This visit was conducted by Turkessha Casterlow, Childcare Consultant with Dorothy McIntosh, Lead Teacher. Your program currently operates with a Five Star license. Restrictions include Daytime care only; No children in diapers; Meets enhanced ratios; Meets enhanced space; The license was observed, and the restrictions were found in compliance. The program’s compliance history was 97% prior to today’s visit. A walk-through of the facility was completed today. Indoor and outdoor areas were observed. Staff/child ratio and space capacity were monitored as indicated on the attached worksheet. Supervision was adequate during the visit. I observed staff/child interactions today. I observed children during free time and transitioning to go out and explore their outside learning environment. I observed materials and activities that were available to the children in the classrooms. Your program was also monitored for compliance with implementing an approved curriculum as required for all four- and five-star licensed facilities where four-year-old children are enrolled. Your facility uses Creative Curriculum. The last annual compliance visit was conducted on April 29, 2025. A sanitation inspection was completed on October 13, 2025, with a Superior classification. The last fire inspection was conducted on February 10, 2026. Program records and required postings were monitored. A fire drill was conducted on March 30, 2026. A shelter in place drill was documented on February 10, 2026. An outdoor inspection was documented on April 1, 2026. Staff files and children’s records were monitored per DCDEE procedures. Nutrition requirements were monitored per DCDEE procedures. Storage of hazardous items was monitored today. Storage and administration of medication were monitored. Medication authorization was monitored. The program does not provide transportation. The following violations were cited during today’s visit: Violation Number Comment Rule 106 Operator has not scheduled and obtained a fire inspection within 12 months of the previous inspection. Operator did not submit the original approved report to DCDEE within one week of the inspection visit on a form provided by the Division. This visit was conducted by Turkessha Casterlow, Childcare Consultant with Dorothy Mcintonsh, Lead Teacher. Your program currently operates with a Five Star license effective October 23, 2023. Restrictions include Daytime care only; No children in diapers; Meets enhanced ratios; Meets enhanced space. The program’s compliance history was 97% prior to today’s visit. I observed children participating in free play, routine care activities, and playing in their outside learning environment. One new staff member was reported at this program. Files for new staff were reviewed. Limited monitoring of staff files was conducted for the purpose of reviewing training regarding CPR/First Aid, EPR, Playground Safety, ITS-SIDS, Special Training including Health and Safety Training and Criminal Background Checks. Storage and administration of medication were monitored. Medication authorization was monitored. Storage hazardous items were monitored today. The program began operating for the new school year on September 2, 2025. Please be sure to complete a fire drill, playground inspection, and emergency drill before September 30, 2025 The most recent fire inspection was on December 12, 2024. I observed a sanitation inspection was last conducted on March 18, 2025 earning a Superior classification. The program does not provide transportation. The following violations were cited during today’s visit: Violation Number Comment Rule 812 Electrical outlets and power strips, not in use, which were located in space used by children did not have safety outlets or were not covered with safety plugs unless located behind furniture or equipment that cannot be moved by a child. Outlets on the right wall by the cubbies and the two power strips by the teacher's desk had uncovered outlets. 10A NCAC 09 .0604(c) 1032 Child care providers and uncompensated providers who are not substitute providers or volunteers, including the director did not have a medical report on file prior to employment that was signed by a health care professional and/ or the medical report was older than 12 months. Ortiz was not available for review. 10A NCAC 09 .0701(a) 1033 On or before the first day of work, all staff, including the director and individuals who volunteer more than once per week did not provide results indicating that they were free of active TB and/or TB test or screening was older than 12 months. The screenings identify individuals with latent or active infections, allowing for early treatment and reducing transmission in a high-contact environment. This protects both the children and the community while ensuring the workplace is safe and compliant with public health regulations. • Updating emergency care plans in child care is crucial for child safety, ensuring preparedness for changing risks, and meeting legal requirements. Regular updates guarantee that plans are relevant to current community threats, facility changes, and each child's unique, evolving needs, especially for infants and children with special health conditions. An up-to-date plan also ensures staff are well-trained, supplies are current, and parents are informed, fostering confidence and effective communication during an emergency. • The process of notifying the Division of new employees has changed and is now captured in ABCMS. This change has been in effect since February 2024. Effective immediately, you will need to obtain a Business NCID and complete Provider Portal training in Moodle at https://www.dcdee.moodle.nc.gov/course/view.php?id=119. No action is needed on your part if you have completed the reference training and are currently using the ABCMS Provider Portal to update information regarding new hires or residents. Once the training has been completed and access has been given, you must verify your facility roster to ensure current staff are noted on the roster. This information should be updated in ABCMS on an ongoing basis as staff members are hired and when their employment is terminated. Technical assistance discussed during today’s visit: • TB and medical screenings are important for first-day childcare employees to prevent the spread of Tuberculosis (TB), a contagious bacterial infection, especially to vulnerable children who are at higher risk of severe illness. The screenings identify individuals with latent or active infections, allowing for early treatment and reducing transmission in a high-contact environment. This protects both the children and the community while ensuring the workplace is safe and compliant with public health regulations. • Updating emergency care plans in child care is crucial for child safety, ensuring preparedness for changing risks, and meeting legal requirements. Regular updates guarantee that plans are relevant to current community threats, facility changes, and each child's unique, evolving needs, especially for infants and children with special health conditions. An up-to-date plan also ensures staff are well-trained, supplies are current, and parents are informed, fostering confidence and effective communication during an emergency. • The process of notifying the Division of new employees has changed and is now captured in ABCMS. This change has been in effect since February 2024. Effective immediately, you will need to obtain a Business NCID and complete Provider Portal training in Moodle at https://www.dcdee.moodle.nc.gov/course/view.php?id=119. No action is needed on your part if you have completed the reference training and are currently using the ABCMS Provider Portal to update information regarding new hires or residents. Once the training has been completed and access has been given, you must verify your facility roster to ensure current staff are noted on the roster. This information should be updated in ABCMS on an ongoing basis as staff members are hired and when their employment is terminated. This satisfies the requirement to notify the Division of new childcare providers working who were hired or moved into the childcare facility within five business days. Additional Information/Consultation: • You must notify your consultant 30 days prior to changing the ownership status of your facility. • Preparing for North Carolina’s New QRIS System North Carolina’s Quality Rating Improvement System (QRIS) is being modernized to provide more flexible and equitable ways for programs to demonstrate quality. Beginning later this year, providers will be able to choose from three star-rating pathways: Program Assessment, Classroom & Instructional Quality Accreditation. This new approach allows programs to align their strengths with the pathway that best fits their operations. Now is the time to prepare. Programs should begin evaluating staff qualifications including updating staff education in NC Works, curriculum, and classroom interactions to determine the most suitable pathway. Updates to staff qualification standards and assessment tools are expected, and while current star ratings remain valid for now, all new ratings will be issued under the revised QRIS system. Your program’s quality should reflect not only compliance, but also a strong commitment to best practices, child development, and continuous improvement. Providers are encouraged to take advantage of available technical assistance and training opportunities. As you prepare for the QRIS be sure to double-check that all your staff’s education is listed in NC Works.
