Upon arrival, I was greeted by the provider, Sandra White. You assisted me with the visit. I completed a general walk through of the children's space, kitchen area, outdoor space and bathroom. At the time of visit, there were 4 children present. The children were observed playing with materials, interacting with the provider, eating lunch and completing routine tasks. LICENSE STATUS Currently this Family Childcare Home (FCCH) operates with a five-star license issued 9/27/17. This FCCH is in cohort 1. The preparation year is June 30, 2023 and the assessment year is June 30, 2024. INSPECTIONS I reviewed required inspections including fire drills, emergency drills, and outdoor inspections. All inspections were found to be in compliance. DESIGNATED CHILDREN’S SPACE The children’s space is located at the back of the home. In the children’s space, there are a variety of manipulatives, pretend play toys, books and art materials for the children to explore throughout the day. The provider uses a changing mat for children who are still diapering and the bathroom is located in the in the adjacent room near the kitchen and children’s space. Cots are used for nap time. MONITORING During this visit, a full assessment of Child Care Requirements was conducted. A checklist was used to note the requirements monitored today. All spaces were monitored for supervision, staff/child ratios, materials, equipment and required postings. I reviewed the provider’s information and two children’s files. The following violation was observed and documented: Violation Number Comment Rule 1406 Authorization for up to 6 months to administer prescription or over-the-counter medication, when needed, for chronic medical conditions and for allergic reactions was not in writing and did not contain information required by rule 10 NCAC 09 .1720(c)(6)(A-I). One child file was missing the authorization to administer medication. 10A NCAC 09 .1720(b)(4) Please mail or email your letter of compliance by June 19, 2024. The letter must address each violation and describe how the violations were corrected. Any violations documented during visits to your program may have an impact on the total compliance history score and cause your score to drop below the mandated level. **Prior to today’s visit, the compliance history score for the center was 100%. Any violations cited during today's visit may affect this percentage. TECHNICAL ASSISTANCE **We discussed a child’s medication permission form and you stated that you sent in home to be updated and you forgot to follow up with the parents. Also the child’s epi-pen expires in July and I reminded you to renew the medical actin plan and you stated that you will update all the forms. RATED LICENSE **We discussed your prep year assessment, and I encouraged you to have it completed and you stated that you will wait to see what happens with the new QRIS system. I stated that it is a possibility of extending the hold harmless status. I left the request forms with you and asked you to send them before June 30th. AGENCY UPDATES WATER TESTING Currently, there is funding for FCCH to get their water, paint and asbestos tested for lead for free.
