Your program currently operates with a Four-Star license. Your program was also monitored for compliance with implementing an approved curriculum as required for all four- and five-star licensed facilities where four-year-old children are enrolled. Today’s visit was conducted with both Ms. Smith and Ms. Golden, Lead Teachers. I monitored two classrooms in use for compliance with supervision, and staff interaction/discipline. The NC Pre-K requirements in section .3000 of the childcare rules were monitored for compliance. Staff-child ratios and maximum group sizes required in Child Care Rule 10A NCAC 09 .3009 were verified in compliance. Children participated in group learning activities, completed routine tasks, ate lunch, and prepared for rest time. I observed proper handwashing. Staff facilitated group time, assisted children with meals, and met the children’s developmental needs. The facility was monitored for health and safety requirements both indoors and outdoors. One violation related to outdoor safety was cited. The program requirements, along with equipment and furnishings were also observed. I monitored new staff and sampling of existing staff and children records. Information was documented on the staff and training worksheet and the children’s records form. One violation related to staff records was documented. A checklist was also used to monitor compliance today. The last annual compliance visit was conducted 1/3/2024. The sanitation and fire inspections were current. NC PRE-K REQUIREMENTS: The site monitoring tool was not observed. The center uses the Teaching Strategies Gold instrument to document evidence of children's ongoing progress and is documented electronically. I monitored requirements related to children screenings, parent involvement, developmental assessments, referrals, and parent communication. NC Pre-K requirements complied. Violation Number Comment Rule 807 A safe indoor and outdoor environment was not provided for the children. Tripping hazards from exposed tree roots were on the playground. 10A NCAC 09 .0601(a) 1899 Health and safety training topics were not included as part of on-going training within five years of completing the previous health and safety training topics. Upon my arrival, I was greeted by the Lead Teachers for each room, Emily Golden and LaShonda Smith and they assisted me with the visit today. The children in each classroom were observed participating in center time, circle time, having lunch and going down for nap. There are two NC Pre-K classrooms at Yates Mill Elementary School. Currently, this program operates a 4-star license issued December 9, 2019. There are eighteen four- and five-year-old children currently enrolled in classroom 325 and fourteen were present today. Classroom 323 currently has fifteen four- and five-year-old children enrolled and all fifteen children were present during today’s visit. ATTENDANCE Attendance was monitored and found to be in compliance. Arrival and departure times are documented in a notebook each day. POSTINGS/PROGRAM RECORDS All required postings were observed to be in compliance. All requested paperwork was available for review today. Fire inspections and sanitation inspections were current and on file. Fire drills and emergency drills were found to be in compliance today. There is an incident log notebook, but no incidents logged to date. STAFF RECORDS Staff records were monitored today. VIOLATIONS Two violations were cited today. Violation Number Comment Rule 1041 Prior to employment a Criminal Background Check was not completed. One of the teacher assistants working in the classroom today had not completed the required criminal background check that is necessary to work in a licensed child care program. G.S. 110-90.2(b) 9999 A violation was found for which there is no item number. Sanitation.2801(11): "Designated Emergency Medication" means a medication needed to immediately treat a life-threatening medical event that is administered in accordance with 10A NCAC 09 .0803(10) and G.S. 110-102.1A. A child in the NC Pre-K program at Yates Mill Elementary has an allergy that requires an epi pen. The Epi Pen is currently kept in the school front office making it not immediately available in the event the child experiences an allergic reaction requiring the Epi Pen injection. TECHNICAL ASSISTANCE/REMINDERS *I discussed the rules associated with the violations cited today involving emergency medications for children enrolled in the NC Pre-K program, and criminal background checks for staff working in the NC Pre-K classrooms with the teacher in classroom 323, Ms. LaShonda Smith, the principal Ms. Vonda Jenkins and the Assistant Principal, Ms. Rene Alfred. The administrators of Yates Mill Elementary School are addressing this policy with the Wake County Public School district office for further clarification and direction. The emergency medication was brought down to the classroom while the administrators continue to receive further direction from the school nurse assigned to their school. *Please continue to update your NC Pre-K/DCDEE Licensing notebook and children’s portfolios as they will be monitored throughout the year. CONSULTATION: RATED LICENSE Yates Mill Elementary School NC Pre-K program is in COHORT 2 for the rated license assessment. Their reassessment year will be from July 1, 2025. I shared the importance of all staff creating and maintaining a personal WORKS account in preparation for their reassessment. COMPLIANCE HISTORY The compliance history as of 11/30/2023 was 89% and does not include today’s visit. You are responsible for accessing information for any updates to the rules and laws on the DCDEE website and demonstrating full compliance at all times. Please submit a corrective action letter for the above violations. While the Division allows you time to explain how you have corrected violations when they are documented, it is expected that you correct all violations immediately. Your corrective action letter must include the following: Name of your program, ID number of your program, Date of Letter, Violation(s) number(s), and How and When each violation was corrected. Upon my arrival, I was greeted by the Lead Teachers for each room, Emily Golden and LaShonda Smith and they assisted me with the visit today. The children in each classroom were observed participating in center time, circle time, having lunch and going down for nap. There are two NC Pre-K classrooms at Yates Mill Elementary School. Currently, this program operates a 4-star license issued December 9, 2019. There are eighteen four- and five-year-old children currently enrolled in classroom 325 and fourteen were present today. Classroom 323 currently has fifteen four- and five-year-old children enrolled and all fifteen children were present during today’s visit. ATTENDANCE Attendance was monitored and found to be in compliance. Arrival and departure times are documented in a notebook each day. POSTINGS/PROGRAM RECORDS All required postings were observed to be in compliance. All requested paperwork was available for review today. Fire inspections and sanitation inspections were current and on file. Fire drills and emergency drills were found to be in compliance today. There is an incident log notebook, but no incidents logged to date. STAFF RECORDS Staff records were monitored today. VIOLATIONS Two violations were cited today. Violation Number Comment Rule 1041 Prior to employment a Criminal Background Check was not completed. One of the teacher assistants working in the classroom today had not completed the required criminal background check that is necessary to work in a licensed child care program. G.S. 110-90.2(b) 9999 A violation was found for which there is no item number. Sanitation.2801(11): "Designated Emergency Medication" means a medication needed to immediately treat a life-threatening medical event that is administered in accordance with 10A NCAC 09 .0803(10) and G.S. 110-102.1A. A child in the NC Pre-K program at Yates Mill Elementary has an allergy that requires an epi pen. The Epi Pen is currently kept in the school front office making it not immediately available in the event the child experiences an allergic reaction requiring the Epi Pen injection. TECHNICAL ASSISTANCE/REMINDERS *I discussed the rules associated with the violations cited today involving emergency medications for children enrolled in the NC Pre-K program, and criminal background checks for staff working in the NC Pre-K classrooms with the teacher in classroom 323, Ms. LaShonda Smith, the principal Ms. Vonda Jenkins and the Assistant Principal, Ms. Rene Alfred. The administrators of Yates Mill Elementary School are addressing this policy with the Wake County Public School district office for further clarification and direction. The emergency medication was brought down to the classroom while the administrators continue to receive further direction from the school nurse assigned to their school. *Please continue to update your NC Pre-K/DCDEE Licensing notebook and children’s portfolios as they will be monitored throughout the year. CONSULTATION: RATED LICENSE Yates Mill Elementary School NC Pre-K program is in COHORT 2 for the rated license assessment. Their reassessment year will be from July 1, 2025. I shared the importance of all staff creating and maintaining a personal WORKS account in preparation for their reassessment. COMPLIANCE HISTORY The compliance history as of 11/30/2023 was 89% and does not include today’s visit. You are responsible for accessing information for any updates to the rules and laws on the DCDEE website and demonstrating full compliance at all times. Please submit a corrective action letter for the above violations. While the Division allows you time to explain how you have corrected violations when they are documented, it is expected that you correct all violations immediately. Your corrective action letter must include the following: Name of your program, ID number of your program, Date of Letter, Violation(s) number(s), and How and When each violation was corrected.
