The center was short-staffed today, so we discussed the visit and I will email the report to you within one day. Upon arrival, I was greeted by the center administrator, Janna Elliot, and later, when Shannon Starbucks arrived, we conducted a general walk through of the indoor and outdoor learning environments. I observed infant rest time and meal time, diapering and free play or group activities in the classrooms. RATED LICENSE STATUS The program operates with a 5-star rated license issued on 3/26/20. They received 6 points in program standards, 7 points in staff education and one quality point through the programmatic option of having a staff benefit package and an infrastructure of parent involvement. The program’s “held harmless status” has been extended until the new QRIS is implemented. This means that rated license visits will not be processed until the new rated license process is completed unless the program requests to proceed with a new rated license. You should ensure that all staff have registered and have their education assessed in WORKS. It is recommended that you complete this process as part of your on boarding with new hires and as existing staff members complete additional coursework. INSPECTIONS The last sanitation inspection was completed on 8/16/23. I reviewed documentation that the director had contacted the sanitation inspector to request a visit and the inspector stated that they will conduct a visit by the end of this month. The last fire inspection was completed 9/31/24. If you have not already done so, you should contact your fire inspector to schedule this visit. The last documented fire drill was conducted 8/29/24. The last documented lock down or shelter-in-place drill was conducted 6/7/24. The last documented playground inspection was conducted 9/9/24. MONITORING During this visit a full assessment of Child Care Requirements was conducted, except for a review of staff files. I will schedule a return visit for the staff file review. The facility was assessed using items noted on the Annual Compliance for Centers checklist. The center does not provide transportation, therefore these requirements were not assessed. The status of the owner, Surya Learning, LLC., is listed as current and active on the NC Secretary of State’s website. The following violation was observed and recorded during today’s visit: Violation Number Comment Rule 1756 Enhanced staff/child ratios and group sizes were not met. There were 14 children, ranging in age from one to two years, combined in a classroom with 3 teachers. 10A NCAC 09 .2818 Due to the nature of the violation documented, a follow-up visit is required, so a compliance letter is not required. COMPLIANCE HISTORY Prior to today’s visit, the compliance history score for the center was 94%. TECHNICAL ASSISTANCE Staff child ratios and maximum group sizes are based on the youngest age of the children who are present. This ensures that children and staff do not become overwhelmed, and that center staff are able to provide good care to the children. Because staffing was low today, rooms 6 and 7 were combined. There were 14 children, ranging in age from 1 to 2 years old, present with 3 teachers in the classroom. The staff/child ratios were in compliance, however there is a maximum group size of 12 children required for one year old children. When using a space, you must consider the capacity of the classroom, the staff and ratios and the maximum group size for the age of the youngest child present. The director stated that she had never thought about maximum group sizes, but now that she is aware of this requirement, she will take this in consideration when rooms are combined. CONSULTATION The center’s water was last tested for lead on 8/30/24. In addition, child care facilities are now required to test for lead and asbestos hazards in their buildings. The Clean Classrooms for Carolina Kids program is designed to identify and eliminate exposure to lead and asbestos hazards in building infrastructure, while providing child care facilities with the support needed to meet all rule requirements. The program is free and the first step is to watch one of the pre-enrollment webinars, which are available at www.cleanwaterforUSkids.org/carolina or paste https://rtiorg.zoom.us/webinar/register/WN_vmlhG6iRS26BdkV8SZSaDw#/registration into your web browser. Additional information can be found in the RaiseNC newsletter sent on December 7, 2023. Training in all the required health and safety topics must be completed at least every 5 years. It may be time for your staff to complete this training again. Health and Safety training is available, at no charge, on the DCDEE website, in Moodle. Medication Administration is also a required health and safety training topic. This training is available through Moodle as a separate training. AGENCY UPDATES Please check the DCDEE website frequently for updates and changes to child care requirements. The director, Shannon, assisted us with today’s visit. Antoinette Williams, Child Care Consultant, assisted me with today’s visit. We conducted a walk-through of the indoor and outdoor learning environments I observed diapering and handwashing, infant rest times and groups of children engaged in free play in their classrooms and outdoors. RATED LICENSE STATUS The program operates with a 5-star rated license issued on 3/26/20. They received 6 points in program standards, 7 points in staff education and one quality point through the programmatic option of having a staff benefit package and an infrastructure of parent involvement. Full rated license assessments will occur based on the program’s Cohort assignment; your program is in Cohort 2, therefore your assessment period would begin on or around July 1, 2025. You should begin preparing and planning for your assessment on or around July 1, 2024, if you wish to complete the Environment Rating Scales. You should also ensure that all staff have registered and have their education assessed in WORKS. It is recommended that you complete this process as part of your on boarding process with new hires or as existing staff members complete additional coursework. The following violations were observed and recorded during today’s visit. Violation Number Comment Rule 615 Beds, cots and mats were not in good repair, properly handled, stored, or clean and sanitized between users. Many mats, throughout the center, had small tears in the corners. 15A NCAC 18A .2821(a) 619 Diaper changing surfaces were not cleaned with a detergent solution and disinfected after each use. The diapering changing pad in Tiny Turtles had small tears along the edges. 15A NCAC 18A .2819(c) 807 A safe indoor and outdoor environment was not provided for the children. A staff member's purse was stored on the top of a cubby approximately 4 feet from the ground. 10A NCAC 09 .0601(a) 1033 On or before the first day of work, all staff, including the director and individuals who volunteer more than once per week did not provide results indicating that they were free of active TB and/or TB test or screening was older than 12 months. One staff member's tb test was dated over 1 year of their hire date. .0701(a) 1045 New staff, who had contact with children, did not receive at least 16 hrs. orientation within first 6 weeks. 7 staff members did not have documentation that required orientation topics were completed. .1101(a) 1899 Health and safety training topics were not included as part of on-going training within five years of completing the previous health and safety training topics. 2 staff members had not completed training in these topics after 5 years. .1103(b) The above violations must be corrected immediately. By 10/11/23 please email me a signed letter of compliance. Before the letter is sent, all violations should be corrected; please contact me if additional time is needed to correct a specific violation. Include the following in your letter: facility ID number, list the item number describing how and when the violations were corrected and tell me how you plan to maintain compliance in the future with each item. If you prefer to mail your letter, you may send it to 2201 Mail Service Center Raleigh, NC 27699-2201 If the compliance action letter is not received by the due date another visit to the center may be made and additional violations may be added. COMPLIANCE HISTORY Prior to today’s visit, the program’s compliance history was 94%. TECHNICAL ASSISTANCE In one of the classrooms, a staff member’s purse was stored on top of a cubby, that was approximately 4 feet above the ground. All staff personal belongings, that may contain potentially hazardous items, must be inaccessible to the children, meaning that it should be stored at least 5 feet above the ground or in locked storage. This violation was corrected during today’s visit. The diaper changing mat in the Tiny Turtles has small tears along the edge, with foam exposed. Diaper changing mats must be in good repair and easily cleanable. This mat should be repaired or replaced. Some of the rest mats in most of the rooms have small tears in the corners and along the edge of the folds. Rest mats should be in good repair and easily cleanable. The damaged mats should be repaired or replaced. Two staff members had completed training in the required health and safety topics in 2017, but they had not been completed every 5 years. All staff should complete the required training within their first year of hire and every 5 years, thereafter. Seven staff did not have orientation documented on the state form. Orientation should be documented on the state form to ensure that all orientation topics are completed. One staff member’s tb test was over 1 year old when the employee began employment. TB tests should be no more than 1 year old upon hire. CONSULTATION Several staff member’s tb test and medical exams were completed after their hire date. During the pandemic, there was a flexibility in place that permitted tb test and medical exams to be completed within 30 days of hire. This flexibility has now expired, so all new staff, moving forward should have these documents completed upon hire. CPR and First Aid certificates, reviewed today, did not include an expiration date and no cards were provided. If there is no expiration date on the certificate, then a card is required. When reviewing staff files, we need confirmation of the expiration date for CPR and First Aid. There are new sanitation requirements effective 7/1/23. I recommend you contact your Environmental Health Specialist or review their website for more information. Training in all the required health and safety topics must be completed at least every 5 years. It may be time for your staff to complete this training again. Health and Safety training is available, at no charge, on the DCDEE website, in Moodle. Staff should use their individual NCID user name and password to access this training. Recognizing and Responding to Suspicions of Child Maltreatment is a part of the health and safety training and should be done at least every 5 years. There is a link to this training on Moodle.
