A checklist was used today to note the childcare requirements monitored today, and a copy was left with you. The last annual compliance visit was conducted 12/4/24. Currently this program operates with a GS 110 Notice of Compliance issued 10/11/19. The status of the corporation is current/active. The center director, Brooke York, assisted me today. I conducted an assisted walk-through of the center. The last sanitation inspection was completed 9/29/25 with a ‘Superior’ classification and 3 demerits. The last fire inspection was conducted on 4/10/25. The last well water analysis was completed 8/29/24. Children were observed playing in activity areas, large group activities, and routine care activities. The facility does not provide transportation. A full assessment of applicable childcare rules was conducted today. The following violation(s) were documented and technical assistance regarding maintaining compliance was offered. Violation Number Comment Rule 1033 On or before the first day of work, all staff, including the director and individuals who volunteer more than once per week did not provide results indicating that they were free of active TB and/or TB test or screening was older than 12 months. The TB test for C. Maness was dated 4/10/25. She began as a volunteer o 4/2/25. The TB test for M. Staley was dated 8/2/25 and she began employment on 8/1/25. .0701(a) 1041 Prior to employment a Criminal Background Check was not completed. The qualifying letter for M. Staley was dated 8/14/25. Her date of employment was 8/1/25. G.S. 110-90.2(b) 1908 A child's file did not have a statement with parent signature acknowledging receipt and explanation of the Prevention of Shaken Baby Syndrome and Abusive Head Trauma policy and/or the acknowledgement did not have all the required information. CONSULTATION We discussed the requirements for the Emergency Preparedness Plan. The website is https://rmp.nc.gov. Each licensed childcare center is required to have a plan per childcare rule 10A NCAC 09 .0607. One staff member is required to complete the Emergency Preparedness training. This staff member is required to complete the online plan. Once you complete the plan, the plan will be reviewed with staff and documentation of the review will remain on file. The plan will be reviewed annually and documented thereafter along with the center’s Emergency Medical Care plan. You may reach out to the Randolph County Partnership for Children for the EPR training. If they do not have one, please let me know and I will assist you in locating one. You need to plan to complete the training within four months of today’s visit and then complete the plan. The facility currently operates with a Notice of Compliance that was issued on September 30, 2029. The last annual compliance visit was conducted on December 15, 2023. The Director was present and assisted me during the visit. Children were observed participating in free play in their classrooms and outside and eating a snack. The last sanitation inspection was completed November 15, 2023 with a “Superior” classification. The last fire inspection was conducted April 21, 2023. There are no new staff since the last annual compliance visit. During today’s visit, a partial assessment of applicable child care requirements was conducted. I observed the following violations. The violation(s) must be corrected immediately, and a letter must be sent to me no later than May 15, 2024 stating how each item was corrected and how you plan to maintain compliance with the requirement. Please e-mail the letter to me at mona.ndiaye@dhhs.nc.gov . Violation Number Comment Rule 106 Operator has not scheduled and obtained a fire inspection within 12 months of the previous inspection. Operator did not submit the original approved report to DCDEE within one week of the inspection visit on a form provided by the Division. The lasst fire inspection was conducted April 21, 2023. 10A NCAC 09 .0304(a) 1850 Signage was not posted regarding the smoking and tobacco restriction at the entrance of the center and/or in vehicles used to transport children. The was no signage regarding the smoking restriction on the premises. .0604(i) You were provided an opportunity to ask questions regarding interpretation and implementation of all child care requirements. Child care licensing rules and regulations are established to ensure a safe and healthy environment for children in care. Therefore, it is important for you to be knowledgeable of all of the licensing requirements that apply to your program. It is your responsibility to maintain compliance with all of the applicable laws and rules at all times.
