This facility currently operates with a Three (3) Star License issued on 12-22-17. The last annual compliance visit was conducted on 10-15-24. Today’s visit was conducted with you, Patricia Ewing, owner/operator. Upon arrival to the facility, I was greeted by you. A walk-through of the residence was conducted visiting the one (1) bedroom used for childcare. The required postings, along with appropriate equipment and furnishings were observed. The emergency drills, outdoor inspections, the activity plan, daily attendance, and children’s files were also observed. Documentation of program records, as well as the operator verification information was observed and has been documented on the attached form. Your vehicle was monitored for transportation requirements. You stated no medications are being administered at this time. The children were eating breakfast at the table. Once breakfast was completed, handwashing routines were completed. The infant in care was observed on the floor moving about with various age appropriate toys. The schoolagers in care were reading books and reciting sight words. You completed diapering and handwashing routines as required. A caring and nurturing environment was observed as you interacted with the children and attended to their needs. During today’s visit, an assessment of applicable child care requirements was conducted. The following violations of child care requirements was cited and must be corrected immediately. Technical assistance on how to maintain compliance was provided. Violation Number Comment Rule 714 Monthly check for hazards on the outdoor play area was not completed using a form supplied by the Division. Documentation of a check for hazards on the outdoor play area was not available for September 2025. 10A NCAC .1721(e)(5)(A-F) 907 Written record of up-to-date pet vaccinations as required by NC law and local ordinances, including rabies vaccination for dogs and cats, was not available. Rabies vaccinations for your feline, Ashlynn expired on 8-24-25. .1719(b)(1) 1853 The operator did not conduct a monthly fire drill. A fire drill was not conducted for the month of September 2025. .1719(a)(15) & .1721( e)(2) The above violations must be corrected immediately and a letter must be sent to me no later than 10-29-25 stating how the items have been corrected and a plan to maintain compliance with the requirements. Any violation(s) documented may impact the compliance history score. • Technical Assistance was provided with the following suggestions that may be beneficial: Monthly Fire Drills & Outdoor Playground Inspections: Conducting monthly fire drills help reduce the potential for panic and confusion during an actual fire, as you and the children are familiar with the procedures. By practicing various scenarios, both you and children become better prepared to respond effectively during an emergency. These drills confirm the reliability of the emergency plan and identify areas for improvement, which are essential for maintaining a safe environment. Overall, monthly fire drills are a vital component of emergency preparedness in child care settings. It is strongly suggested to set a reminder in your phone monthly, mark your calendar monthly and conduct your fire drill and outdoor inspections on the same day. It is important to immediately document the fire drill/outdoor inspection on the required emergency drill log and outdoor inspection form in order to ensure you do not forget to log that you conducted the fire drill and outdoor inspections as required. Pet Vaccination: Rabies vaccinations are essential for cats and dogs because rabies is a fatal viral disease that affects the brain and nervous system of cats/dogs and humans. Once symptoms appear, it is almost always fatal. Vaccinated cats/dogs help prevent the spread of rabies to others—especially children, who are more vulnerable. It was suggested to please mark your calendar and set a reminder in your phone at least two months prior to the date of the rabies vaccination expiring. This provides ample time to get a new rabies vaccination. • You have completed the ABCMS Moodle training now ASAP connect/link you and your husband. FCCH owners can now access the Automated Background Check Management System (ABCMS), the DCDEE’s criminal background check system. This access will allow providers to see the real-time background check status of you and household members. Instructions were emailed to you today on how to connect/link in the ABCMS portal. This facility currently operates with a Three (3) Star License that was issued on 12-22-17. Today’s visit was conducted with you, Patricia Ewing, owner/operator. Upon arrival to the residence, I was greeted by you. The one (1) infant enrolled was observed having tummy time with age appropriate toys. You were doing mobility exercises with the infant, singing and reading a book to the infant. You were nurturing and communicative as you met the infant’s needs. I monitored the room to right, at the front door that is used for child care. Compliance with capacity, supervision, discipline, attendance, program requirements, storage of hazardous substances, CPR & First Aid, permit restrictions, the required postings, and staff record documentation was monitored as a partial assessment of applicable child care requirements. You stated no medications are given at this time and no permission to administer medications was observed. A partial assessment of child care requirements was monitored. The following violation was cited during today’s visit and must be corrected immediately. Technical assistance on how to maintain compliance was provided. Violation Number Comment Rule 701 All indoor and outdoor areas used by the children were not kept clean, orderly, and free of items which are potentially hazardous to children including removal of items a child can swallow; the removal of loose nails or screws and splinters on inside; and use of outdoor equipment that is too hot to touch. The grass within the fenced yard where the children play was not maintained in a manner which does not create conditions that attract or harbor pests. .1719(a)(1)&(17) The above violation must be corrected immediately and a corrective action letter must be sent to me no later than 5-29-25 stating how the violation has been corrected and a plan to maintain compliance with the requirements. We discussed that a well-maintained outdoor area can reduce the chances of accidents and promote a safe and fun play space. A regular maintenance routine should be in place to ensure that the outdoor play area is free of hazards, debris and that the grass is maintained in a manner which does not create conditions that attract or harbor pests. By integrating consistent cleaning practices and proactive maintenance routines, you can create an outdoor space that is not only safe and hygienic but also inviting and conducive to positive play experiences. This commitment to cleanliness and care ultimately supports children’s safety and well-being, creating a safe and enjoyable environment for them to grow and explore. • Please link you and your husband asap. This facility currently operates with a Three (3) Star License that was issued on 12/22/17. The last annual compliance visit was conducted on 10-20-23. Today’s visit was conducted with you, Patricia Ewing, owner/provider. Upon arrival to the residence, I was greeted by you. The children were observed in the bedroom used for childcare, which is to the right as you enter the front door. The children were in circle time where you were working with the children on spelling and reading. The children were reading the book “Big Thoughts for Little People.” You were helping the children sound out words they couldn’t pronounce. The children expressed their thoughts and mood as you encouraged them to talk about feelings. The residence was monitored to ensure compliance with safe indoor environment requirements and the fenced side/back yard where the children engage in gross motor play was observed to ensure compliance with safe outdoor environment requirements. The required postings, along with appropriate equipment and furnishings were observed. The EPR Plan, emergency drills, outdoor inspections, the activity plan, and daily attendance. You added that currently there are no children with medication to administer. Today as I was having computer issues, you stated that it was one of the related children’s birthday and you indicated that you had birthday lunch reservations and had to leave. Before you and the children left the residence, I was able to monitor the vehicle for transportation requirements. I explained that I would come back to finish monitoring paperwork. An assessment of applicable child care requirements was conducted. The following violations of child care requirements were cited and must be corrected immediately. Violation Number Comment Rule 1103 For each child being transported, the operator or other transportation provider did not have identifying information, including the child's name, photograph, emergency contact information, and a copy of the emergency medical care information form required by Rule .1721(a )(3) in the vehicle whenever children were transported. Children were transported without identifying information, including their photograph. .1723(13) 1997 Vehicle used for transportation did not have a fire extinguisher. The 2018 Nissan Armanda used for transportation did not have a fire extinguisher in the vehicle. .1723(11) The above violations must be corrected immediately and a corrective action letter must be sent to me no later than 10-31-24 describing accurately and in detail, how and when the violations were corrected and a plan to prevent future violations. Any violation(s) documented may impact the compliance history score. • Technical Assistance was provided with the following suggestions that may be beneficial: • Transportation: A fire extinguisher is crucial to have in the vehicle when transporting children because it provides a vital tool to potentially contain or extinguish a small fire in its early stages, which is especially important when transporting children who may not be able to evacuate quickly in an emergency situation. Having readily accessible fire suppression equipment can significantly increase the safety of the children in the vehicle during transport. When storing the fire extinguisher, you must secure it in the vehicle, so it does not move around and potentially become a projectile object. Do not remove the fire extinguisher to ensure that it is always in place when transporting children. We also discussed the importance of identifying information in the vehicle. Identifying information in a vehicle when transporting children is crucial for ensuring their safety in case of an emergency, allowing you to quickly contact parents or guardians, provide necessary medical details, and properly account for each child during transport. As well, in case of an accident or medical emergency, having readily accessible information like the child's name, a photograph, contact details, and any relevant medical conditions allows first responders to quickly reach out to parents and provide appropriate care. It was suggested to always leave your Mickey Mouse binder with the transportation emergency information in the vehicle to ensure it is in place and not left in the residence. In addition, when a new child is enrolled, immediately take a picture of that child and attach it with their emergency information/application, placing it in your Mickey Mouse binder in the vehicle. • Your CPR and First Aid certification will expire on October 23, 2024. • A suggestion was made to save the most recent child care requirements in Chapter 9 effective January 2024, to your desktop computer for easy access. As well as, bookmark the Division of Child Development and Early Education on your computer for access to valuable resources like, the criminal background check link, forms and documents, what’s new, etc. • Please connect/link you and your husband in the criminal record check portal asap.
