be in addition to the new staff orientation requirements. Orientation topics should be a review of the policies specific to your facility. All staff, including the director, must receive 16 hours of orientation. The orientation form is available on the DCDEE website www.ncchildcare.ncdhhs.gov under provider documents and forms. All documents listed on the staff file checklist must be in staff files and available for review. All CPR/FA training must be in-person by an approved training. Online only CPR/FA courses will not be accepted. On-Going Professional Development If staff complete approved college courses in child development, for every one college credit hour (CEU) completed staff will earn 16 hours of on-going training. All staff must complete the required number of on-going training hours based upon education and experience within one year of hire and annually. The required number of training hours can be determined for each staff member using the information provided in Child Care Rule 10A NCAC 09 .1103. In addition to the required Health and Safety Trainings, staff can access professional development trainings on Moodle under Early Childhood Professional Development in the categories of Child Care and Development Fund (CCDF), Early Educator Support, Licensure, and Professional Development (EESLPD), Foundations and Regulatory. Specifically, under the Regulatory section, I would encourage all staff to complete the training for “Child Care Rule Rollout” which includes a section for the overview of new rules and rule amendments effective July 2023. Staff Development Plan and Annual Evaluation All staff members must complete a Professional Development Plan and receive an annual evaluation within one year of hire. A sample staff development plan is available on the DCDEE website under Provider Documents and Forms. The Staff Development Plan and the annual evaluation are two different requirements. The purpose of the Staff Development Plan is to review staff goals and if these goals have been met, and to set new professional goals each year. The staff evaluation is to review each staff member’s job performance and to identify areas where the staff member is excelling and the areas where performance should be improved. For some areas for improvements that are of concern regarding the health and safety of children, a specific plan of action should be clearly outlined and show the plan will be monitored. Emergency Preparedness and Response Plan Ms. Wilson and Ms. Galloway have completed the Emergency Preparedness and Response in Child Care training while employed at other child care facilities. An EPR Plan must be created and entered for this facility on the designated website https://rmp.nc.gov/portal/portal.aspx within thirty (30) days. Once completed, the plan must be printed and reviewed with staff. You are required to individually review the plan annually, regardless of whether changes are made, print a cover page with the current year’s date, and review the plan annually with all staff, regardless of whether changes are made. Ready to go files and materials should be reviewed and kept current. The date of the plan review should be entered on Staff and Training Worksheets for each staff member, and I would recommend a statement with the review date and signature for all staff to be on file. Continue to document monthly fire drills, using the fire alarm system, and emergency drills (lockdown or shelter-in-place) quarterly. The Secretary of State website was checked today, and the owner, Milestone Learning Centre LLC, is current on all required documents and active on the NC Business Registry. If any changes to the corporation need to be made or you decide to sale your business, then you must notify your consultant at least thirty days prior to any changes occurring. All designated indoor and outdoor spaces for child care were monitored, all staff files, all program records and documents required to be posted were monitored during today’s visit. The area is enclosed by chain linked fencing, is surfaced by wood mulch and grass, and has no stationary equipment, and no hazards were observed. The facility does not provide transportation. Emergency Preparedness and Response Plan Emergency Preparedness and Response Training has been completed and you must complete an EPR Plan on a template at the following website https://rmp.nc.gov/portal/portal.aspx within four (4) months of licensure (April 1, 2024). A copy of the EPR plan must be printed and on file at the facility for review. After you complete the Emergency Preparedness and Response in Child Care training, you must conduct either a shelter-in-place or a lockdown drill every three months. Use the Emergency Drill Log to document these additional drills as well as the fire drills. The form is located under the Provider Documents tab on the DCDEE website at www.ncchildcare.ncdhhs.gov. The EPR Plan will be reviewed with all staff upon hire during orientation, whenever changes to the plan occur, and at least yearly. The date of the review of the plan with staff should be documented on the Staff and Training Worksheets. In addition, the administrator must review the plan annually in the portal and make updates if required. Once reviewed, print off the first page of the plan which will reflect the date of the review. Orientation For New Staff Health and safety training shall be in addition to the new staff orientation requirements. Orientation topics should be a review of the policies specific to your facility. The dates for orientation must be within two weeks and six weeks of the employee’s date of hire and must not include orientation dates that occurred while the staff person was working at another facility. The staff orientation form, found on the DCDEE website, under Provider Documents and forms was reviewed with Ms. Wilson and Ms. Galloway during today’s visit. Emergency Medical Care Plan The people listed on the Emergency Medical Care Plan must be on-site and the alternative named individual must be on-site when the first person is absent. The names of the individuals should never be listed until they are actively employed. A summary of today’s was reviewed with Ms. Wilson and she was given the opportunity to ask any questions. The visit was conducted with Bisceglari Wilson, Owner/Director, and Annette Galloway, Director, by Tammy Childress, Lead Child Care Consultant. Ms. Wilson and Ms. Galloway are also acting as Lead Teachers at this time. Currently this center operates with a Temporary License, issued on December 1, 2023, to operate first shift. The facility is licensed to provide care to children ages three to twelve-years-old and is approved to operate one classroom within the building with an occupancy of seventy-six. Restrictions are daytime care only, children in care on ground level only, children may not use lobby elevator, and no children under 3. The Secretary of State website was checked today, and the owner, Milestone Learning Centre LLC, is current on all required documents and active on the NC Business Registry. If any changes to the corporation need to be made or you decide to sale your business, then you must notify your consultant at least thirty days prior to any changes occurring. The license cannot be bought, sold, subleased, transferred to another person or location, or inherited.
