This facility currently operates with a Notice of Compliance issued on 3-14-25. The last annual compliance visit was conducted on 9-9-25. Today’s visit was conducted with you, Crystal Pace, center director. Upon arrival to the facility, I was greeted by you. I did a walk-through of the facility with you visiting the five (5) classrooms occupied by children. Compliance with staff/child ratios, supervision, discipline, attendance, program requirements, storage of hazardous substances, permit restrictions, the required postings, and your new staff record documentation was monitored as a partial assessment of applicable child care requirements. Medications observed on the premises were monitored. The children were observed at the table with puzzles, coloring with crayons, practicing writing on dry erase boards, building with Legos, and having music and movement. Appropriate handwashing and toileting routines were completed as required. The following violation of child care requirements was cited and must be corrected immediately. Technical assistance on how to maintain requirements was provided today. Violation Number Comment Rule 846 Over-the-counter medicines were not in their original containers or administered as authorized in writing by parent, physician or authorized health professional. Located in space #4104 was Off Insect Repellent, Boogie Diaper Rash Spray, and Desitin that were not labeled with the child's names and located in space #4110 was Chapstick not labeled with the child's name. 10A NCAC 09 .0803(4) The following were discussed with you today: • Please submit the Pre-Service Requirements for Administrators Form to me asap with your signature and your owner’s signature. • Update your posted Emergency Medical Care Plan with more names. • We discussed that some of your children are using their linens for the cots as a pillow. Please ensure that the children are laying on linens during naptime. • You asked questions about your children’s files paperwork and suggestions were made to eliminate paperwork and/or condense some of the required paperwork that needs parent’s/guardian’s signatures. • It was recommended to post the updated November 2025 version of the Summary of the NC Law & Rules for Child Care Centers that was emailed to you today. • Although you operate with a Notice of Compliance, you were informed about the new star rated license rules that went into effect July 1, 2025. Quality Rating and Improvement System (QRIS), is North Carolina’s new star rating system for early childhood education facilities. QRIS is a systemic approach to assess, improve, and communicate the level of quality in child care. It encourages programs to engage in continuous quality improvement and families may use the star rating to make informed decisions when choosing a child care facility. Today it was explained that you need to review the new QRIS requirements, which include three pathways to choose for the rated license process. As well it was explained that your staff’s education must be updated in the WORKS system asap. You stated that at this time you are not interest in transitioning to a star rated license, It was strongly recommended to complete an outreach assessment through the North Carolina Rated License Assessment Project (NCRLAP) to see any areas of improvement since you have not completed an ITERS-3 and ECERS-3. An outreach assessment from NCRLAP is a free, supportive evaluation of a childcare program's quality that provides a realistic preview of the official assessment process without impacting the program's Star Rating. It helps you understand your strengths and areas for improvement by familiarizing you with the Environment Rating Scale (ERS) assessments and their requirements. Think of it as a friendly, no-pressure practice run that helps you pinpoint your program’s strengths and areas for improvement. This facility currently operates with a Notice of Compliance that was issued on 3-14-25. The last sanitation inspection was completed on 6-30-25 with an “Superior” classification. The last approved fire inspection was conducted on 8-15-25. The NC Secretary of State website was reviewed today and this facility’s corporation’s name, Mount Vernon Baptist Church of Wake County was listed as “Current-Active.” Today’s visit was conducted with you, Dana Keefer, center director. I monitored the eight (8) classrooms in use for compliance with staff/child ratios, supervision, and staff interaction/discipline. The program was also monitored for health and safety requirements both indoors and outdoors using the annual compliance monitoring checklist. Children completed daily activities with adequate supervision and engagement from the caregivers. The children were running about and interacting on the stationary play structure on the playground. The children were also practicing writing the letter “U” on a white board, and a caregiver was reading a bible story to the children. The children washed their hands, ate prepared lunch from home and rested on cots with linens. The program requirements, along with equipment and furnishings were observed. Compliance with staff records was documented by you. New staff records, along with children’s records were reviewed. There were no medications observed, as at this time there are no children with medications that need to be administered. This facility does not provide transportation. The following violation of child care requirements was cited during today’s visit and must be corrected immediately. Technical assistance on how to maintain compliance was provided. Violation Number Comment Rule 807 A safe indoor and outdoor environment was not provided for the children. On the outdoor playground, roots from the large tree are causing the turf to buckle and ripple, resulting in a tripping hazard. 10A NCAC 09 .0601(a) The above violation must be corrected immediately and a corrective action letter must be sent to me no later than 9-23-25 stating how the violation has been corrected and a plan to maintain compliance with the requirements. Any violation(s) documented may impact the compliance history score. • Technical Assistance was provided with the following suggestion that may be beneficial: Playground Safety: Providing a smooth and level playground surface is important because a smooth surface reduces the risk of falls, which is a common cause of injuries on playgrounds. A safe surface allows children to play more confidently, knowing they are protected from falls, minimizing the likelihood of serious injuries, including fractures, concussions, and traumatic brain injuries. Providing a smooth surface provides a stable environment for children to develop their motor skills and balance. A well-maintained surface can also add character to the outside area, making it a more enjoyable place for children to play. Please have a caregiver stand near this area to prevent children from running and falling near the roots until the playground can be repaired. It is essential to ensure that the playground surface meets safety standards and is installed correctly to provide a safe and enjoyable environment for children. The following were discussed with you during today’s visit: • • Add a tab in your Ready to Go File for an allergy list. • Your external refrigerator thermometer is working properly, however it was suggested to purchase two more thermometers and place one inside the refrigerator in case the external thermometer is inoperable and there will be a spare thermometer as a backup. • The Healthy Social Behaviors (HSB) Project supports teachers to promote healthy social-emotional development and reduce the expulsion rate among young children in licensed child care centers across North Carolina. Specialists are available with early childhood education backgrounds who are passionate about empowering teachers to develop learning environments and teaching practices that promote prosocial skills in young children. A flyer with the challenging behaviors hotline information was provided today. • The NC Institute for Child Development Professionals (NCICDP) began offering on-demand online trainings for Early Childhood professionals. This facility currently operates with a Notice of Compliance that was issued on 10-25-18. The last sanitation inspection was completed on 8-18-24 with an “Superior” classification. The last approved fire inspection was conducted on 8-16-24. Today’s visit was conducted with you, Dana Keefer, center director. I monitored the five (5) classrooms in use for compliance with staff/child ratios, supervision, and staff interaction/discipline. The program was also monitored for health and safety requirements both indoors and outdoors using the annual compliance monitoring checklist. Children completed daily activities with adequate supervision and engagement from the caregivers. The children were observed in circle time where the caregivers were reading to the children, outside on the playground running about and interacting on the stationary play structure and completing handwashing routines when the children came in from outside. The program requirements, along with equipment and furnishings were observed. Compliance with staff records was documented by you. New staff records, along with children’s records were reviewed today. No medication and no permission to administer medications were observed on the premises, as you stated there were no children with medications to administer at this time. This facility also does not provide transportation. The following violations of child care requirements were cited during today’s visit and must be corrected immediately. Verification of staff completion of First Aid training from an approved training organization was not in the staff file. Verification of staff completion of the CPR course from an approved training organization was not in the staff file. Any violation(s) documented may impact the compliance history score. • Technical Assistance was provided with the following suggestions that may be beneficial: •CPR & First Aid Training – CPR and First Aid training must be renew every two years before the expiration date on the certification card. Current CPR and First Aid training is important for childcare providers because it can help keep children safe and save lives. CPR and First Aid training can help childcare providers respond quickly and effectively to a wide range of emergencies, including choking, seizures, severe bleeding, and allergic reactions, which can make the difference between life and death. Pediatric First Aid and CPR training teaches techniques that are designed for children's unique anatomy and developmental stages. For example, children have smaller airways, which makes choking more common and potentially more dangerous. It was suggested to place expiration dates on your calendar and have an alarm set as a reminder prior to due dates. You are very organized and indicated that when the other childcare providers completed the training, one child care provider was on a mission trip that could not be changed. • We discussed that play is an important part of a child’s development. Playing outside in the fresh air can be fun and adventurous. With careful planning and monitoring, play environments can be safe for children. There have been numerous reports of snakes on the playground in outdoor environments. Remember child care rule 10A NCAC 09 .0604 reads that once a day, prior to initial use, the indoor and outdoor premises shall be checked for debris, vandalism, and broken equipment, the temperature of outdoor equipment, in addition look for other hazards and/or snakes. Handouts with information regarding burn safety and protecting children from thermal burns were provided today. • Please connect/link you and your staff in the criminal record check portal asap. Center directors/owners can now access the Automated Background Check Management System (ABCMS), the DCDEE’s criminal background check system. This access will allow providers to: • See the real-time background check status of staff members. • Run a printable report of the staff roster to assist with compliance visits. • See new background check applicants and add to staff roster. ABCMS requires the successful completion of a Moodle course consisting of a short video followed by a test. The course is titled ABCMS Child Care Provider Portal Training - under the Early Childhood Professional Development Tab, then under the Criminal Background (CBC) tab. Ensure staff know their NCID Username and Password. This facility currently operates with a Notice of Compliance that was issued on 10-25-18. The last sanitation inspection was completed on 8-18-24 with an “Superior” classification. The last approved fire inspection was conducted on 8-16-24. Today’s visit was conducted with you, Dana Keefer, center director. I monitored the five (5) classrooms in use for compliance with staff/child ratios, supervision, and staff interaction/discipline. The program was also monitored for health and safety requirements both indoors and outdoors using the annual compliance monitoring checklist. Children completed daily activities with adequate supervision and engagement from the caregivers. The children were observed in circle time where the caregivers were reading to the children, outside on the playground running about and interacting on the stationary play structure and completing handwashing routines when the children came in from outside. The program requirements, along with equipment and furnishings were observed. Compliance with staff records was documented by you. New staff records, along with children’s records were reviewed today. No medication and no permission to administer medications were observed on the premises, as you stated there were no children with medications to administer at this time. This facility also does not provide transportation. The following violations of child care requirements were cited during today’s visit and must be corrected immediately. Technical assistance on how to maintain compliance was provided. Violation Number Comment Rule 1048 All staff did not successfully complete certification in First Aid appropriate to the age of children in care. Verification of staff completion of First Aid training from an approved training organization was not in the staff file. First Aid for one staff member expired on 6-24-24. .1102(c) 1049 All staff did not successfully complete certification in CPR training appropriate to the age of the children in care. Verification of staff completion of the CPR course from an approved training organization was not in the staff file. CPR certification for one staff member expired on 6-24-24. .1102(d) The above violations must be corrected immediately and a letter must be sent to me no later than 9-26-24 stating how the items were corrected and a plan to maintain compliance with the requirements. The following were discussed with you during today’s visit: • Child care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4)(d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. Any violation(s) documented may impact the compliance history score. • Technical Assistance was provided with the following suggestions that may be beneficial: •CPR & First Aid Training – CPR and First Aid training must be renew every two years before the expiration date on the certification card. Current CPR and First Aid training is important for childcare providers because it can help keep children safe and save lives. CPR and First Aid training can help childcare providers respond quickly and effectively to a wide range of emergencies, including choking, seizures, severe bleeding, and allergic reactions, which can make the difference between life and death. Pediatric First Aid and CPR training teaches techniques that are designed for children's unique anatomy and developmental stages. For example, children have smaller airways, which makes choking more common and potentially more dangerous. It was suggested to place expiration dates on your calendar and have an alarm set as a reminder prior to due dates. You are very organized and indicated that when the other childcare providers completed the training, one child care provider was on a mission trip that could not be changed. • We discussed that play is an important part of a child’s development. Playing outside in the fresh air can be fun and adventurous. With careful planning and monitoring, play environments can be safe for children. There have been numerous reports of snakes on the playground in outdoor environments. Remember child care rule 10A NCAC 09 .0604 reads that once a day, prior to initial use, the indoor and outdoor premises shall be checked for debris, vandalism, and broken equipment, the temperature of outdoor equipment, in addition look for other hazards and/or snakes. Handouts with information regarding burn safety and protecting children from thermal burns were provided today. • Please connect/link you and your staff in the criminal record check portal asap. Center directors/owners can now access the Automated Background Check Management System (ABCMS), the DCDEE’s criminal background check system. This access will allow providers to: • See the real-time background check status of staff members. • Run a printable report of the staff roster to assist with compliance visits. • See new background check applicants and add to staff roster. ABCMS requires the successful completion of a Moodle course consisting of a short video followed by a test. The course is titled ABCMS Child Care Provider Portal Training - under the Early Childhood Professional Development Tab, then under the Criminal Background (CBC) tab. Ensure staff know their NCID Username and Password. This facility currently operates with a Notice of Compliance that was issued on 10-25-18. The last annual compliance visit was conducted on 9-29-22. The NC Secretary of State website was reviewed on 9-25-23 and the corporation’s name, Mount Vernon Baptist Church of Wake County was listed as “Current-Active.” The last sanitation inspection was completed on 9-18-23 with a “Superior” classification. The last fire inspection was approved on 9-25-23 indicating your facility was approved for daytime care only. Today’s visit was conducted with you, Dana Keefer, the new center director. You visited each indoor and outdoor space with me. I monitored the five (5) classrooms in use for compliance with staff/child ratios, supervision, and staff interaction/discipline. Today you explained that the four (4) classrooms at the other end of the hallway are now used by the school and no longer used for the care of the children in child care. Children were observed running about outdoors on the playground, in circle time as the early educator was reading books to them, completing handwashing and toileting routines, and eating lunch at the tables. Proper hand washing techniques were observed. The facility was monitored for health and safety requirements both indoors and outdoors. The program requirements, along with equipment and furnishings were also observed. Compliance with staff records was documented by you and your new staff records, along with children’s records, were reviewed today. This facility only serves a morning snack and afternoon snack, as the parents provide lunch for their children. During today’s visit all lunches brought from home did not meet requirements. Supplements were provided to children with food brought from home that did not meet meal pattern requirements. This facility is not approved for transportation and no medications to be administered were observed on the premises. During today’s visit, an assessment of applicable child care requirements was conducted. The following violations of child care requirements were cited and must be corrected immediately. Violation Number Comment Rule 1032 Child care providers and uncompensated providers who are not substitute providers or volunteers, including the director did not have a medical report on file prior to employment that was signed by a health care professional and/ or the medical report was older than 12 months. One staff member did not have a medical report on file prior to employment. 10A NCAC 09 .0701(a) 1033 On or before the first day of work, all staff, including the director and individuals who volunteer more than once per week did not provide results indicating that they were free of active TB and/or TB test or screening was older than 12 months. One staff member did not have TB test results on file. .0701(a) The above violations must be corrected immediately and a letter must be sent to me no later than 10-9-23 stating how the items were corrected and a plan to maintain compliance with the requirements.
