The last annual compliance visit was conducted on January 9th, 2026, and only staff and children’s files were reviewed and were not done during your annual visit. Today’s visit was conducted with Ms. Marcia, covering director, and Ms. Charlene, assistant director. The center has a 4- star license, operating at 30sq. ft. inside, & 100 sq ft outdoors enhanced ratios and space. QRIS information was discussed and instructions on how to navigate the division’s website and where to find the videos and forms were provided to help the facility decide on what pathway to choose and achieve. During your annual compliance visit a violation was cited related to the completion of your ABCMS provider and individual portals. Please make sure to complete this requirement and submit a letter of corrective action. During today’s visit violations were cited related to staff in-service training hours and review of the EMC (emergency care plan). There was a staff meeting book but only one month available for review, no documentation was available to show that the plan was reviewed in February during a staff meeting, the agenda was not available. *Two violations of childcare requirements were cited today. Violation Number Comment Rule 862 The EMC plan was not reviewed with all staff annually and whenever the plan was revised. There was no documentation in the staff meeting binder showing the completion of EMC plan review. 10A NCAC 09 .0802(a) 1052 Staff required to receive on-going training had not completed the required number of hours according to their education and experience. During file review it was determined that eight staff did not meet their annual in-service training requirement. .1103(a) A violation of childcare requirements was cited today and requires a letter of corrective action by January 27th, 2026, documenting how the violations were corrected and how you will keep in compliance in the future. Technical assistance/recommendations: If staff meetings are conducted each month and the agenda includes a requirement like review of the EPR or EMC plan, make sure the agenda and sign-in sheets are immediately placed in your staff meeting book so violations can be avoided because documentation is available to prove the task was completed per NC Childcare rule. A violation related to staff in-service training hours was cited. Yearly training is a requirement that staff have a year to complete prior to the annual compliance visit. something needs to be put in place to guide staff to complete their yearly requirement. (ex. A free training taken via Moodle when staff return from lunch and there are additional staff in the building to cover for a 15 min- half hour training session before the staff person returns to the classroom) or during naptime when classrooms for children ages three and older can have one staff person and the staff can rotate by the lead teacher taking a training on-line, returning to the room and then the teacher assistant taking the training before children wake up from nap. The last annual compliance visit was conducted on January 15th, 2025. A checklist was used to note the requirements I monitored today. The center has a 4- star license, operating at 30sq. ft. inside, & 100 sq ft outdoors enhanced ratios and space. My visit was conducted with you, Jessica Horne, Director and center staff. We discussed the new QRIS rated license, and your facility has not chosen a pathway yet. We will discuss it at your next visit. An AC follow-up visit will be conducted due to the inability to review staff and children’s files during today’s visit due to circumstances out of your control. The children were observed participating in free/center play indoors and outdoor play. During the walkthrough to obtain enrollment and conduct my observations. I monitored required posted documents (EPR, EMC, Menu, Curriculum/activity schedule, sanitation, safe arrival/departure), interactions between staff and children, discipline, supervision, group sizes, space capacity, program requirements, equipment and furnishing requirements. Transportation was not monitored all pick up and drop off is done by the parents of the enrolled children. VERIFICATION OF RECORDS: *Last fire inspection= 11/4/2025 and approved for daytime care. The last annual compliance visit was conducted on February 7th, 2024. A checklist was used to note the requirements I monitored today. The center has a 4- star license, operating at 30sq. ft. inside, & 100 sq ft outdoors enhanced ratios and space. My visit was conducted with you, Jessica Horne, Director and Wanda Franklin, Asst. Director, and center staff. The children were observed participating in free/center play indoors and heard a teacher call to review the weather chart to determine if it was acceptable to participate in outdoor play. During the walkthrough to obtain enrollment and conduct my observations I monitored for required posted documents (EPR, EMC, Menu, Curriculum/activity schedule, sanitation, safe arrival/departure), interactions between staff and children, discipline, supervision, group sizes, space capacity, program requirements, equipment, medication and furnishing requirements. Jessica Horne, director, accompanied me during a walk-through of the facility. During the visit, I discussed the allegations with Ms. Horne and additional staff members. Limited monitoring of child care requirements occurred during today’s visit. Violation Number Comment Rule 1756 Enhanced staff/child ratios and group sizes were not met.
