This facility operates with a Five (5) Star license issued on 8-14-20. The last annual compliance visit was conducted on 5-31-24. The last fire inspection was completed on 9-13-24. The last sanitation inspection was completed on 3-25-25 with a “Superior” classification. Today’s visit was conducted with you, Ms. Stacey Evans, Lead Teacher. This facility has two (2) NC Pre-K classrooms occupying spaces #1505 and #1501. The NC Pre-K program operates for a minimum of 7.0 hours a day from 8:00am – 3:00pm and attendance documenting time of arrival and departure is taken daily. USDA approved meals are served and menus are planned at least one week in advance. The center uses the approved Creative Curriculum Teaching Strategies Gold (TSG) as an early childhood teaching resource. The NC Pre-K classroom abides by a 1:9 staff/child ratio. NC Pre-K paperwork was reviewed during today’s visit. Four (4) children’s files were randomly selected verifying health assessments and/or developmental screenings were on file. The DCDEE NC Pre-K Program Monitoring Tool is used as a guide to ensure compliance with NC Pre-K requirements. During circle time the children discussed their recent visits to their new kindergarten classrooms. The conversation evolved into what transportation they would take to school and the safety needed when crossing the crosswalk at school. The children painted with sponge paintbrushes on large cardboard letters that will be used during their graduation ceremony. The children also engaged in outdoor gross motor play within the fenced playground. They were observed coming inside and completing toileting and handwashing routines prior to eating lunch. The facility was monitored for staff/child ratios, supervision, discipline, and health and safety requirements indoors and outdoors. The program requirements, along with equipment and furnishings were also observed. Compliance with staff records was documented by you and your new staff records, along with three (3) children’s records were reviewed today. This facility is not approved for transportation. There are children with medication in both classrooms and the permission to administer the medications were observed. A full assessment of applicable child care requirements was conducted today. The following violation of child care requirements was cited and must be corrected immediately. Technical assistance on how to maintain requirements was provided. Violation Number Comment Rule 842 A drug or medication was administered without written authorization and/or instructions from a child's parent or authorized health professional. In space #1501 permission to administer an Epinephrine Injection, USP expired on 2-28-25. 10A NCAC 09 .0803(1)(a & b) The above violation must be corrected immediately and a letter must be sent to me no later than 6-12-25 stating how the item was corrected and a plan to maintain compliance with the requirements. Any violation(s) documented may impact the compliance history score. • Technical Assistance was provided with the following suggestions that may be beneficial: Medication Authorization: Any medication on the premises must be labeled with the child’s name, be accompanied by a permission slip (good for 6-months for asthma and allergy medication), filled out completely with the parent/guardian’s signature and date. Set a reminder to review/update medications in each classroom. You must remember the “five rights” of medication use: the right child, the right medication, the right time, the right dose, and the right route—all of which are generally regarded as a standard for safe medication practices. You and the caregivers must know what medication the parent/guardian authorized to be administered and how the parent/guardian wants medication given. If documentation is required after the medication is administered, then documentation with the required information must be completed and kept on file for six months. Having clear, detailed procedures for obtaining administered medication protects children from mistakes that can affect their health. It was suggested to reiterate in your monthly newsletter to parents that medication must accompany a permission slip and it was suggested all staff complete training on administering medications. Access to the ABCMS requires the successful completion of a Moodle course consisting of a short video followed by a test. The course is titled ABCMS Child Care Provider Portal Training - under the Early Childhood Professional Development Tab, then under the Criminal Background (CBC) tab. A handout titled Creating Connecting Applications in the Automated Background Check Management System (ABCMS) was provided today that provides instructions for connecting/linking you and your staff. This facility currently operates with a Five (5) Star License issued on 8-14-20. The last annual compliance visit was conducted on 6-2-23. Today’s visit was conducted with you, Stacey Evans, lead teacher. Upon arrival to the facility, I was greeted by you. The children were in circle time where they discussed the letter of the week, letter “S” and they talked about buildings. One caregiver was observed reading a book about buildings. During circle time there was also discussion about classroom rules and friendships. After circle time the children went to interact in activity centers. Toileting and handwashing routines were completed during the visit. Today I did a walk-through visiting the two (2) classrooms where children are in care. Compliance with capacity, supervision, discipline, attendance, program requirements, storage of hazardous substances, CPR & First Aid, permit restrictions, the required postings, and your staff record documentation was monitored as a partial assessment of applicable child care requirements. Medication and permission to administer medication was monitored. The following violation of child care requirements was cited during today’s visit and must be corrected immediately. Please update your Emergency Medical Care Plan to include names of staff in both classrooms that may be available to complete the items listed on the Emergency Medical Care Plan. • It is suggested that you add a sign on the cabinet door that reads “Emergency Medications Here.” • As stated in G.S. 110-90.2 & .2703(r) child care operators are to notify the Division of any new child care providers working who were hired or who have moved into the family child care home within five business days. The process of notifying the Division has changed and is now captured in the ABCMS Portal. This change has been in effect since February 2024. Effective immediately, you will need to obtain a Business NCID and complete the ABCMS Portal training in Moodle at https://www.dcdee.moodle.nc.gov/course/view.php?id=119. Access to the ABCMS requires the successful completion of the ABCMS Portal Moodle course consisting of a short video followed by a test. This facility currently operates with a Five (5) Star License issued on 8-14-20. The last annual compliance visit was conducted on 6-2-23. Today’s visit was conducted with you, Stacey Evans, lead teacher. Upon arrival to the facility, I was greeted by you. The children were in circle time where they discussed the letter of the week, letter “S” and they talked about buildings. One caregiver was observed reading a book about buildings. During circle time there was also discussion about classroom rules and friendships. After circle time the children went to interact in activity centers. Toileting and handwashing routines were completed during the visit. Today I did a walk-through visiting the two (2) classrooms where children are in care. Compliance with capacity, supervision, discipline, attendance, program requirements, storage of hazardous substances, CPR & First Aid, permit restrictions, the required postings, and your staff record documentation was monitored as a partial assessment of applicable child care requirements. Medication and permission to administer medication was monitored. The following violation of child care requirements was cited during today’s visit and must be corrected immediately. Technical assistance on how to maintain compliance was provided. Violation Number Comment Rule 842 A drug or medication was administered without written authorization and/or instructions from a child's parent or authorized health professional. Permission to administer Ventolin HFA for a child in space #1505 was not available. 10A NCAC 09 .0803(1)(a & b) The above violation must be corrected immediately and a letter must be sent to me no later than 2-26-25 stating how the item was corrected and a plan to maintain compliance with the requirements. Any violation(s) documented may impact the compliance history score. • Technical Assistance was provided with the following suggestion that may be beneficial: Medication Authorization: Any medication on the premises must be labeled with the child’s name or a prescription label, be accompanied by a permission slip, filled out completely with the parent/guardian’s signature and date. Set a reminder to review/update medications in each classroom. You must remember the “five rights” of medication use: the right child, the right medication, the right time, the right dose, and the right route—all of which are generally regarded as a standard for safe medication practices. You and the caregivers must know what medication the parent/guardian authorized to be administered and how the parent/guardian wants medication given. If documentation is required after the medication is administered, then documentation with the required information must be completed and kept on file for six months. Having clear, detailed procedures for obtaining administered medication protects children from mistakes that can affect their health. It was suggested to reiterate in your monthly newsletter to parents that medication must accompany a signed permission slip and it was suggested all staff complete training on administering medications.
