A full assessment of child care requirements was conducted. Your program currently operates with a Five-star license. The facility was monitored for approved curriculum. Today’s visit was conducted with you, Coprecia Robinson, Center Director. You visited each child care area with me. I monitored classrooms in use for compliance with staff/child ratios, supervision, capacity, group size and staff interaction/discipline. Children throughout the facility were playing in activity areas, participating in group time, listening to music, and preparing for lunch. Staff assisted children with using materials, play, toileting, routines, and facilitated group time activities. The facility was monitored for health and safety requirements indoors. The outdoor area was not monitored due to inclement weather. Program requirements, along with equipment and furnishings were monitored. All inspections and drills were current. A sampling of children’s and existing staff’s records, and all new staff records was monitored and information documented on the children’s records form and the staff and training worksheet. Two violations regarding staff’s records and one violation regarding children’s records were documented. A checklist was also used to monitor compliance. The last annual compliance visit was conducted 10/4/24. Prior to today’s visit, the facility’s compliance history was 100 percent. The owner/operator is listed as Active-Not Current with the NC Secretary of State. The facility does not transport children. Violation Number Comment Rule 1032 Child care providers and uncompensated providers who are not substitute providers or volunteers, including the director did not have a medical report on file prior to employment that was signed by a health care professional and/ or the medical report was older than 12 months. Two staff employed in August 2024 had medical reports that were older than 12 months. 10A NCAC 09 .0701(a) 1033 On or before the first day of work, all staff, including the director and individuals who volunteer more than once per week did not provide results indicating that they were free of active TB and/or TB test or screening was older than 12 months. Two staff employed in August 2024 had TB test results that were older than 12 months. .0701(a) 1303 Application was not signed by the parent. Applications for six children were not signed. 10A NCAC 09 .0801(a) Child Care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. Any violation(s) documented may impact the compliance history score. The violation(s) documented must be corrected immediately. On or before 9/27/2024, I must receive a written, dated, and signed compliance letter that describes accurately and in detail how and when the violations were corrected. Please be aware any information submitted by you is legal documentation. If it is determined the information provided in the letter is not true, this may be considered falsification of information. If sufficient information is not received by the due date, a follow-up visit will be conducted. It is an important part of licensure. You explained that this is the first week children have attended since you closed for summer break. Staff returned in August to prepare for children. You also explained you did not realize the TB and the medical reports for staff were not completed within the required timeframe. Today you informed both staff they will need to immediately obtain current TB testing and medical reports. Today we reviewed the required components for a child’s application. You created your own application. I reviewed with you the children records form and how it corresponds to parts of the application such as chronic illness and medication, acknowledgments, and other items. You explained you did not realize a signature was required for the child’s enrollment application. You stated you will add the missing information to the child’s application and obtain the signatures on the current application. CONSULTATION: NC Secretary of State: Today I observed the Owner information is not current with the NCSOS. You will need to contact them to determine what to do to become current. If the Owner information becomes suspended or dissolved, additional action may be taken. Lead Free Classrooms: It appears you have completed the lead-free water test, and started the lead in paint, and asbestos testing requirements with cleanwaterforuskids.org . Hold harmless - Hold harmless has been extended until the new QRIS is implemented (SB 425). You already completed the rated license reassessment on 5/10/2024. The violations were reviewed with you at the end of my visit. The computer-generated visit summary was sent to you.
