A checklist was used to note the requirements that were monitored. Applicable child care requirements were reviewed with Ms. Kimberly Martin, Operator. During the visit four (4) children were present. The children were observed washing their hands after eating lunch, resting on their cots, and during free play activities. The operator was observed holding an infant while bottle feeding. Appropriate interaction between the operator and children were observed. I also monitored the program and space requirements, equipment and furnishings, capacity and supervision, storage of hazardous products and administration procedures, and outdoor area. Children do not participate in aquatic activities and Ms. Martin does not provide transportation. The operator’s records, children records, and program records were reviewed. The operator completed the verification of required information for operators and caregivers. Violation Number Comment Rule 706 Corrosive agents, pesticides, bleaches, detergents, cleansers, polishes, and products under pressure in an aerosol dispenser and any substance that may be hazardous to a child if ingested, inhaled, or handled were not kept in locked storage when children were in care. Laundry detergent was not stored in locked storage. .1719 (a)(7) 1894 Application did not include names and contact information of individuals to whom the operator may release the child as authorized by the person who signs the application. The application included the name to whom the operator may release the child but it didn't include the contact information. .1721(a)(3)(F) 1916 A shaded area was not available to children while using the outdoor learning environment. If you cannot meet the requirements by March 13, 2024, you must contact me with a purposed timeline of the corrections. In some cases, this timeline may be extended. You are required to always maintain compliance with all applicable child care rules and regulations. NC GS 110-90(4)(d) requires all child care facilities to maintain a compliance history score of at least 75% for the past eighteen (18) months or during the length of time the facility has operated, whichever is less. Failure to do so may result in the issuance of a provisional license or other administrative action. Violations cited during today’s visit, may negatively affect your compliance score. As of today’s, visit your compliance history is 94%. Technical Assistance: Health and Safety training topics must be completed within the first year of employment and then within every five (5) years after. Documentation of the training must be kept on file for review. You should record the training on the Health and Safety Training Record Form. This form can be found on the DCDEE website. Health and Safety Training may be found on the DCDEE website using the Moodle platform. Child Care Rule, 10A NCAC 41C .1001 through .1007 and 15A NCAC 18A .2816 require all child care facilities (homes and centers) to test for lead in drinking, water, identify lead-based paint hazard, and mitigate or restrict access to identified hazards. The Clean Classrooms for Carolina Kids program is designed to identify and eliminate exposure to lead and asbestos hazards in building infrastructure, while providing child care facilities with the support needed to meet all rule requirements. The program is free, and the first step is to watch one of the pre-enrollment webinars, which are available at www.cleanwaterforUSkids.org/carolina or paste https://bit.ly/3CK-webinar into your web browser. Additional information can be found in the RaiseNC newsletter sent on December 7, 2023. You are required to complete at least ten (10) in-service training annually. You should complete your hours by March 23, 2024. You are also required to complete a professional development plan annually. This should be completed March 23, 2024. Your health questionnaire should be completed by March 2, 2024. This program currently operates with a temporary license effective March 23, 2023 to September 23, 2023. Applicable childcare requirements were discussed with Ms. Kimberly Martin, Operator. All indoor and outdoor areas of the home approved for child care were monitored during today’s visit. One child, three (3) years old child was present; he is the operator’s grandson. The child was observed during free play activities. Program records, operator file, and one new child’s file was monitored during today’s visit. For lunch the child was served meat balls, mashed potatoes and gravy, mixed fruit, and milk. The following was also observed during today’s visit: capacity; CPR/FA and SIDS; storage of hazardous products; adequate/approved space; posted license and permit restrictions. According to Ms. Martin there has been no change to her contact information. At the completion of the visit, the Visit Summary was reviewed with Ms. Martin. Violation Number Comment Rule 714 Monthly check for hazards on the outdoor play area was not completed using a form supplied by the Division. According to the operator she is conducting the inspections monthly, but she has not documented it monthly. The last inspection was documented on March 3, 2023. 10A NCAC .1721(e)(5)(A-F) 1718 The written plan of care was not given and explained to parents of children in care on or before the first day the child attended the home. Parents did not sign a statement acknowledging the receipt and explanation of the plan. Parents did not give written permission for their child to be transported by the operator for specific routine tasks that are included on the written schedule. This was not on file for one child enrolled. .1712(e )(6) The violation/s documented above must be corrected immediately. Rated License Assessment: Ms. Martin will continue to receive subsidy payments at a 3star market rate until her Temporary License expires on September 23, 2023. In order to continue to be eligible for subsidy she will have to transition to at least a 3star license. Ms. Martin stated that she does not want to apply for a higher star license at this time and she understands that she will receive a 1star license. She also stated that she understands she will no longer be eligible for subsidy. She has one child that receives subsidy. She stated that she has talked with the parent about this. I discussed with Ms. Martin that once she receives her 1star license she can apply for a higher star license at any time. I also discussed with Ms. Martin that I cannot process her 1star license until all violations have been corrected and I have received her compliance letter. Technical Assistance: Your required In-service training hours were discussed with you along with how to calculate in-service training hours. All in-service training hours should be logged on the On-Going Training Documentation Form. Copies of certificates should be kept in your file and available for review by a representative of DCDEE. You are required to complete health and safety trainings by March 23, 2024. The health and safety training shall include the following topic areas: (1) Prevention and control of infectious diseases, including immunization; (2) Administration of medication, with standards for parental consent; (3) Prevention of and response to emergencies due to food and allergic reactions; (4) Building and physical premises safety, including identification of and protection from hazards that can cause bodily injury such as electrical hazards, bodies of water, and vehicular traffic; (5) Emergency preparedness and response planning for emergencies resulting from a natural disaster, or a man-caused event; (6) Handling and storage of hazardous materials and the appropriate disposal of biocontaminants; (7) Precautions in transporting children, if applicable; (8) Prevention of shaken baby syndrome, abusive head trauma, and child maltreatment; (9) CPR and First Aid training as required in Rule .1102(c) and (d) of this Chapter; (10) Recognizing and reporting child abuse, child neglect, and child maltreatment; and (11) Prevention of sudden infant death syndrome and use of safe sleeping practices.
