You, Marilynn Roberts, Operator, assisted me today. This facility currently operates with a Four-Star Child-Care License. The last annual compliance visit was conducted on 12/4/2023. You arrived home soon after I arrived at the facility. You assisted the children with exiting the vehicle. I walked through the family child care home today and observed. children were present today. You prepared lunch while the children played with materials. I observed handwashing procedures and toileting. Supervision and capacity complied. You met the developmental needs of each of the children. One infant was present. I observed documentation of current safe sleep charts. The safe sleep poster was posted. The infant was using play devices for infants. You assisted the infant and met their needs. The license was posted. Storage of hazardous products was monitored and one violation was cited and corrected during our visit. General safety requirements, and general health met compliance. Program records, fire safety, and sanitation complied. Your and one substitute’s qualifying letter are current. Your CPR/FA training is current. Violation Number Comment Rule 1103 For each child being transported, the operator or other transportation provider did not have identifying information, including the child's name, photograph, emergency contact information, and a copy of the emergency medical care information form required by Rule .1721(a )(3) in the vehicle whenever children were transported. Emergency medical care information form for three children transported was not in the vehicle when children were transported on May 8, 2024. .1723(13) 1940 Hazardous cleaning supplies and other items that might be poisonous e.g. toxic plants, were not out of reach or in locked storage when children were in care. An aerosol spray can of Lysol and Febreeze were stored on the kitchen counter top. .1719(a)(6) Child Care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. Any violation(s) documented may impact the compliance history score. The violation(s) documented must be corrected immediately. On or before 5/22/2024, I must receive a written, dated, and signed compliance letter that describes accurately and in detail how and when the violations were corrected. Please be aware any information submitted by you is legal documentation. If it is determined the information provided in the letter is not true, this may be considered falsification of information. If sufficient information is not received by the due date, a follow-up visit will be conducted. An example is: Jane Doe, Administrator AAA Child Care ID # 12345678 TECHNICAL ASSISTANCE: Motor vehicle crashes are the leading cause of death in children two to fourteen years of age in the United States. It is necessary for the safety of children to require that the caregiver/teacher comply with requirements governing the transportation of children in care, in the absence of the parent/guardian. This is helpful in identifying children in the event of an emergency/accident. Each child transported must have emergency medical information form in the vehicle when transported. Today you stated you would make a copy of the child’s and attach the child’s picture to them. Intentional planning of the environment ensures a safe environment has been created which prevents and reduces injuries to young children. Items that require locked storage were improperly stored in the unlocked on the kitchen countertop. You explained you use the items when the infant has a stinky diaper. You immediately locked the items up. unlocked storage closet, and on shelves and in unlocked cabinets in classrooms. You can find the list of hazardous items and how to store them in the provider documents tab on DCDEE website. One other adult was present with you. You explained the other adult is a grandparent that visits and is never left with the kids. Today you explained the substitute provides care only on some weekends or some evenings when you work. CONSULTATION LEAD TESTING - You have completed the lead water testing and received the results. You have added a water filter to the kitchen sink. You also completed the pre-enrollment webinar for asbestos and lead paint. UPDATES/RESOURCES: Be sure to visit DCDEE website and access the What’s New section for the newest child care rules that were effective January 1, 2024. You may visit MOODLE and complete training for the new child care requirements effective January 1, 2024. You, Marilyn Roberts, Operator, assisted me with the visit. Your program currently operates with a Four-star license. The facility was monitored for health and safety requirements both indoors and outdoors. The program requirements, equipment and furnishings were also observed. A checklist was used to monitor compliance. Your file was monitored during today’s visit. Children records were monitored. Program records were monitored and one violation related to EPR plan was documented. The last annual compliance visit was conducted 2/2/23. The facility’s compliance history was reviewed with you. The program’s compliance history was one hundred percent prior to today’s visit. Children were completing an art activity and playing in the activity area. You assisted children with using materials and provided supervision. Violation Number Comment Rule 1876 The operator did not review the EPR Plan annually or when information in the plan changed to ensure all information is current. Recently, this area was under a tornado warning with a confirmed tornado touching down a few miles from here. Although this occurred on the weekend, keeping the EPR plan current will aid you and emergency responders with a plan of safety and reunification with parents in case you are impacted when children are present. You will access https://ncchildcare.ncdhhs.gov/Provider/Training-and-Professional-Development/Emergency-Preparedness-and-Response to get all the information regarding the EPR plan and how to access and update your plan. Rated License: Your facility is in cohort one. Your preparation year started July 1, 2023. I recommend you consider completing the following tasks and activities that will help prepare you for your reassessment year, which will be sometime between July 1, 2024 and June 30, 2025. If possible, we will align the reassessment with your annual compliance visit timeframe. • Have all staff update WORKS accounts to reflect all currently completed coursework (DCDEE-WORKS) • Determine whether you wish to have Environment Rating Scale (ERS) Assessments [ITERS-R, ECERS-R, SACERS-U, FCCERS-R] conducted • Request technical assistance with your child care consultant and local partners • Participate in local CCR&R and NCPC quality related training/workshops (CCRR Information) • Reach out to your local Community College to discuss educational opportunities • Review NCRLAP website ERS resources (www.NCRLAP.org). To help review ERS requirements and think carefully about the current characteristics and practices in your program and classrooms, check out the Thinking More worksheets. A worksheet is available for each subscale of each rating scale. • Family child care home providers consider the FCCERS-R self-study process and timeline for completion since it must occur over a 3-month time period prior to your rated license reassessment • Consider requesting an ERS assessment (free of charge) during the preparation year. Remember the scores can be used in a variety of ways o ERS assessments can be requested during the preparation year and scores can be used as guidance to enhance program quality o If desired, assessment scores can be used during the preparation year if the operator wants to move forward with a rated license reassessment o Assessment scores can be saved to use during the reassessment year o Facilities can request another ERS assessment (free of charge) in your reassessment year for any ERS assessment scoring less than a 5.0 Updates/Resources: Make sure you are using the most recent child care requirements dated July 2023.
