A checklist was used to note the childcare requirements monitored today, and a copy was left with you. The last annual compliance visit was conducted 2/24/25. Currently this program operates with a three (3) star license on 8/15/17. The facility earned two (2) points in program standards, four (4) points in staff education, and one (1) quality point for programmatic options met. Center assistant directors Tiffany Crozier and Amanda Allred assisted me today. I conducted an assisted walkthrough of the center. The last sanitation inspection was completed 8/8/25 with a ‘Superior’ classification and 12 demerits. The last fire inspection was conducted on 3/19/25. Children were observed playing in activity areas, large group activities, eating lunch and involved in routine care activities. A full assessment of applicable childcare requirements was conducted today. The following violation(s) were documented and technical assistance regarding maintaining compliance was offered. Violation Number Comment Rule 106 Operator has not scheduled and obtained a fire inspection within 12 months of the previous inspection. Operator did not submit the original approved report to DCDEE within one week of the inspection visit on a form provided by the Division. The current inspection was dated 3/19/25. The previous inspection was dated 2/29/24. 10A NCAC 09 .0304(a) 1033 On or before the first day of work, all staff, including the director and individuals who volunteer more than once per week did not provide results indicating that they were free of active TB and/or TB test or screening was older than 12 months. The TB test on file for D. DeCeasare was older than twelve months on date of hire. .0701(a) 1052 Staff required to receive on-going training had not completed the required number of hours according to their education and experience. Staff member T. Crozier was short seven hours, A. Allred was short eight hours, and A. Beeson was short seven hours. .1103(a) 1897 The child care administrator and all staff did not complete the Recognizing and Responding to Suspicions of Child Maltreatment training within 90 days of employment. The training on file for D. DeCeasare was dated 5/10/22. Her hire date was 10/20/25. .1102(g) 1901 All administrators and staff did not complete a professional development plan within one year of employment, that included all the required information. Staff member H. Russell did not have a plan on file. .1104(1-5) You were provided an opportunity to ask questions regarding interpretation and implementation of all childcare requirements. Each violation cited was discussed with Ms. Allred. The violation(s) documented above must be corrected immediately. A signed and dated letter of compliance must be received on or before 3/3/26. The letter must include the following: *Facility Name and ID number *List of each violation number *How each violation was corrected All items must be included, or the corrective action letter will be returned for corrections. CONSULTATION AND RESOURCES: Rated License Assessment: During today’s visit, you indicated you are choosing pathway one. We reviewed the requirements for maintaining a three-star rated license today. You will be required to complete the ERS assessments. The scores will be used to develop your CQI plan for your facility. You may reach out to schedule a practice assessment with NCRLAP. You indicated today that you are hiring teachers for some of the classrooms. You should wait to request assessment until staff have been hired and are settled into their classrooms. All lead teachers and the administrator must complete the self-study review and use the forms on the NCRLAP website. Staff are required to complete the self-assessment tool in the back of the ITERS-3 and ECERS-3 books. You stated you are in the process of ordering the books now. Every classroom lead teacher should complete this. They will use this information for their three-month self-study. A copy of the administrator’s self-study form was reviewed and left with you today. We discussed visiting the QRIS Modernization page on the NC Childcare website, https://ncchildcare.ncdhhs.gov, under the ‘What’s New’ tab. In addition, the NCRLAP website, https://ncrlap.org, has many resources and trainings for the ITERS3 and ECERS3 assessments. Once all staff are in place, I can evaluate for your education. All staff must be registered into the WORKS system and have all education submitted and evaluated for their positions. You can email me the staff assigned to each classroom, their birthdays, and years of experience. I will evaluate these and follow-up with you. We reviewed the family and community engagement standards today. For three stars, you will choose two additional items from the three categories. A copy of the form was left with you today. We reviewed the facility and staff continuous quality improvement plans today. Copies of these forms were left with you. Items 1-4 should be completed at the time of the completion of the rated license. I will monitor for recorded progress on the goals during monitoring visits. Technical Assistance: We attempted to login to your EPR plan today but could not locate it. You may contact Amia Eaton at 919-814-6365. Explain that you are trying to access your plan and need assistance. She will help you. Staff who are short on annual training hours are required to complete the hours they are short. In addition, they are required to complete their required number of hours for the 2025-2026 training year. If K. Barillas works in a classroom she will be required to complete the health and safety trainings. Recognizing and Responding to Suspicions of Child Maltreatment will be due within 90 days of her start in a room. The remainder of the trainings will be due within one year. Medical an TB tests can be used for new hires if they are not older than twelve months on date of hire. This also applies to the health and safety trainings. A new fire inspection must be completed by 3/18/26 and a copy sent to me within seven days. The facility currently operates with a Three Star License that was issued on August 15, 2027 . The last annual compliance visit was conducted on February 24, 2025. Upon my arrival I was greeted by the owner. The Director, Amanda Rochelle arrived later during the visit and assisted me during the visit. One group of children were observed participating in free play outside on their designated playground, the teachers were observed moving about the playground interacting and supervising the children. One group of children was observed participating in free play in their classroom, the teachers were observed interacting in a positive manner with the children. The infants were observed participating in free play and one was observed sleeping. The last sanitation inspection was completed February 24, 2025 with a “Superior” classification. The last fire inspection was conducted March 19, 2025. During today’s visit, a partial assessment of applicable child care requirements was conducted, including supervision, child staff ration and permit re4strictions. I observed the following violations. Violation Number Comment Rule 805 Fire drills were not practiced monthly and/or the drill record was incomplete. No fire drills were documented for April and May of 2025. .0604(t); .0302(d)(5) 859 Monthly playground inspections were not completed and/or they were not completed by an individual trained in playground safety requirements. Playground inspections were not available for review. .0605(q) 1041 Prior to employment a Criminal Background Check was not completed. The teacher G. Hall did have a criminal background check completed. G.S. 110-90.2(b) The violation(s) must be corrected immediately, and a letter must be sent to me no later than June 24, 2025 stating how each item was corrected and how you plan to maintain compliance with the requirement. Child care licensing rules and regulations are established to ensure a safe and healthy environment for children in care. Therefore, it is important for you to be knowledgeable of all of the licensing requirements that apply to your program. It is your responsibility to maintain compliance with all of the applicable laws and rules at all times. The best way for you to make sure that you are meeting all requirements is to periodically review the child care law and rules. Both are available on the DCDEE website https://ncchildcare.ncdhhs.gov/ , on the home page under the ‘Services’ tab. The center was issued a written warning on April 24, 2025. The Director has scheduled the required technical assistance visit for June 19, 2025. The Director may not be able to attend the technical assistance training because she may be out on maternity leave, she stated she will attend if she is able. Technical assistance During today’s visit I spoke with the Owner, Pam Hall and Assistant Director, Tiffany Crozier, they both stated they are short staffed. They also stated they have had issues accessing the criminal background check portal. I contacted the CBC unit during today’s visit and there have been no reported issues with the system. I had no issues contacting the Criminal Records unit by phone today. Please contact 919-814-6401 for assistance with criminal background checks. Whenever you have issues with the CBC system contact them immediately. One teacher did not have a criminal background check on file. No person can work in licensed childcare without a criminal background check from the Division of Child Development and Early Education. The staff person Gracyn Hall cannot return to work until she has a criminal background check and qualifying letter on file. There was no fire drill documented for April or May of 2025. Fire drills must be conducted and documented monthly. Playground Inspections were not onsite. I would strongly recommend not taking program documents off site. The Director has started the staffing pattern that is required as part of the Written Warning, we discussed some things that should be included in the pattern. RESOURCES The Health and Safety Trainings are offered on the DCDEE Moodle format. On the Division’s website look under the “SERVICES” tab for “DCDEE Moodle”. These trainings are free, and participants will receive training certificates when completed. Upon my arrival I was greeted by the Owner Pam Hall. I observed 15 children ages 1 year to 3 years with three staff in space 5. The one year old children were observed in high chairs and two were being held by staff. The children were also observed watching television. During the complaint visit conducted on February 24, 2025, allegations of staff child ratio and program records were substantiated. The following violations were observed during today’s visit. Violation Number Comment Rule 301 Minimum staff/child ratios and group sizes for the number and ages of children in care were not met. There was 15 children ages 1 to 3 years grouped together in space 5. This is a repeat violation from 2/24/2025. GS 110-91(7);.0713(a-d) 544 Screen time was offered to children under three years of age. Children ages 1 and 2 years were observed watching television in space 5. .0510(f) The above violation must be corrected immediately, and a letter must be sent to me no later than March 14, 2025 stating how each item was corrected and a plan to maintain compliance with the requirement. The facility holds a Three-Star License, issued on August 15, 2017. The last annual compliance visit was conducted on March 12, 2024. The Director, Amanda Rochelle, and Owner, Pam Hall, were present and assisted me. I informed them of the reason for my visit. During my visit, I observed children engaged in free play in their classrooms and interacting with their teachers. Allegation 1: There is concern regarding supervision Observation: All classrooms were supervised adequately during my visit. Finding The allegation regarding supervision is unsubstantiated based on today’s observations. Allegation 2: There is concern regarding staff-child ratio Observation: Classrooms were within the required ratios during my visit. However, after reviewing arrival times for children and staff, the center was out of ratio from 7:00 AM to 8:00 AM. Infants (6 infants, 1 teacher) were out of ratio and One-year-olds (18 children, 3 staff), group size was out of compliance. The group size should not exceed 12. Finding The allegation regarding staff child ratio is substantiated based on today’s observations. Allegation 3: There is concern regarding records Observation: Incident reports were not completed when required. Finding: The allegation regarding records is substantiated based on today’s observations. Allegation 4: There is concern regarding Developmentally Appropriate Environment Observation: Classrooms had appropriate materials, activities, interactions, and setup. Finding: The allegation regarding developmentally appropriate environment is unsubstantiated based on today’s observations. Two violations were documented during today’s visit. Violation Number Comment Rule 301 Minimum staff/child ratios and group sizes for the number and ages of children in care were not met. Ratios were not in compliance from 7:00am to 8:00 am based on arrival times for the children and the number of staff present during that time frame. GS 110-91(7);.0713(a-d) 852 Incident reports were not completed each time a child was injured, it did not include all the information required in rule, it was not signed by the parent and/or it was not maintained in the child's file. In each classroom, there was a incident log documenting when children were injured however many of the incidents were not documented on incident reports. .0802 (e) ________________________________________ The violations must be corrected immediately. A written corrective action plan must be submitted by March 10, 2025 to mona.ndiaye@dhhs.nc.gov, explaining how each issue was addressed and how compliance will be maintained. The facility currently operates with a Three Star License that was issued on August 15, 2017 . The last annual compliance visit was conducted on March 12, 2024. The Directors were present and assisted me during the visit. Children were observed participating in free play inside their classrooms and participating in short group times and stories. The children were observed eating lunch which consisted of chicken alfredo, pears, green beans and milk. The last sanitation inspection was completed May 23, 2024 with a “Superior” classification. The last fire inspection was conducted February 29, 2024. During today’s visit, a partial assessment of applicable child care requirements was conducted. I observed the following violations. Violation Number Comment Rule 716 All stationary equipment, more than 18 inches high, was not installed over protective surfacing. There are swings and a stationary climber that does not have at least 6 inches of mulch in the required areas. .0605(j) 721 All equipment and furnishings were not in good repair. On the playground for preschool aged children, there was a sit and spin toy that two children could use at once, the middle part the children use to spin themselves was broken off. G.S. 110-91(6); .0601(b) 841 Medications including prescription and non-prescription items were not stored in a locked cabinet or other locked container. There were medications stored in a floor level cabinet that was unlocked int he classroom for toddlers. This is a repeat violation from 3/12/24. 15A NCAC 18A .2820(d) 844 Prescribed medicine was not in original labeled container or accompanied by signed and dated written instructions from prescribing physician or health care professional. There was Nystatin creams that did not have prescription labels. This is a repeat violation from 3/12/24. .0803(2)(a) 847 Parent's medication authorization did not include required information. Several medications were incomplete, outdated or missing. This is a repeat violation from 3/12/24. 10A NCAC 09 .0803(4)(6-9) 1045 New staff, who had contact with children, did not receive at least 16 hrs. orientation within first 6 weeks. Four new staff did not have completed orientations on file. .1101(a) 1048 All staff did not successfully complete certification in First Aid appropriate to the age of children in care. Verification of staff completion of First Aid training from an approved training organization was not in the staff file. One staff did not have current First Aid training. .1102(c) 1049 All staff did not successfully complete certification in CPR training appropriate to the age of the children in care. Verification of staff completion of the CPR course from an approved training organization was not in the staff file. One staff did not have current CPR training. .1102(d) 1874 The Prevention of Shaken Baby Syndrome and Abusive Head Trauma policy was not reviewed with new staff prior to providing care with children and/or a signed acknowledgement with all the required information was not maintained in the staff person's file. Technical Assistance Medication parental authorizations were either not complete, missing or outdated in all the classrooms. Medication were not labeled with the child’s name. Medication was observed in an unlocked floor level cabinet with the key in the lock in the classroom for toddlers. Violations were cited during the annual compliance visit conducted on March 12, 2024 for medications violations and a corrective action letter was received stating the medication violations had been corrected. Ensure you are using the correct medication forms. There is a form for topical medication and a form for prescription medications. Visit the DCDEE website to get the forms. Several new staff files were not complete. The facility is using a checklist for the staff files however staff was still missing paperwork. The playground for the preschool aged children had less then 6 inches of mulch under the swings and stationary slide. Broken toys should be removed from the the playground. Rated License Please be reminded your facility is in cohort one. Your prep year starts July 1, 2023. Your assessment year will begin July 1, 2024 and end June 30, 2025. Ensure you are using the correct medication forms. There is a form for topical medication and a form for prescription medications. Visit the DCDEE website to get the forms. Several new staff files were not complete. The facility is using a checklist for the staff files however staff was still missing paperwork. The children were observed participating in free play in their classrooms. The last annual compliance visit was conducted March 30, 2023. The center is operating under a Three Star License that was issued August 15, 2017. The last sanitation inspection was conducted November 8, 2023 the center received an Superior rating. The last fire inspection was conducted February 29, 2024. Fire drills and playground inspections were monitored. During today’s visit, a full assessment of applicable Child Care Requirements was conducted. The following violations were observed during today’s visit.
