A report was received by the agency on April 4, 2025. Upon arrival I rang the bell and was buzzed in. I was greeted by the assistant director, Ebonie and the director, Arielle Lundsford. Ms. Lundsford assisted me throughout the visit. ALLEGATION There is a concern about an unsafe environment. There is an allegation of incident reports not being completed at the time of the incident. ALLEGATION #1 There is a concern about an unsafe environment. OBSERVATIONS AND DISCUSSIONS -I discussed the allegation with Ms. Lundsford and she stated that she was aware of an incident that occurred with a one-year-old child. On February 10, 2025, a one-year-old child was put into a highchair. The staff member did not strap the child in and the child wiggled, the tray fell and the child fell out of the high chair hitting their head. -During my interview with the assistant director, Ebonie, she reported the incident to the parents as a ‘hard fall’ but it was not reported that the child fell out of the highchair. The staff in the classroom did not report accurate information to the assistant director or complete an incident report. -Another staff member reported the accurate information about the incident and the director immediately reviewed the cameras. Ms. Lundsford reported that she saw the child get placed in the highchair, the teacher put the tray on and then the child started wiggling. As the child was wiggling the tray came off causing the child to fall out of the highchair. Once Ms. Lunsford viewed the cameras; she and another staff member completed the incident report. -I was not able to view the video today because the farthest the cameras go back was March 18th. -I completed a walk-through of the center. As I was entering space 113, the children and teachers started down the hall coming towards their classroom. I observed three children were in a wagon, but they were not strapped in. The wagon is made with only two (2) straps. RESOLUTION Based on my observations, discussions with director and staff members this allegation has been determined to be SUBSTANTIATED. ALLEGATION #2 There is an allegation that incident reports are not being completed at the time of the incident. OBSERVATIONS AND DISCUSSIONS -The incident that occurred on 2/10/25 was not properly reported to the assistant director or to the parents. An incident report was not completed at the time of the incident. -I reviewed the incident reports for February and observed the report from 2/10/25 however the report was completed the following day and was not signed by the parents. -I also reviewed the incident log and it was not complete or current. The staff thought they only had to record incidents where children need medical care after an incident. RESOLUTION Based on my discussions with the center director and review of documents this allegation has been determined to be SUBSTANTIATED. The following violations were documented. Violation Number Comment Rule 807 A safe indoor and outdoor environment was not provided for the children. On 2/10/25 a child was not strapped into a highchair and the child fell. During today's visit three children observed in a wagon were not strapped in. 10A NCAC 09 .0601(a) 852 Incident reports were not completed each time a child was injured, it did not include all the information required in rule, it was not signed by the parent and/or it was not maintained in the child's file. On 2/10/25 an incident occurred and the staff failed to complete an incident report until the next day. .0802 (e) 853 Incident logs were not completed and maintained as required. All incident reports were not being logged. The only report logged was a report that was sent to DCDEE. .0802(g)(1-6) Please mail or email your letter of compliance by April 22, 2025. Highchairs should have a wide base and a securely locking tray, along with a crotch bar/guard to prevent a child from slipping down and becoming entrapped between the tray and the seat. Highchairs should also be equipped with a safety strap to prevent a child from falling or climbing out of the chair. The safety strap should be fastened with every use. Additionally, children must be strapped in while in a wagon, buggy, and/or bouncy seat to prevent falls and/or entrapment. -Incident reports must be completed each time a child is injured as a result of an incident occurring while the child is in care. The incident must be report before the child leaves for the day or, if severe, at the time of the injury. The report must be signed by the person completing it and by the parent, a copy given to the parent or the parent declining a copy and the report maintained in the child's file. A copy of the form may be found on the Division's website. -I recommended calling parents any time a child hits their head. -The director met with the board of the church and the staff members involved in the incident and reviewed safety procedures and reminded staff the children must be strapped in while in a highchair or wagon. The parents also met with the administration team, and the church board. Ms. Lundsford assured me that she is reminding the staff to strap the children in at all times. She also stated that she is not going to allow the staff to use the wagon due to them not strapping the children in. During my walk-through I observed Ms. Lundsford ask the toddler teacher if the children were strapped in the wagon. Upon arrival, I was greeted by the Director, Arielle Lundsford and she assisted me with the visit. I completed a walk-through of the facility. At the time of visit there were a total of 84 children present. The children were observed participating in circle time, preparing for lunch, eating lunch, playing outside, and resting/napping on their assigned cot. The infants were observed enjoying floor lay and resting in their cribs. LICENSE STATUS Currently this center operates with a Notice of Compliance license issued 8/18/23. I reviewed the NC Secretary of State's website and observed that the owner is listed as current/active. INSPECTIONS I reviewed all required inspections including fire drills, emergency drills, sanitation, fire and outdoor inspections. Inspections were found to be in compliance. MONITORING During today’s visit, a full assessment was conducted, monitoring all space occupied by the children. A selection staff files were reviewed. A selection of children files was reviewed. All spaces were monitored for supervision, staff/child ratios, materials, equipment and required postings. A checklist was used to note the requirements I monitored today. The checklist will be provided at the conclusion of the visit. The following violations were observed and documented. Violation Number Comment Rule 847 Parent's medication authorization did not include required information. a child with a medical action plan did not have permission to administer form completed. 10A NCAC 09 .0803(4)(6-9) 1835 The medical action plan was not updated on an annual basis or when changes to the plan were made by the child's parent or health care professional. A child's medical action plan expired in 2022. .0801(b) The above violations MUST be corrected immediately. Please mail or email your letter of compliance by October 2, 2024. Any violations documented during visits to your program may have an impact on the total compliance history score and cause your score to drop below the mandated level. **Prior to today’s visit, the compliance history score for the center was 94%. Any violations cited during today's visit may affect this percentage. TECHNICAL ASSISTANCE >> One child with a medical action plan the permission to administer medication form was missing. We discussed permission to administer medication form. When a child has a medical action plan you will need two forms: the medical action plan and permission to administer medication form. Please have the parents complete the permission form and update the medical action plan. **One staff member who works in the infant room did not complete ITS-SIDS training before the required expiration date. (8/2024) You stated that she is signed up to take this training next week (9/25/24). She is not going to work in the infant room until she attends the training. CONSULTATION **There was a large branch on the playground that dropped from the storm. Please be sure debris is removed immediately. You said that you let someone know but they had not removed it. The branch was removed during the visit today. >>Please remind staff to check bottles for name and DATE. The name and date must be on the bottle not just the bag they were brought in. STAFF DISCUSSION **An staff member from the infant class asked a question about feeding babies that can hold their own bottles on a boppy pillow. I explained that those pillows are not easily cleanable and not appropriate for feeding. I suggested highchairs, bumbos, or small feeding chairs for children who can hold their bottles. The rule states: “Each infant shall be held for bottle feeding until able to hold his or her own bottle. Bottles shall not be propped. Each child shall be held or placed in feeding chairs or other age-appropriate seating apparatus to be fed. The feeding chair or other seating apparatus shall be disassembled for cleaning purposes”. Upon arrival, I was greeted by the director Airelle Lunsford. I completed a walk-through of the facility. At the time of visit, there were a total of 106 children present. The children were observed eating lunch and napping/resting on their cots. Infants were observed enjoying tummy time. LICENSE STATUS Currently this center operates with a Notice of Compliance GS110 license issues October 11, 2019. INSPECTIONS I reviewed required inspections including fire drills, emergency drills, and playground inspections. All inspections were found to be in compliance. MONITORING During today’s visit, a partial assessment was conducted, monitoring all space occupied by the children. All spaces were monitored for supervision, staff/child ratios, materials, equipment and required postings. Please be sure the teachers are completing infant sleep checks in real time (as the infants are sleeping). I reviewed sleep checks in three infant rooms and they were not complete for two of the infant rooms. In the infant B classroom the sleep checks were great and recommended the you use them as an example of how to complete the sleep checks. REMINDER CLEAN WATER/WATER TESTING Rule, 10A NCAC 41C .1001 through .1007 and 15A NCAC 18A .2816 require all child care facilities (homes and centers) to test for lead in drinking, water, identify lead-based paint hazard, and mitigate or restrict access to identified hazards. The program is free and the first step is to watch one of the pre-enrollment webinars, which are available at www.cleanwaterforUSkids.org/carolina or paste https://bit.ly/3CK-webinar into your web browser. Please check your lead water testing date. Per sanitation this must be completed every three (3) years.
