I was greeted by a teacher Youstina F. I completed a walk-through with the director Dalia Nessief. At the time of the visit, there were a total of 9 children present. The children were observed playing with materials, playdoh, completing routine tasks, working on a worksheet and completing an art project. LICENSE STATUS Currently this center operates with Notice of compliance issued August 11, 2023. I reviewed the NC Secretary of State's website and observed that the owner is listed as current/active. INSPECTIONS I reviewed all required inspections including fire drills, emergency drills, sanitation, fire and outdoor inspections. Inspections were found to be in compliance. They are scheduled to have their fire inspection on March 28th. MONITORING During today’s visit, a full assessment was conducted, monitoring all space occupied by the children, including the outdoor play environment. I reviewed all staff and a selection of children files. All spaces were monitored for supervision, staff/child ratios, materials, equipment and required postings. A checklist was used to note the requirements I monitored today. The following violations were observed and documented: Violation Number Comment Rule 303 Children were not adequately supervised at all times. Two children walked to a classroom by themselves to use the bathroom. .1801(a)(1-5) 866 A caregiver administered medication to a child without parental permission or authorization from a bona fide medical care provider during the event of an emergency medical condition when the child's parent was not available. A child with emergency medication was missing the written permission to administer medication form. 10A NCAC 09 .0803(10) 1321 Medical exam or health assessment record was not on file before or within 30 days after admission. One child's file was missing their medical exam GS110-91(1) 1835 The medical action plan was not updated on an annual basis or when changes to the plan were made by the child's parent or health care professional. A child did not have a medical action plan on file. .0801(b) 1867 The depth of the loose surfacing was not based on critical height of the equipment. The surfacing under the climbing structure was less than 2 inches. .0605(k)(1-4) Please mail or email your letter of compliance by April 5, 2024. The letter must address each violation and describe how the violations were corrected. Any violations documented during visits to your program may have an impact on the total compliance history score and cause your score to drop below the mandated level. **Prior to today’s visit, the compliance history score for the center was 99%. Any violations cited during today's visit may affect this percentage. TECHNICAL ASSISTANCE **Two children were observed walking into the classroom unsupervised to use the bathroom. You recently opened another classroom for older preschool children. This classroom does not have a bathroom or sink, so the children use the young preschool classroom. I explained that the children can not walk the classroom by themselves; a staff member must accompany them or the whole class needs to come to the bathroom. All the children must be supervised at all times throughout the day. Children can not be left alone without supervision. **One child’s file was missing their medical/health assessment. Please remember that each child must have a medical/health assessment. You stated that you will contact the child’s mother today. **One child’s file was missing the medical action plan and permission to administer medication. The child has emergency medication. **We discussed the playground and the equipment. The surfacing is not at the appropriate depth and there is a tarp exposed under the swings. I explained that the swings need to be measured. When I come to conduct the follow up visit, I will measure. REMINDERS Please check your lead water testing date.
