Today's visit was conducted with the owner/operator of Your Cousins In Home Daycare, Mrs. Mandy Cousins. The FCCH operates with a 1 STAR license issued on December 16, 2006. This FCCH operates first shift from 7:30am- 5:30pm M-F. You are licensed to care for 5 children ages 0-12 years old on the first shift. Currently you have five (5) children enrolled full-time in your family child care home between the ages of 1 yr.- 3yrs. old. Today, I completed a walk-through of the family child care space, five (5) 1yr-3yr. year old children were present during my visit. I observed the children engaged in indoor activities, playing outside, and eating lunch. The provider, Mrs. Cousins, was engaged in the children’s activities and providing appropriate supervision. During today’s visit, a full assessment of childcare requirements was conducted. A checklist was used to note the items monitored and provided to the owner/operator at the conclusion of the visit. Mrs. Cousins completed all of the required Health and Safety training in June 2023 and is required to renew her Health and Safety training by June of 2028. Emergency telephone numbers were readily accessible and emergency procedures were posted. A written daily schedule was posted. The daily schedule reflected outside time. Child Care Space: The child care space is located on the first floor of the providers split level home. There is a private entrance for child care families on the left side of the home and an area specifically for her child care program only. For outside play, the children use the fence in backyard. Safety/Inspections: Fire drills, outdoor playground inspections, and EPR drills were current and on file. The EPR plan was also observed to be current and on file. The smoke detector was in working order during my visit today and fire extinguisher were observed to be in working order during my visit today. Transportation: Mrs. Cousins is currently not approved for transportation, and she understands if she decides to provide transportation, she must contact me first so I can come out and confirm all requirements are being met prior to any child care children being transported. ANNUAL INSPECTIONS: Fire drills, shelter in place and lock down drills, playground inspections, and EPR plan were observed on file and up to date during today’s visit. *****Violations****** One Violation was cited during today’s visit. Violation Number Comment Rule 1406 Authorization for up to 6 months to administer prescription or over-the-counter medication, when needed, for chronic medical conditions and for allergic reactions was not in writing and did not contain information required by rule 10 NCAC 09 .1720(c)(6)(A-I). One child has an EPI Pen and Benadryl for emergency purposes but the required medication forms were not available for review. 10A NCAC 09 .1720(b)(4) COMMENTS COMPLIANCE HISTORY According to NC General Statute all day care centers must maintain a compliance history of at least seventy-five percent (75%) for the past 18 months or during the length of time the facility has operated, whichever is less. Any violations documented during visits to your program may have an impact on the total compliance history score and cause your score to drop below the mandated level. **Prior to today’s visit, the compliance history score for this family child care home was 100%. Any violations cited during today's visit may affect this percentage. AGENCY UPDATES DCDEE UPDATES AND NEWS WATER TESTING Please register for the required pre-enrollment webinar https://rtiorg.zoom.us/webinar/register/WN_vmlhG6iRS26BdkV8SZSaDw#/ to participate in the Clean Classrooms for Carolina Kids Program. The Clean Classrooms for Carolina Kids program helps fulfill North Carolina requirements (NC 10A NCAC 41C.1001-1007, NC 154A NCAC 18A.2816) for public schools and child care facilities related to lead in water, lead-based paint, and asbestos. This is currently free of charge. The goal is to identify and eliminate exposure to lead and asbestos hazards where North Carolina children learn and play. Lead Water test results are required by January 1, 2025. Family Child Care Home providers must be registered for the required lead paint and asbestos testing by May 1, 2024. • Mrs. Cousins sent off her water test and lead and asbestos test and received notice that her test results came back indicating normal range in all areas. REMINDERS: The following requirements expire in the future; remember to complete these prior to their expiration. -CRC - every 5-years -CPR/FA – every two years -Health and Safety Trainings will need to be completed again by June 2028 and every 5 years thereafter *Ms. Cousins completed her required 15 training hours since her last annual compliance visit conducted on September 7, 2023. *Please note, first aid and CPR training is no longer counted towards training hour requirements. TECHNICAL ASSISTANCE AND CONSULTATION • One child enrolled in Mrs. Cousins FCCH requires an EPI Pen and Benadryl. Both medications were present in the home today BUT the required medical forms were not available for review. Mrs. Cousins and I discussed the importance of reviewing each child’s required paperwork BEFORE the child starts care in her FCCH. I suggested Ms. Cousins request parents of new child care children return the required child care paperwork packet to Ms. Cousins at least 3 days prior to their start date for review which will allow Ms. Cousins to request any missing paperwork before the child’s first day of care. • The NC Child Care Health and Safety Resource Center is a valuable resource where you can find skills-based training and technical assistance from your Child Care Health Consultant on specific medications or health conditions, as well as the new poster on Stand-Up Changing Procedures. Sign up for their quarterly newsletter and find updated resources at www.healthychildcare.unc.edu or 800-367-2229, choose option 1, then 2. • The Healthy Social Behaviors (HSB) Project supports teachers to promote healthy social-emotional development and reduce the expulsion rate among young children in licensed child care centers across North Carolina. Specialists are available with early childhood education backgrounds who are passionate about empowering teachers to develop learning environments and teaching practices that promote prosocial skills in young children. • Please continue to visit DCDEE’s website for child care updates at https://ncchildcare.ncdhhs.gov/. As you enroll new children in your FCCH, confirm you are using the most up to date forms which can be found on the DCDEE website under Provider and the Provider forms. Please submit a corrective action letter for the above violations. While the Division allows you time to explain how you have corrected violations when they are documented, it is expected that you correct all violations immediately. Your corrective action letter must include the following: Name of your program, Id number of your program, Date of Letter, Violation(s) number(s), and How and When each violation was corrected. Your letter is due no later than August 28, 2024, and should be emailed to Kimberly Girouard at Kimberly.Girouard@dhhs.nc.gov.
