Upon my arrival, I was greeted by the Teachers, they assisted me throughout the visit. I conducted a walkthrough of all classroom spaces, documented enrollment, attendance records, and completed observations. The children were observed participating in free play in centers in their classrooms. The last annual compliance visit was conducted on February 13, 2025. The center is currently operating with a 5-star license issued on September 9, 2019 . The most recent sanitation inspection was conducted on September 9, 2025, with the center receiving a Superior rating. Fire drills were reviewed. Three staff files were monitored along with five children’s files. Your compliance history is currently 95% this percentage may change based on the results of today’s visit. During today’s visit, a full assessment of applicable Child Care Requirements was conducted. I observed the following violations during today’s visit. Violation Number Comment Rule 428 A current activity plan was not posted for each group of children for reference. Two of the three spaces did not have a current lesson plan and the plans were not posted. GS 110-91(12); .0508(a) 812 Electrical outlets and power strips, not in use, which were located in space used by children did not have safety outlets or were not covered with safety plugs unless located behind furniture or equipment that cannot be moved by a child. Outlets were not covered in spaces A12 and A7 10A NCAC 09 .0604(c) 859 Monthly playground inspections were not completed and/or they were not completed by an individual trained in playground safety requirements.Playground inspections were not accessible. .0605(q) 898 All electrical appliances were not used in accordance with the manufacturers instruction. Appliances with heating elements, such as bottle warmers, crock pots, curling irons, irons, coffee pots, and/or their cords were accessible to preschool-age children. In space A12, there was a glue gun accessible to the children. The gun glue was not hot. .0604(e) 1043 All staff records, except financial records, were not made available for review.All staff files were not accessible and the staff and training worksheets were not completed. G.S. 110-91( 9) The above violations must be corrected immediately, and a letter must be sent to me no later than November 21, 2025 stating how each item was corrected and a plan to maintain compliance with the requirement. Starting later this year, providers will be able to choose from three star-rating pathways—Program Assessment, Classroom & Instructional Quality, or Accreditation—allowing you to align your strengths with the rating method that best fits your program. Now is the time to evaluate your current practices, staff credentials, curriculum use, and classroom interactions to determine which pathway is the best fit. Programs are encouraged to begin preparing for updates to staff qualification standards and assessment tools. While existing star ratings will remain valid for now, all new star ratings will be subject to the new system. Your program’s quality should reflect not just compliance—but a commitment to best practices, child development, and continuous improvement. Stay informed, stay proactive, and take advantage of technical assistance and training opportunities as they become available. Technical Assistance During today’s visit, several classrooms were observed, and a few key areas were identified that require immediate attention to ensure compliance with NC Child Care Rules and promote program quality. 1. Access to Program Records Several teachers were absent today; however, regular staff members should have access to all program documentation, including playground inspection records, fire and emergency drill logs, and daily attendance sheets. It is recommended that these records be stored in a designated binder or file box accessible to classroom teachers in case of a visit or emergency. 2. Staff and Training Documentation Please complete and return a Staff and Training Worksheet for each classroom by November 14, 2025. This documentation should include each staff member’s name, position, qualification, hire date, and completion dates for required trainings such as CPR/First Aid, ITS-SIDS (if applicable), and Emergency Preparedness and Response. 3. Emergency Medical Care Plans The Emergency Medical Care Plan must be visibly posted in every classroom, typically near the first aid kit or emergency exit. Ensure that all staff are familiar with the plan’s contents, including procedures for contacting emergency services and notifying parents. 4. Lesson Plans Lesson plans were not current or posted in two classrooms. Plans should be dated, individualized, and aligned with the developmental needs of the children enrolled. Review and update lesson plans weekly to reflect current activities and learning goals. 5. Electrical Safety All electrical outlets must be covered when not in use or made inaccessible to children. This includes outlets located in surge protectors or multi-outlet extensions. Regularly check that safety caps are securely in place and have not been removed during cleaning or maintenance. 6. Equipment with Heating Elements Any equipment or appliances with heating elements (such as coffee makers, crockpots, bottle warmers, or space heaters) must be kept out of children’s reach and located in areas not accessible during daily routines. Upon my arrival I was greeted by the Teachers, they assisted me during today’s visit. I completed a walk-through of the classroom spaces, enrollment and attendance was documented, and observations were made. The children were observed participating in free play in their classrooms. The last annual compliance visit was conducted March 7, 2024. The center is operating five star license that was issued September 9, 2019. The license was earned with 7 points in program standards, 7 points in education and 1 quality point. Permit restrictions were in compliance. The last sanitation inspection was conducted October 8, 2024 the center received a Superior rating. Fire drills and playground inspections were monitored. Staff files and three children’s files were monitored. NC Pre-k requirements were monitored. Developmental Day requirements were monitored. The facility currently operates with a Five Star License that was issued on September 9, 2019 . The last annual compliance visit was conducted on March 7, 2024. The Teachers were present and assisted me during the visit. Children were observed participating in free play outside on their designated playgrounds, the teachers were observed moving about the playground interacting and supervising the children. One group of children was observed participating in free play in their classroom, the teachers were observed interacting in a positive manner with the children. The last sanitation inspection was completed April 30, 2024 with a “Superior” classification. There were no new staff with the exception of two long term subs. During today’s visit, a partial assessment of applicable child care requirements was conducted. I observed the following violations. Violation Number Comment Rule 812 Electrical outlets and power strips, not in use, which were located in space used by children did not have safety outlets or were not covered with safety plugs unless located behind furniture or equipment that cannot be moved by a child. There was two open outlets that was not covered. 10A NCAC 09 .0604(c) 860 Balloons were accessible to children. There was a bag of balloons inside a container sitting on a low counter that was accessible by the children. .0604(q) No corrective action letter is required the violations were corrected during today’s visit. You were provided an opportunity to ask questions regarding interpretation and implementation of all child care requirements. Child care licensing rules and regulations are established to ensure a safe and healthy environment for children in care. Therefore, it is important for you to be knowledgeable of all of the licensing requirements that apply to your program. It is your responsibility to maintain compliance with all of the applicable laws and rules at all times. The best way for you to make sure that you are meeting all requirements is to periodically review the child care law and rules. Both are available on the DCDEE website https://ncchildcare.ncdhhs.gov/ , on the home page under the ‘Services’ tab. Technical assistance I spoke with the lead teacher in regards to the two violations, outlets must be covered when not in use. Balloons cannot be used with children. 10A NCAC 09 .0604 SAFETY REQUIREMENTS FOR CHILD CARE CENTERS (q) Plastic bags, toys, toy parts small enough to be swallowed, and materials that can be torn apart, such as foam rubber and styrofoam, shall not be accessible to children under three years of age. However, styrofoam plates and larger pieces of foam rubber may be used for supervised art activities and styrofoam plates may be used for food service. Jump ropes and rubber bands shall not be accessible to children under five years of age without adult supervision. Balloons shall be prohibited for children of all ages. I spoke with the principal and reviewed my visit with her, I will email her the pre-service form following today's visit. RESOURCES The Health and Safety Trainings are offered on the DCDEE Moodle format. On the Division’s website look under the “SERVICES” tab for “DCDEE Moodle”.
