I was greeted by the director LaKeisha Milling and she assisted me with the visit. Later in the visit Toniya arrived. There was a total of 22 children present. The children were observed playing with materials, participating in circle time, coloring, and interacting with staff. The infants were enjoying tummy time and playing with toys. LICENSE STATUS Currently this center operates with a four star license issued 9/11/2024 I reviewed the NC Secretary of State's website and observed that the owner is listed as current/active. INSPECTIONS I reviewed all required inspections including fire drills, emergency drills, sanitation, fire and outdoor inspections. The last fire inspection was April 2025. MONITORING During today’s visit, a partial assessment was conducted, monitoring all space occupied by the children, including the outdoor play environment. All spaces were monitored for supervision, staff/child ratios, materials, equipment and required postings. I reviewed 2 new staff files. The following violations were documented. Violation Number Comment Rule 106 Operator has not scheduled and obtained a fire inspection within 12 months of the previous inspection. Operator did not submit the original approved report to DCDEE within one week of the inspection visit on a form provided by the Division. The alst fire inspection was in April 2025. 10A NCAC 09 .0304(a) 301 Minimum staff/child ratios and group sizes for the number and ages of children in care were not met. In space four #4, there were 11 children present ranging in age 2-6. GS 110-91(7);.0713(a-d) 1048 All staff did not successfully complete certification in First Aid appropriate to the age of children in care. Verification of staff completion of First Aid training from an approved training organization was not in the staff file. One new staff member did not complete first aid within 90 days of employment. .1102(c) 1049 All staff did not successfully complete certification in CPR training appropriate to the age of the children in care. Verification of staff completion of the CPR course from an approved training organization was not in the staff file. One new staff member did not complete CPR training within 90 days of employment .1102(d) Please mail or email your letter of compliance by June 19, 2026. The letter must address the violation and describe how the violation was corrected. Any violations documented during visits to your program may have an impact on the total compliance history score and cause your score to drop below the mandated level. **Prior to today’s visit, the compliance history score for the center was 100%. Any violations cited during today's visit may affect this percentage. TECHNICAL ASSISTANCE **In space #4, there were 11 children present with one (1) staff member. The children ranged in age from two (2) years old to school age. You were able to take two children with you. You stated that you were short staffed and you also were on a bus run. You must always remain in ratio. During my visit, an additional staff member arrived. **The last fire inspection was conducted April 2025. Please schedule your inspection ASAP. **New staff are required to have certain trainings within 90 days including CPR and FA. One new staff member did not completed CPR/FA training within 90 days. You stated that you will schedule her training and your regular trainer was not available due to family issues. RATED LICENSE >>You recently completed your rated license and you will be due in 2027. I completed a walk-through on my own because the director, Angela Hinton was in a classroom. At the time of visit, there were a total of nine (9) children present. The children were observed playing outside, playing with materials and playing in centers. LICENSE STATUS Currently this center operates with a three star license issued 10/3/2018. This center is in cohort 1 (one). The preparation year started July 1, 2023 and the assessment year will start July 1, 2024. I reviewed the NC Secretary of State's website and observed that the owner is listed as current/active. INSPECTIONS I reviewed all required inspections including fire drills, emergency drills, sanitation, fire and outdoor inspections. Inspections were found to be in compliance. MONITORING During today’s visit, a full assessment was conducted, monitoring all space occupied by the children, including the outdoor play environment. All spaces were monitored for supervision, staff/child ratios, materials, equipment and required postings. I reviewed staff files and a selection children’s files. A checklist was used to note the requirements I monitored today. I left a copy of the checklist at the end of the visit. ANNUAL IN-SERVICE TRAININGS This provider’s annual in-service calendar dates are January 1st-December 31st. The following violation was observed and documented: Violation Number Comment Rule 1321 Medical exam or health assessment record was not on file before or within 30 days after admission. One child's file was missing a medical exam. GS110-91(1) The above violation MUST be corrected immediately. Please mail or email your letter of compliance by February 1, 2024. We also discussed assessment and I suggested that you see ncrlap.org for resources, videos and other information. RATED LICENSE EDUCATION Meets 4 points in education. The center is looking to hire an additional staff member. We will evaluate education at a later date. PROGRAM STANDARDS Meets 2 points in program standards. This program declined to complete ITERS/ECERS assessment. QUALITY POINT This program chose to reduce group size by one in each group.
