This facility currently operates with a Four (4) Star License issued on 1-27-21. The last annual compliance visit was conducted on 1-31-25. Today’s visit was conducted with you, Grace Ogbonnaya, owner/operator. Upon arrival to the residence, I was greeted by you. You explained that no children were present today. I did a walk-through of the lower level of the home with you, visiting the room where child care occurs, the kitchen, and the bathroom that is occupied by children. Compliance with attendance, program requirements, storage of hazardous substances, CPR & First Aid, permit restrictions, the required postings, and your staff record documentation and children’s records were monitored. You stated that no medications are administered and no medications were observed on the premises. Your van that is used for transportation was also monitored for compliance with transportation requirements. An assessment of applicable child care requirements was monitored and the following violation was cited during today’s visit. Technical assistance on how to maintain compliance was provided today. Violation Number Comment Rule 1301 Operator did not complete the required number of on-going training hours as specified in rule. The operator only completed six (6) out of the ten (10) required annual on-going training hours. Training boosts caregiver’s confidence, improves staff morale, and can open up opportunities for higher-level roles or increased earning potential. It is suggested that you mark your calendar and set an alarm monthly to complete the annual required ongoing training hours. This facility currently operates with a Four (4) Star License issued on 1-27-21. The last annual compliance visit was conducted on 1-31-25. Today’s visit was conducted with you, Grace Ogbonnaya, owner/operator. Upon arrival to the residence, I was greeted by you. I did a walk-through of the lower level of the home with you, visiting the downstairs converted bedroom where child care occurs, the kitchen, and the bathroom that is occupied by children. Compliance with capacity, supervision, discipline, attendance, program requirements, storage of hazardous substances, CPR & First Aid, permit restrictions, the required postings, and your staff record documentation was monitored as a partial assessment of applicable child care requirements. No medications are administered or were observed on the premises. During the visit, the children were observed coloring July 4th sheets and creating free art with markers and crayons. Afterwards they played musical instruments and had free play time in activity centers. The following violation of child care requirements was cited during today’s visit and must be corrected immediately. Technical assistance on how to maintain compliance was provided. Violation Number Comment Rule 1875 The EPR Plan did not include the location of the Ready to Go File and/or required information. The Ready to Go File did not include all children's emergency information as required. .1714(d)(10) The above violation must be corrected immediately and a letter must be sent to me no later than 7-14-25 stating how the items were corrected and a plan to maintain compliance with the requirements. Any violation(s) documented may impact the compliance history score. • Technical Assistance was provided with the following suggestions that may be beneficial: Ready to Go File: The Ready to Go File requirement is crucial because it acts as a readily accessible collection of critical information needed to quickly respond to an emergency situation, allowing you to efficiently evacuate children, contact parents, and address specific needs during a crisis, ensuring the safety and well-being of all individuals at the residence. Your Ready to Go File notebook needs to be updated with all of the required paperwork. A Ready to Go File checklist was provided today for you to ensure required paperwork is added to your notebook. At least twice a month, your Ready to Go File should be monitored to ensure that it is up to date. The file should be updated when new children enroll or disenroll, and/or when new medication is provided for a child or when the medication is no longer needed, and/or when a child has an allergy, the allergy list needs to be added or modified. It is suggested to mark your calendar or set an alarm to remind you at least every other week to update your EPR Plan/Ready to Go File. • Today, your EPR Plan read “DRAFT.” You were instructed to log into the risk management portal and update any information left blank in order to remove the “DRAFT. Today you printed an updated EPR Plan that did not indicate “DRAFT.” • You provided an updated insurance card from Progressive that showed “temporary” in a watermark on the card. The effective date is from 4-16-25 to 10-16-25. Place your updated insurance card in your files once it expires. • It was explained that you do not have to do emergency drills every two (2) months. The rules require the emergency drills to be documented every three (3) months. • You have linked/connected you and your husband in the ABCMS Portal and your last lead-in water testing was completed on 5-24-24. • Infant and early childhood mental health supports the development in young children of key social and emotional skills, like empathy, self-regulation and communication, which are vital for building healthy relationships and going through life. This facility currently operates with a Four (4) Star License issued on 1-27-21. The last annual compliance visit was conducted on 2-26-24. Today’s visit was conducted with you, Grace Ogbonnaya, owner/operator. Upon arrival to the residence, I was greeted by you. A walk-through of the downstairs area of the residence was conducted visiting the one (1) classroom used for childcare. The back yard was monitored where the children engage in gross motor play and was observed to ensure compliance with safe outdoor environment requirements. The required postings, along with appropriate equipment and furnishings were observed. The EPR Plan, emergency drills, outdoor inspections, the activity plan, daily attendance, and two children’s files were also observed and in compliance. Documentation of program records, as well as the operator verification information was observed and has been documented on the attached form. Your van used to transport children was monitored for compliance with transportation requirements. No medications were observed, as at this time no children have medication to be administered. The children engaged in free play with age appropriate toys and then cleaned up for lunch. Handwashing routines were completed and the children were served lunch. Afterwards toileting and handwashing occurred prior to naptime where the children rested on cots and mats with linens. A caring and nurturing environment was observed as you interacted with the children and attended to their needs. During today’s visit, an assessment of applicable child care requirements was conducted. The following violations were cited and must be corrected immediately. Technical assistance on how to maintain compliance was provided. It was suggested to mark on your calendar and set an alarm on your phone one (1) year and ten (10) months from the date that you last completed CPR and First Aid training. This reminder will be to contact a trainer so the CPR and First Aid training can be completed within the two (2) year renewal timeframe. • A suggestion was made to save the most recent child care requirements in Chapter 9 effective November 2024, to your desktop computer for easy access. Please log back in to the Risk Management Portal and edit your plan. Look to the left of the screen and make sure there are no red dots next to any of the items. The red dot means that information is missing, which will cause your EPR Plan to read “DRAFT.” After you go through each page of the plan input the missing information and you should see all green dots. Once you print your plan the “DRAFT” should no longer appear. • Center directors/owners can now access the Automated Background Check Management System (ABCMS), the DCDEE’s criminal background check system. This access will allow providers to: • See the real-time background check status of household or staff members. • Run a printable report of the household/staff roster to assist with compliance visits. • See new background check applicants and add to the household/staff roster. Access to the ABCMS requires the successful completion of a Moodle course consisting of a short video followed by a test. The course is titled ABCMS Child Care Provider Portal Training - under the Early Childhood Professional Development Tab, then under the Criminal Background (CBC) tab. You and your household member will complete a connecting application to link you with Wee Care Christian Learning Center. • The NC Child Care Health and Safety Resource Center is a valuable resource where you can find skills-based training and technical assistance from your Child Care Health Consultant on specific medications or health conditions, as well as the new poster on Stand-Up Changing Procedures.
