This facility currently operates with a Three (3) Star License that was issued on 2-24-20. The last sanitation inspection was completed on 5-24-24 with an “Superior” classification. The last approved fire inspection was conducted on 9-30-24. The NC Secretary of State website was reviewed today and this facility’s corporation’s name, Southeast Childcare Services Inc. was listed as “Current-Active.” Today’s visit was conducted with you, Maria Marshall, owner/director. I monitored the four (4) classrooms in use for compliance with staff/child ratios, supervision, and staff interaction/discipline. The program was also monitored for health and safety requirements both indoors and outdoors using the annual compliance monitoring checklist. Children completed daily activities with adequate supervision and engagement from the caregivers. The children were observed interacting in activity centers, running about outside on the playground, at the table building with Legos, and at the table making paper plate Santa hats. Lunch was served and the children completed handwashing and toileting routines before naptime, where the children rested on cots with linens. The program requirements, along with equipment and furnishings were observed. Compliance with staff records was documented by you. All staff records, along with children’s records were reviewed today. No children have medications to be administered, and no medications were observed on the premises. This facility does not provide transportation. The following violations of child care requirements were cited during today’s visit and must be corrected immediately. Technical assistance on how to maintain compliance was provided. Violation Number Comment Rule 1048 All staff did not successfully complete certification in First Aid appropriate to the age of children in care. Verification of staff completion of First Aid training from an approved training organization was not in the staff file. First Aid training for two (2) staff members expired on 8-1-24. .1102(c) 1049 All staff did not successfully complete certification in CPR training appropriate to the age of the children in care. Verification of staff completion of the CPR course from an approved training organization was not in the staff file. CPR certification for two (2) staff members expired on 8-1-24. .1102(d) 1324 Signed and dated statement by parent that discipline policy received and explained at enrollment was not in child's file. One child's file did not have a signed and dated discipline policy. .1804(c) The above violations must be corrected immediately and a letter must be sent to me no later than 12-30-24 stating how the items were corrected and a plan to maintain compliance with the requirements. The following were discussed with you during today’s visit: • Child care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4)(d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. Any violation(s) documented may impact the compliance history score. • Technical Assistance was provided with the following suggestions that may be beneficial: CPR & First Aid: Maintaining current CPR and First Aid training is crucial for staff because they equip staff with the skills to respond effectively to potential emergencies with children, which can include choking, severe bleeding, allergic reactions, or cardiac arrest, potentially saving a child's life by providing immediate care until professional medical help arrives. CPR and First Aid training is especially important as children are naturally curious and active, making them more prone to accidents. It was suggested that you mark your calendar and set a reminder one month prior to all staff members CPR and First Aid expiring so you can schedule renewal of your training. As well, having all staff take CPR and First Aid together will eliminate having to keep track of multiple dates of the CPR and First Aid renewal dates. Discipline Policy Receipt: Having a discipline policy signed by parents in the children’s files is important because it maintains a safe and respectful environment, provides guidelines for behavior expectations, ensures consistency in addressing behavior issues, and promotes a positive and inclusive atmosphere for children to thrive. It was suggested to make packets of the information that is required in each child’s file. When parents enroll their children, a packet already prepared to include the discipline policy can be given to each parent. The prepared packet of children’s file paperwork ensures that no forms are missing. Once the parent fills out the paperwork, review the paperwork to ensure that all the required children’s file paperwork has been submitted by the parent. Use the children’s file checklist as a tool to monitor for compliance with each child’s file. A copy of the children’s file checklist was provided for you today. • There were blank incident report forms and copies of the summary of the North Carolina Child Care Law and Rule for Child Care Centers occupying space the children’s files. It was explained that the blank incident report forms should not be maintained in the children’s files and that the North Carolina Child Care Law and Rule for Child Care Centers should be returned to the parent for their future review. • Hold harmless has been extended until the new Quality Rating Improvement System (QRIS_ Star Rated License System rules are implemented (SB 425). The Governor signed Senate Bill 425 (now Session Law 2024-34), which includes the Quality Rating Improvement System (QRIS) modifications recommended to the Legislature by the NC Child Care Commission to update QRIS and create additional pathways for licensed child care facilities to earn star rated licenses. The Child Care Commission is currently in the process of drafting proposed child care rule language to begin the rulemaking process for implementing the new requirements. There will be three pathways for the QRIS Modernization: program assessment, classroom and instructional quality, and national accreditation. • Keep an eye on your inbox for the 2024 annual license fee invoice. Invoices were emailed to the facility address on file by 11-30-24. Online payments will be due by December 31, 2024. For help with the annual license fee, contact DCDEE_LF@dhhs.nc.gov. • Complete the Annual Immunization Report by 1-15-25. The reporting period for child care immunizations is now open. Reporting for 2024-2025 has been extended due to Hurricane Helene. Children have a grace period from requirements until 11-30-24, and reports will be accepted until 1-15-25. This facility currently operates with a 3 Star License that was issued on 2-24-20. Today’s visit was conducted with you, Maria Marshall, owner/center director. Upon arrival to the facility, I was greeted by staff. You arrived shortly afterwards. I did a walk-through of the three (3) classrooms in use, occupied by children. Compliance with capacity, supervision, discipline, attendance, program requirements, storage of hazardous substances, CPR & First Aid, permit restrictions, the required postings, and staff record documentation was monitored as a partial assessment of applicable child care requirements. You stated that there are no children needing medication and no medication or permission to administer medication was observed. You stated that the facility does not provide any transportation and that at this time there are no children enrolled on second shift. During the visit, the caregivers were observed interacting with the children on the playground as the children rode in cozy cars, pushed push toys, rode on riding toys, played with balls, and built structures with waffle blocks and giant Lego blocks at tables outside. Once inside the children washed their hands and sat at the table to be served lunch. A partial assessment of child care requirements was monitored. The following violations were cited during today’s visit and must be corrected immediately. Technical assistance on how to maintain compliance was provided. Violation Number Comment Rule 808 The child care center premises, including the outdoor learning environment, was not clean, drained to minimize standing water, free of litter and hazards, and/or maintained in a manner which does not create conditions that attract or harbor pests. Near the wooded area, there was vegetation growing wildly all along the right side and right rear side of the outdoor playground fence. 15A NCAC 18A .2832(a) 1757 A valid qualification letter was not on file and available to review at the facility. A caregiver hired on 9-9-24 had a qualifying letter on file that expired on 3-13-24. G.S. 110-90.2(b) & (d) & .2703(e) The above violations must be corrected immediately and a letter must be sent to me no later than 9-24- 24 stating how the items were corrected and a plan to maintain compliance with the requirements. We discussed as an administrator, it is your responsibility to review all employment file documentation and verify that required documents are received and employment onboarding tasks are completed within the specified time frame, using the Staff File Checklist. The following were discussed with you during today’s visit: • Vegetation on the fence - There are plants and weeds that are poisonous and can cause an allergic reaction or are hazardous because of possible spikes. Children are at risk because they like to explore and may accidentally touch and taste potentially harmful plants that may be growing along the fence. Adequately supervising the children is the best way to avoid danger on the playground and it is also very important to make your playground safe, clearing away all of the vegetation along the fence. • Criminal background checks - Criminal background checks are completed to protect the children from abuse and reduce liability risks. The Criminal Background Check process must be completed before a person can begin work; therefore, a current Qualification Letter must be on file prior to employment. Any employee in a licensed child care facility who is ever left alone with a child or group of children for any length of time must have a background check completed through the Division of Child Development and Early Education (DCDEE). A Criminal Background Check must be completed every five years. An expiration date is listed on each Qualification Letter. Submissions can be made six months before the expiration date. Operators will need to be proactive and make staffing decisions and arrangements in advance to avoid non-compliance with this and other DCDEE staffing requirements. Having applicants submit a copy of their Qualification Letter along with their resume and application can facilitate a shorter selection and hiring process. Criminal background checks must be completed online. To complete online criminal background check applications, go to www.ncchildcare.nc.gov/general/dhhscrc.asp. The $26.50 payment is a part of the online criminal background check application process. Fingerprints must be completed through LIVESCAN at your local county sheriff’s office. If you have any questions regarding the criminal background check process, dial 1-800-859-0829 and ask for the criminal background unit. • Your annual fire inspection is due by October 27, 2024. Please get your equipment serviced, conduct your fire training and request your inspection asap. • Hold harmless has been extended until the new QRIS (Star Rated License System) rules are implemented (SB 425). Providers in Cohort 1 are not required to continue with a rated license assessment unless preferring to, and providers in Cohort 2 do not need to start their preparation year assessment unless preferring to move forward with an assessment. Prep year scales and assessment year scales are optional and up to your discretion at this time. Your scales window was scheduled for next month and you elected to cancel the environmental rating scales assessments which was received via email. NCRLAP was also notified. • Update your EPR Plan/Ready To Go File to ensure children no longer enrolled are removed and that the Ready to Go File is organized. It was suggested to reprint your EPR Plan and add binder dividers with tabs to label each section that is required in the Ready to Go File. Refer to your Ready to Go File checklist that is in your Ready to Go File binder. • We discussed that play is an important part of a child’s development. Playing outside in the fresh air can be fun and adventurous. With careful planning and monitoring, play environments can be safe for children. There have been numerous reports of snakes on the playground in outdoor environments. Remember child care rule 10A NCAC 09 .0604 reads that once a day, prior to initial use, the indoor and outdoor premises shall be checked for debris, vandalism, and broken equipment, the temperature of outdoor equipment, in addition look for other hazards and/or snakes. Handouts with information regarding burn safety and protecting children from thermal burns were provided today. • A suggestion was also made to save the most recent child care requirements in Chapter 9 effective January 2024, to your desktop computer for easy access. As well as, bookmark the Division of Child Development and Early Education on your computer for access to valuable resources like, the criminal background check link, forms and documents, what’s new, etc. • Please connect/link you and your staff in the criminal record check portal asap. Center directors/owners can now access the Automated Background Check Management System (ABCMS), the DCDEE’s criminal background check system. This access will allow providers to: • See the real-time background check status of staff members. • Run a printable report of the staff roster to assist with compliance visits. • See new background check applicants and add to staff roster. ABCMS requires the successful completion of a Moodle course consisting of a short video followed by a test. The course is titled ABCMS Child Care Provider Portal Training - under the Early Childhood Professional Development Tab, then under the Criminal Background (CBC) tab. Ensure staff know their NCID Username and Password. Please use your Bright Wheel curriculum, along with the North Carolina Foundations for Early Learning and Development, found at https://ncchildcare.ncdhhs.gov/Portals/0/documents/pdf/N/NC_Foundations.pdf, as the NC Foundations for Early Learning and Development serves as a set of standards in North Carolina that outlines developmental goals for children from birth to kindergarten, guiding teachers in planning learning experiences and informing parents about what developmental expectations to anticipate for their children at each stage. This facility currently operates with a 3 Star License that was issued on 2-24-20. Today’s visit was conducted with you, Maria Marshall, owner/center director. Upon arrival to the facility, I was greeted by staff. You arrived shortly afterwards. I did a walk-through of the three (3) classrooms in use, occupied by children. Compliance with capacity, supervision, discipline, attendance, program requirements, storage of hazardous substances, CPR & First Aid, permit restrictions, the required postings, and staff record documentation was monitored as a partial assessment of applicable child care requirements. You stated that there are no children needing medication and no medication or permission to administer medication was observed. You stated that the facility does not provide any transportation and that at this time there are no children enrolled on second shift. During the visit, the caregivers were observed interacting with the children on the playground as the children rode in cozy cars, pushed push toys, rode on riding toys, played with balls, and built structures with waffle blocks and giant Lego blocks at tables outside. Once inside the children washed their hands and sat at the table to be served lunch. A partial assessment of child care requirements was monitored. The following violations were cited during today’s visit and must be corrected immediately. Technical assistance on how to maintain compliance was provided. Violation Number Comment Rule 808 The child care center premises, including the outdoor learning environment, was not clean, drained to minimize standing water, free of litter and hazards, and/or maintained in a manner which does not create conditions that attract or harbor pests. Near the wooded area, there was vegetation growing wildly all along the right side and right rear side of the outdoor playground fence. 15A NCAC 18A .2832(a) 1757 A valid qualification letter was not on file and available to review at the facility. A caregiver hired on 9-9-24 had a qualifying letter on file that expired on 3-13-24. The following were discussed with you during today’s visit: • Child care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4)(d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. Any violation(s) documented may impact the compliance history score. • Technical Assistance was provided with the following suggestions that may be beneficial: - Organization and completeness of records is important to ensure easy access to information in emergency situations. We discussed as an administrator, it is your responsibility to review all employment file documentation and verify that required documents are received and employment onboarding tasks are completed within the specified time frame, using the Staff File Checklist. The following were discussed with you during today’s visit: • Vegetation on the fence - There are plants and weeds that are poisonous and can cause an allergic reaction or are hazardous because of possible spikes. Children are at risk because they like to explore and may accidentally touch and taste potentially harmful plants that may be growing along the fence. Adequately supervising the children is the best way to avoid danger on the playground and it is also very important to make your playground safe, clearing away all of the vegetation along the fence. • Criminal background checks - Criminal background checks are completed to protect the children from abuse and reduce liability risks. The Criminal Background Check process must be completed before a person can begin work; therefore, a current Qualification Letter must be on file prior to employment. Any employee in a licensed child care facility who is ever left alone with a child or group of children for any length of time must have a background check completed through the Division of Child Development and Early Education (DCDEE). A Criminal Background Check must be completed every five years. An expiration date is listed on each Qualification Letter. Submissions can be made six months before the expiration date. Operators will need to be proactive and make staffing decisions and arrangements in advance to avoid non-compliance with this and other DCDEE staffing requirements. Having applicants submit a copy of their Qualification Letter along with their resume and application can facilitate a shorter selection and hiring process. Criminal background checks must be completed online. To complete online criminal background check applications, go to www.ncchildcare.nc.gov/general/dhhscrc.asp. The $26.50 payment is a part of the online criminal background check application process. Fingerprints must be completed through LIVESCAN at your local county sheriff’s office. If you have any questions regarding the criminal background check process, dial 1-800-859-0829 and ask for the criminal background unit. • Your annual fire inspection is due by October 27, 2024. Please get your equipment serviced, conduct your fire training and request your inspection asap. • Hold harmless has been extended until the new QRIS (Star Rated License System) rules are implemented (SB 425). Providers in Cohort 1 are not required to continue with a rated license assessment unless preferring to, and providers in Cohort 2 do not need to start their preparation year assessment unless preferring to move forward with an assessment. Prep year scales and assessment year scales are optional and up to your discretion at this time. Your scales window was scheduled for next month and you elected to cancel the environmental rating scales assessments which was received via email. NCRLAP was also notified. • Update your EPR Plan/Ready To Go File to ensure children no longer enrolled are removed and that the Ready to Go File is organized. It was suggested to reprint your EPR Plan and add binder dividers with tabs to label each section that is required in the Ready to Go File. Refer to your Ready to Go File checklist that is in your Ready to Go File binder. • We discussed that play is an important part of a child’s development. Playing outside in the fresh air can be fun and adventurous. With careful planning and monitoring, play environments can be safe for children. There have been numerous reports of snakes on the playground in outdoor environments. Remember child care rule 10A NCAC 09 .0604 reads that once a day, prior to initial use, the indoor and outdoor premises shall be checked for debris, vandalism, and broken equipment, the temperature of outdoor equipment, in addition look for other hazards and/or snakes. Handouts with information regarding burn safety and protecting children from thermal burns were provided today. • A suggestion was also made to save the most recent child care requirements in Chapter 9 effective January 2024, to your desktop computer for easy access. As well as, bookmark the Division of Child Development and Early Education on your computer for access to valuable resources like, the criminal background check link, forms and documents, what’s new, etc. • Please connect/link you and your staff in the criminal record check portal asap. Center directors/owners can now access the Automated Background Check Management System (ABCMS), the DCDEE’s criminal background check system. This access will allow providers to: • See the real-time background check status of staff members. • Run a printable report of the staff roster to assist with compliance visits. • See new background check applicants and add to staff roster. ABCMS requires the successful completion of a Moodle course consisting of a short video followed by a test. The course is titled ABCMS Child Care Provider Portal Training - under the Early Childhood Professional Development Tab, then under the Criminal Background (CBC) tab. While children were present in space #2, there was a box of Bojangles chicken observed on the table. .0901(i) The following were discussed with you during today’s visit: • Child care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4)(d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. Any violation(s) documented may impact the compliance history score. • Technical Assistance was provided with the following suggestion: • We discussed that staff should eat their lunch in the kitchen, in the classroom where no children are enrolled/present or outside where there are no children present. Modeling healthy eating in children's presence is important because children are more likely to stick to good habits they see, especially when it comes to food. Research suggests that children's experiences with food early on in life can influence their preferences and intake, and that the earlier and broader these experiences are, the healthier their diet will be. • Adding additional shade to the playground was discussed since there was more than half of the playground with no shade. • Add more names of staff to your Emergency Medical Care Plan to ensure there is always someone listed that is present during operating hours, on the Emergency Medical Care Plan that can complete the tasks in case of an emergency. • You stated that there have been no incident reports written due to there being no injuries. You were reminded that incident reports must be filled out and signed by the parent/guardian when a child is injured at the facility. If the child seeks medical attention, then the incident report must be mailed/emailed to me within seven (7) calendar days. Once the incident report is completed, then place the original incident report in the child’s file and a copy of the incident report is given to the parent/guardian. Finally, the incident is logged on the incident report log. It was suggested to maintain copies of blank incident reports and the incident log in a binder/folder for ease of access and review. • Center directors can now access the Automated Background Check Management System (ABCMS), the DCDEE’s criminal background check system. This access will allow providers to: • See the real-time background check status of staff members. • Run a printable report of the staff roster to assist with compliance visits. • See new background check applicants and add to staff roster.
