This facility currently operates with a Five (5) Star License that was issued on 8-23-21. The last sanitation inspection was completed on 2-20-26 with an “Provisional” classification. The last approved fire inspection was conducted on 10-21-24. The NC Secretary of State website was reviewed today and this facility’s corporation’s name, Temple Beth Or Preschool was listed as “Current-Active.” Today’s visit was conducted with you, Sherri Lambeth, center director. I monitored the seven (7) classrooms in use for compliance with staff/child ratios, supervision, and staff interaction/discipline. The program was also monitored for health and safety requirements both indoors and outdoors using the annual compliance monitoring checklist. Children completed daily activities with adequate supervision and engagement from the caregivers. The children were observed outside on the playground, interacting in activity centers, in circle time where a caregiver was reading a book to the children, and the children were observed completing toileting and handwashing routines. This week the facility is celebrating Purim, a Jewish holiday and the children made and decorated paper crowns. The program requirements, along with equipment and furnishings were observed. Compliance with staff records will be documented by you. New staff records, along with children’s records were reviewed today. Medication observed on the premises was monitored. The following violations of child care requirements were cited during today’s visit and must be corrected immediately. Technical assistance on how to maintain compliance was provided. Violation Number Comment Rule 106 Operator has not scheduled and obtained a fire inspection within 12 months of the previous inspection. Operator did not submit the original approved report to DCDEE within one week of the inspection visit on a form provided by the Division. The assistant director, Shannon Ford and the director Megan Hans, greeted me upon my arrival. I completed a walk-through with Mrs. Hans. At the time of visit, there were a total of 64 children present. The children were observed participating in circle, playing outside, washing hands and playing with materials in centers. LICENSE STATUS Currently this center operates with a five star license issued 3/14/2019. This center is in cohort 2 (two). The preparation year started July 1, 2024 and the assessment year will start July 1, 2025. I reviewed the NC Secretary of State's website and observed that the owner is listed as current/active. INSPECTIONS I reviewed all required inspections including fire drills, emergency drills, sanitation, fire and outdoor inspections. Inspections were found to be in compliance. MONITORING During today’s visit, a full assessment was conducted, monitoring all space occupied by the children, including the outdoor play environment. I reviewed a selection of staff and children files. All spaces were monitored for supervision, staff/child ratios, materials, equipment and required postings. A checklist was used to note the requirements I monitored today. I left a copy of the checklist at the end of the visit. ANNUAL IN-SERVICE TRAININGS This provider’s annual in-service calendar dates are September 1st-August 31st. The following violations were observed and documented: Violation Number Comment Rule 1032 Child care providers and uncompensated providers who are not substitute providers or volunteers, including the director did not have a medical report on file prior to employment that was signed by a health care professional and/ or the medical report was older than 12 months. Two (2) new staff members (A.R. & C.T) did not have on file prior to hire. 10A NCAC 09 .0701(a) 1033 On or before the first day of work, all staff, including the director and individuals who volunteer more than once per week did not provide results indicating that they were free of active TB and/or TB test or screening was older than 12 months. Two (2) new staff members (A.R. & C.T) did not have on file prior to the first day of work. .0701(a) 1048 All staff did not successfully complete certification in First Aid appropriate to the age of children in care. Verification of staff completion of First Aid training from an approved training organization was not in the staff file. Two (2) new staff members (A.R. & C.T) did not complete first aid within 90 days of hire. .1102(c) 1049 All staff did not successfully complete certification in CPR training appropriate to the age of the children in care. Verification of staff completion of the CPR course from an approved training organization was not in the staff file. Two (2) new staff members (A.R. & C.T) did not complete CPR within 90 days of hire. .1102(d) 1897 The child care administrator and all staff did not complete the Recognizing and Responding to Suspicions of Child Maltreatment training within 90 days of employment. One new staff member did not complete within 90 days of hire. .1102(g) Please mail or email your letter of compliance by March 14, 2024. The letter must address each violations and describe how the violations were corrected. Any violations documented during visits to your program may have an impact on the total compliance history score and cause your score to drop below the mandated level. **Prior to today’s visit, the compliance history score for the center was 98%. Any violations cited during today's visit may affect this percentage. TECHNICAL ASSISTANCE **CPR/FA must be completed within 90 days of hire. Two (2) new staff members (A.R. & C.T) did not complete within 90 days of hire. **TB screenings and medical assessments must be on file prior to employment and the first day of work. Two (2) new staff members (A.R. & C.T) did not complete. **Recognizing and responding to suspicions of child maltreatment training must be completed within 90 days of employment. One staff member did not complete within 90 days of hire. We discussed the above and you were aware that these were missing and you are working with the staff to get these completed. REMINDERS Please check your lead water testing date. Per sanitation this must be completed every three (3) years.
