Upon arrival I was greeted by the director, Nina Jacobs and she assisted me with my visit. I completed a walk-through of the facility. At the time of the visit, there were a total of children 27 present. The children were observed playing in centers, completing an art project, and building towers in the block center. This center has a Wake Three classroom. LICENSE STATUS Currently this center operates with five star license issued September 9, 2025. INSPECTIONS I reviewed all required inspections including fire drills, emergency drills, fire inspection, sanitation, and outdoor inspections. All inspections were found to be in compliance. MONITORING During today’s visit, a full assessment was conducted, monitoring all space occupied by the children, including the outdoor play environment. A selection of staff files and a selection of children’s files were reviewed. All spaces were monitored for supervision, staff/child ratios, materials, equipment and required postings. A checklist was used to note the requirements I monitored today. I left a copy of the checklist at the end of the visit. The playground is currently under construction and replacing the fence. The children are not using the playground but use the blacktop on the side of the building. They use cones to mark off the space the children can play. Ms. Jacobs stated that the fence should be completed before the end of the week. ANNUAL TRAINING The annual training year for this facility is January 1st-Decemeber 31st. One violation was documented: Violation Number Comment Rule 721 All equipment and furnishings were not in good repair. in Van #1 the van seat had a rip/tear and there was a hole in the back of passenger seat. G.S. 110-91(6); .0601(b) COMPLIANCE HISTORY According to NC General Statute all Child Care programs must maintain a compliance history of at least seventy-five percent (75%) for the past 18 months or during the length of time the facility has operated, whichever is less. Any violations documented during visits to your program may have an impact on the total compliance history score and cause your score to drop below the mandated level. **Prior to today’s visit, the compliance history score for the center was 96%. Any violations cited during today's visit may affect this percentage. TECHNICAL ASSISTANCE **During the inspection of the vans, van #1 has a rip/tear in the front seat and a hole on the back of the passenger seat. I recommended get seat covers to cover the hole so the children cannot continue to pull the foam out of the hole. >>Please make sure you add the pictures of the school aged children to the bus roster. WATER LEAD and ASBESTOS TESTING I verified through documentation that you completed all required testing. RATED LICENSE Your facility recently had scale assessments completed and your next rated license review will be in 2028. ABCMS PORTAL Provider Access to ABCMS is Live! I reviewed the portal and all staff are linked and up-to-date. **DCDEE in collaboration with other agencies have started a challenging behavior helpline. Upon arrival I was greeted by the director, Amani Fields and she assisted me with my visit. I completed a walk-through of the facility. At the time of the visit, there were a total of 15 children present. The children were observed playing in centers, participating in circle time, cleaning up, preparing for lunch, washing hands, and eating lunch. This center has a Wake Three classroom. LICENSE STATUS Currently this center operates with five star license issued January 9, 2025. INSPECTIONS I reviewed all required inspections including fire drills, emergency drills, fire inspection, sanitation, and outdoor inspections. All inspections were found to be in compliance. MONITORING During today’s visit, a partial assessment was conducted, monitoring all space occupied by the children, including the outdoor play environment. All spaces were monitored for supervision, staff/child ratios, materials, equipment and required postings. I reviewed staff qualification letters and required trainings. I reviewed two new staff files. ANNUAL TRAINING The annual training year for this facility is January 1st-Decemeber 31st. The following violation was documented: Violation Number Comment Rule 1044 Prior to the expiration date of the qualification letter, the child care provider did not complete and submit required forms to complete a criminal background check (a qualification letter is valid for a maximum of five years for the date of issuance). Once providers are logged in, you can access codes to allow your staff and applicants to create a connecting application to link themselves to your facility. ***The center has a portal set-up but some staff are missing, so I left instructions on how to link/hire employees. **DCDEE in collaboration with other agencies have started a challenging behavior helpline. Upon arrival, I was greeted by a staff member and the director, Ciera Baker assisted me with my visit. I completed a walk-through of the facility. At the time of the visit, there were a total of 12 children present. The children were observed engaging in circle time and coloring. LICENSE STATUS Currently this center operates with five star license issued June 17, 2019. This program is in cohort 1. Prep year ends June 30, 2024 and assessment year begins July 1, 2024. INSPECTIONS I reviewed all required inspections including fire drills, emergency drills, fire inspection, sanitation, and outdoor inspections. MONITORING During today’s visit, a full assessment was conducted, monitoring all space occupied by the children, including the outdoor play environment. I suggested doing the prep year assessment, so you can get a baseline of your classrooms. You stated that you would like to complete your prep year assessment.
