The last Annual was conducted on February 15, 2024. The Annual Compliance Monitoring Checklist for Child Care Centers was used during the visit to note items that were monitored. Ms. Marilyn McLean, Site Director, accompanied me during today’s visit. You currently operate with a 3-star rated license issued 6-17-2019 with 2 points earned in Program Standards, and 5 points in Education Standards. Restrictions: 1st shift, school age only meets enhanced ratios and other: Playground does not meet DCDEE safety guidelines. All areas, indoor and outdoor, used by the children were monitored. The cafeteria is used for PrimeTime. Twenty (20) children were present with two caregivers. Upon reviewing the staff files, it was verified that Ms. Bonnie Mack did not have a criminal background qualification letter on file nor was one completed. Ms. Mack was present during today’s visit. I informed Ms. Mack that she could not be on the premises and providing care without a criminal background qualification letter and could not return until a qualification letter was obtained. Ms. Mack immediately left the cafeteria area. Staff/child ratios, group size, capacity, use of space, and supervision were observed. I observed children washing hands upon snack and completing homework and participating in teacher-directed activity. A sanitation inspection was completed 8-30-2024 with 4 demerits and a superior rating. A fire inspection was completed on 9-12-2024. The Emergency Drill log was reviewed during today’s visit. The last shelter-in-place/lockdown drill was completed on January 23, 2023. All staff files were reviewed during the visit and the staff and training worksheet was completed. Three children’s files were reviewed. Four violations of child care requirements were observed and documented. Due to internet connectivity issues, today’s visit could not be completed in the DCDEE Regulatory database. A WORD document was completed and printed at the conclusion of today’s visit. Violation Number Comment Rule 1041 Prior to employment a Criminal Background Check was not completed. Two staff did not have verification of completing a criminal background check. G.S. 110-90.2(b) 1314 Emergency information did not name child's health care professional. One child's emergency information on the PrimeTime application did not list the child's health care professional. .0802(c)(2) 1757 A valid qualification letter was not on file and available to review at the facility. Two staff did not have a valid qualification letter on file. G.S. 110-90.2(b) & (d) & .2703(e) 1811 Shelter-in-place or lockdown drills were not practiced every three months and/or drill record was incomplete. The last documented shelter-in-place or lockdown drill was January 23, 2023. .0604(u);.0302(d)(8) The violations must be corrected immediately. You are required to submit a letter of compliance that states how each violation was corrected. The compliance letter must be received by February 25, 2025. You may email the letter to: Miriam.Byrd@dhhs.nc.gov In regards to the criminal background qualification letters, Ms. Mack can not return to PrimeTime until a qualification letter has been obtained and on file. You must notify me of when Ms. Mack has the qualification letter and send me a copy of the letter. Ms. A. Richardson (was not present today), also can not return until a qualification letter is on file. You are required to maintain compliance with all applicable child care rules and regulations at all times. NC GS 110- 90(4)(d) requires all child care facilities to maintain a compliance history score of at least 75% for the past 18 months or during the length of time the facility has operated, whichever is less. Failure to do so may result in the issuance of a provisional license or other administrative action. Child care licensing requirements were established to ensure a safe and healthy child care environment. Therefore, it is important for you to be knowledgeable of all the licensing requirements that apply to your type of program. It is your responsibility to maintain compliance with all the applicable laws and rules at all times. The best way for you to make sure that you are meeting all requirements is to periodically review the child care law and rules. I explained that you have access to the on-line version of the Child Care Rule Book, as well as a hard copy of the rules. I recommend that you visit the Division of Child Development's website (www.ncchildcare.nc.gov) on a regular basis to find out what's new in childcare. You could also find any needed Child Care forms by going on the web site and clicking on the "Provider" tab and then clicking on the "Provider Documents" tab.
