A checklist was used to note the childcare requirements monitored today, and a copy was left with you. The last annual compliance visit was conducted 3/28/25. Currently this program operates with a four (4) star license on 4/4/19. The facility earned six (6) points in program standards, four (4) points in staff education, and one (1) quality point for education options met. Staff member Amanda Rochelle assisted me today. I conducted an unassisted walkthrough of the center. The last sanitation inspection was completed 8/29/25 with a ‘Superior’ classification and 12 demerits. The last fire inspection was conducted on 3/3/26. Children were observed playing in activity areas, large group activities, playing outside, eating lunch, resting, and involved in routine care activities. One mini-bus CRP 1963 was on site and monitored today. The other mini-bus was off-site. I will schedule a time to stop by and monitor this one. A full assessment of applicable childcare requirements was conducted today. The following violation(s) were documented and technical assistance regarding maintaining compliance was offered. Violation Number Comment Rule 808 The child care center premises, including the outdoor learning environment, was not clean, drained to minimize standing water, free of litter and hazards, and/or maintained in a manner which does not create conditions that attract or harbor pests. The pink house had chipping paint and a piece of a broken board with jagged edges. The buddy bench had an exposed nail. The top bolt on one of the yellow rollers was exposed. 15A NCAC 18A .2832(a) 1031 Documentation of staff's education, training, and experience was not on file. Staff member S. Kirkman was short .5 training hours. .0302(d)(1)(B) 1124 Emergency and identifying information, including the child's name, photograph, emergency contact information and/or a copy of the emergency medical care information form was not in the vehicle for each child being transported. The photos for 19 children were not in the emergency notebook. 10A NCAC 09 .1003(d) You were provided an opportunity to ask questions regarding interpretation and implementation of all childcare requirements. Each violation cited was discussed with Ms. Rochelle. The violation(s) documented above must be corrected immediately. A signed and dated letter of compliance must be received on or before 4/7/26. The letter must include the following: *Facility Name and ID number *List of each violation number *How each violation was corrected All items must be included, or the corrective action letter will be returned for corrections. CONSULTATION AND RESOURCES: Rated License Assessment: During today’s visit, you indicated you are choosing pathway one. We reviewed pathway one and two and reviewed the requirements for maintaining a four-star rated license today. The lead teacher needs an ECERS 3 book to use for the self-study and in preparing the room. You will be required to complete the ERS assessments. The lead teacher and group leader will complete the self-study on the NCRLAP website. We discussed the timeline on this should be April-June or May-July. The study will help staff prepare for the assessment. We reviewed the current staff education, and it appears you would be at the four-star level for education. All staff must be in WORKS. P. Hall should have her education evaluated for Program Coordinator and Administrator. We reviewed the family and community engagement standards today. For four stars, you will choose three additional options, one from each category. A copy of the form was left with you today. We reviewed the facility and staff continuous quality improvement plans today. Copies of these forms were left with you. Items 1-4 should be completed at the time of the completion of the rated license. I will monitor for recorded progress on the goals during monitoring visits. Technical Assistance: I observed the preschool classroom today and we discussed the following items: • Moving the cots outside the classroom to provide more space for the activity areas • Activity areas should be more defined • At least one third of items on the walls should be child-led art • Background music should not be played • A sand/water table needs to be added • Pictures should be added to the room displaying different cultures, abilities, and non-gender specific roles I took pictures of the room arrangement I the preschool and school-age rooms today. We will schedule a TA visit to discuss room arrangement ideas and the addition of materials. You stated you are having difficulty obtaining CPR training for staff. I emailed you a list of approved trainers to contact for the training. We discussed the outdoor area today. The large tractor tires must be checked daily, and all debris removed from them. The painted structures must be checked for peeling paint and other hazards. You stated you contacted regarding your EPR plan. The plan must be completed by June 1, 2026 to complete the plan and train the staff. Upon my arrival I was greeted by the Owner, Pam Hall, she assisted me during today’s visit. The Administrator, Amanda Rochelle, arrived later during the visit. I completed a walk-through of the classroom spaces, enrollment and attendance were documented, and observations were made. The children were observed participating in free play in their classrooms and outside. The school age children were in school. The last annual compliance visit was conducted May 16, 2024. The center is operating under a Four star license that was issued April 4, 2019. The license was earned with 6 points in program standards, 4 points in education and 1 quality point. Permit restrictions were in compliance. The last sanitation inspection was conducted September 13, 2024, the center received a Superior rating. Fire drills and playground inspections were monitored. The last fire inspection was conducted on March 19, 2025. Two new staff files and five children’s files were monitored. The vehicles used for transportation of school age children were not on site for monitoring. Your compliance history is 87%, this may change following today’s visit. During today’s visit, a full assessment of applicable Child Care Requirements was conducted. The following violations were observed during today’s visit. Violation Number Comment Rule 526 Menus for all meals and snacks were not current or posted where easily seen by parents and cook. A current menu was not posted. 10A NCAC 09 .0901(b) 847 Parent's medication authorization did not include required information. The medication administration forms was not current. 10A NCAC 09 .0803(4)(6-9) 849 Leftover medicines were not returned to the parent after the course of treatment was completed, after authorization was withdrawn or after authorization had expired and/or medication was not discarded within 72 hours of completion of treatment or withdrawal of authorization. There was a prescription medication for nausea that was no longer being used by the child. .0803(12) 1834 Application did not have a medical action plan attached for any child with health care needs such as allergies, asthma, or other chronic conditions that require specialized health services. A child with Asthma did not have a medical action plan on file. .0801(b) 1897 The child care administrator and all staff did not complete the Recognizing and Responding to Suspicions of Child Maltreatment training within 90 days of employment. The facility currently operates with a Four Star License that was issued on April 4, 2019. The last annual compliance visit was conducted on May 16, 2024. The Lead Teacher and Owner, were present and assisted me during the visit. Children were observed having group time and interacting with the teacher. The last sanitation inspection was completed September 21, 2024 and earned a “Superior” classification. During today’s visit, a partial assessment of applicable child care requirements was conducted. I observed the following violations. Upon my arrival I was greeted by the Director and Owner, they assisted me during today’s visit. I completed a walk-through of the classrooms, attendance was documented, and observations were made. The children were observed participating in free play outdoors, arriving from school and leaving the center for the day. The last annual compliance visit was conducted May 25 2023. The center is operating under a Four Star License that was issued April 4, 2019. The last sanitation inspection was conducted April 1, 2024 the center received a Superior rating. The last fire inspection was conducted February 21, 2024. Fire drills and playground inspections were monitored. Staff and five children’s files were monitored. During today’s visit, a full assessment of applicable Child Care Requirements was conducted. The following violations were observed during today’s visit. (b) The facility shall develop a plan of activities which is posted in a place in the home base or given to the parents. The plan shall include the location, purpose, time and date, person in charge, and telephone number or method for contacting the person in charge. (c) Activities shall be planned to accommodate a variety of individual interests and shall provide opportunities for choice. Upon my arrival I was greeted by the Director and Owner, they assisted me during today’s visit. I completed a walk-through of the classrooms, attendance was documented, and observations were made. The children were observed participating in free play outdoors, arriving from school and leaving the center for the day. The last annual compliance visit was conducted May 25 2023. The center is operating under a Four Star License that was issued April 4, 2019. The last sanitation inspection was conducted April 1, 2024 the center received a Superior rating. The last fire inspection was conducted February 21, 2024. Fire drills and playground inspections were monitored. Staff and five children’s files were monitored. During today’s visit, a full assessment of applicable Child Care Requirements was conducted. The following violations were observed during today’s visit. A fire drill was not documented for April 2024. .0604(t); .0302(d)(5) 812 Electrical outlets and power strips, not in use, which were located in space used by children did not have safety outlets or were not covered with safety plugs unless located behind furniture or equipment that cannot be moved by a child. There is an outlet on the front wall that has an uncovered outlet in the classroom for school age children. In the classroom for preschool age children there in a wall outlet that is uncovered and a power strip with open outlets. 10A NCAC 09 .0604(c) 898 All electrical appliances were not used in accordance with the manufacturers instruction. Appliances with heating elements, such as bottle warmers, crock pots, curling irons, irons, coffee pots, and/or their cords were accessible to preschool-age children.In the classroom for Preschool children there was a laminator sitting on a shelf. .0604(e) 1792 Staff did not model appropriate eating behaviors by consuming food or beverages that meet the nutritional requirements specified in the Meal Patterns for Children in Child Care Programs in the presence of children in care. There was a large cup of McDonald's soda sitting on a table outside on the playground. .0901(i) The above violation must be corrected immediately, and a letter must be sent to me no later than May 28, 2024 stating how each item was corrected and a plan to maintain compliance with the requirement.
