I was greeted by the lead teacher, Pam Alston and she assisted my with the visit. I spoke to the owner, Iasia Pope over the phone. There was a total of three (3) children present. The children were observed finishing up lunch, preparing for nap and sleeping/resting for the duration of the visit. LICENSE STATUS Currently this center operates with a five star license issued 10/17/17. INSPECTIONS I reviewed all required inspections including fire drills, emergency drills, sanitation, fire and outdoor inspections. Inspections were found to be in compliance. MONITORING During today’s visit, a full assessment was conducted, monitoring all space occupied by the children, including the outdoor play environment. All spaces were monitored for supervision, staff/child ratios, materials, equipment and required postings. I reviewed staff files and all children’s files. A checklist was used to note the requirements I monitored today. The following violations were documented: Violation Number Comment Rule 428 A current activity plan was not posted for each group of children for reference. The plan posted was dated for the week of June 29th. GS 110-91(12); .0508(a) 526 Menus for all meals and snacks were not current or posted where easily seen by parents and cook. A current menu was not posted. The menu posted was dated May 29th. 10A NCAC 09 .0901(b) Please mail or email your letter of compliance by July 22, 2026. The letter must address the violation and describe how the violation was corrected. Any violations documented during visits to your program may have an impact on the total compliance history score and cause your score to drop below the mandated level. **Prior to today’s visit, the compliance history score for the center was 99%. Any violations cited during today's visit may affect this percentage. TECHNICAL ASSISTANCE >>During the walk through I observed an activity plan dated for June 29th and the menu was dated May 29th. I spoke to Mrs. Pope and she explained that the printer had run out of ink and then she purchased the wrong size but she will get that resolved today. RATED LICENSE We discussed and reviewed the QRIS- Rated License standards. You have chosen Pathway 1- Program assessment. We went over each standard in detail: enhanced ratios, education standards, family and community engagement standards, CQI plan, Approved curriculum used when four year-olds are enrolled, Program Assessment Self-Study (using applicable ERS) completed for all classrooms. I verified you use Lillio learning curriculum. I asked you to please send me the ERS request from and application so I can send a request to NCRLAP. I also discussed the self-study. Your last annual compliance was on August 11th, 2022. A checklist was used to note the requirements I monitored today. This center located in a residence has five household members including the owner/provider, her husband, two children and a grandchild. The center currently operates with a 5-star license, operating at 30 sq. ft. inside, enhanced ratios/space. My visit was conducted with center staff. I ‘Asia Pope, owner/provider was not present during today’s visit. Tonya Richardson, Director at Ms. Pope’s Knightdale location joined us during the visit to cover in Ms. Pope’s absence. During today’s visit there were 5- enrolled children present one 4-year-old preschool child and 4- school-age.. I monitored for required posted documents, program requirements, children’s files, staff files, equipment, furnishings, discipline, staff/child ratio, supervision, outdoor environment, and nutrition. VERIFICATION OF RECORDS: *Last fire inspection= 7/13/2023- Approved for daytime care only *Sanitation inspection= 3/16/2023 with a superior classification and 0 demerits * Well Water Testing= water testing was completed as per requirements following the last sanitation inspection. Paperwork on file for review. * Compliance history as of today’s visit 100% *The following violations of childcare requirements were cited during today’s visit Violation Number Comment Rule 1043 All staff records, except financial records, were not made available for review. Review of staff and children's files found files were incomplete including but not limited to medical/immunizations in children's files, training, health and emergency questionnaires in staff files. G.S. 110-91( 9) 1811 Shelter-in-place or lockdown drills were not practiced every three months and/or drill record was incomplete. The last drill was conducted on April 3rd, 2023 and within 3 months a drill should have been conducted by July 3rd, 2023. .0604(u);.0302(d)(8) 1914 The person identified as the person or alternate person responsible for carrying out the emergency medical care plan was not on the premises at all times and/or did not accompany children for off premise activities. The three individuals documented on the emergency care plan were not present on site or available in case of an emergency. .0802(b)(1-2) *****TECHNICAL ASSISTANCE/REMINDERS: ******* • My visit was conducted while you were preparing lunch. I reviewed the menu and you informed me that you were serving mixed vegetables and not corn like the menu indicated. I reminded you of the need to update the menu when you know a change needs to be made to what you shared with the parents. • A violation of your EMC (emergency care plan) was cited due to there being specific individuals documented instead of all staff and the people listed were not on site if an emergency occurred. • A violation of paperwork was cited during my visit. The provider file and children’s files were not up to date with all required information. • You informed me during my visit that the staff person that was in charge today the confidential medical portion of her file was on site at the Knightdale location and not where she was working. Although filed separate from the rest of the file it needs to be on site available for review. • A violation of shelter in place-lockdown drill requirement was cited. This drill is required every three months. The last drill was conducted April 3rd, 2023, it should have been conducted by July 3rd, 2023. Violations of childcare requirements were cited during today’s visit. Please provide a letter of correction indicating how you corrected it and how you will keep in compliance going forward by August 18th, 2023. Due to a violation regarding paperwork please make sure to submit your staff and training worksheets along with your letter of correction.
