Freida Dixon, director, accompanied me during a walk-through of the facility. During the visit, I discussed the allegations with Aisha Jenkins, assistant director, Ms. Dixon, and additional staff members. Limited monitoring of child care requirements occurred during today’s visit. The following violations were confirmed during today’s visit: Violation Number Comment Rule 303 Children were not adequately supervised at all times. On 4/28/25, two teachers, who were responsible for a group of 4 y/o and 5 y/o children, remained seated for nearly the entire duration of playground time. A five-year-old child sustained a sprained hand while playing. The teachers only account of how the injury happened is what the child told them. .1801(a)(1-5) 852 Incident reports were not completed each time a child was injured, it did not include all the information required in rule, it was not signed by the parent and/or it was not maintained in the child's file. An incident report was not completed on 4/28/25, the day the incident/injury took place. It was not signed by the parent. .0802 (e) 902 Each child was not attended to in a nurturing and appropriate manner, or in keeping with the child's developmental needs. On 4/28/25, a five-year-old child told their teacher they hurt their hand while playing on the slide. The child was crying. The teacher noted no swelling or lack of movement, however, the child sat down for 10 minutes afterward and didn't act like themself. No ice was applied and the child's hand wasn't checked again later in the day. G.S. 110-91(10) Violations must be corrected immediately. By May 14, 2025, you must submit documentation of the corrections you made and your plan to maintain compliance with the identified child care requirements to me. If the operator fails to correct any documented violations within the established time period, the Division of Child Development may deny, suspend, terminate, or revoke any permit to operate (10A NCAC 09 .2000). The last annual compliance visit was conducted on 9/28/2023. You, Freda Dixon, Center Director, and Aisha Jenkins, Assistant Director, assisted me today. Your program currently operates with a Five-Star license. You accompanied me as I walked through the facility. I observed for an extended amount of time. Children participated group music activity, group art activities, free play, outdoor play, and walking trip. Staff assisted children with group time activities, problem solving, and assisted with routines and transitions. Supervision, group size, permit capacity, staff/child ratios and space capacity complied. One new staff’s file was monitored. Staff’s qualifying letter and Special Trainings was monitored and one violation regarding qualifying letter was cited. Violations regarding CPR and first aid certifications were documented. Fire and sanitation inspections were current. Fire drills and playground inspections were current. One violation related to emergency drills was cited. The emergency medical care plan was accessible. ADDITIONAL REQUIREMENTS: General health requirements complied. The facility continues to meet requirements for adequate and approved space. NC Pre-K Requirements: Today I observed the Site Monitoring Tool. Requirements continue to be met for NC Pre-K requirements. Violation Number Comment Rule 1044 Prior to the expiration date of the qualification letter, the child care provider did not complete and submit required forms to complete a criminal background check (a qualification letter is valid for a maximum of five years for the date of issuance). One staff member qualification letter expired January 2024 and they have not completed and submitted required forms to complete a criminal background check. G.S. 110-90.2(b) & .2703(n)&(o) 1048 All staff did not successfully complete certification in First Aid appropriate to the age of children in care. Verification of staff completion of First Aid training from an approved training organization was not in the staff file. One staff member employed 10/23/23 has not completed first aid certification. .1102(c) 1049 All staff did not successfully complete certification in CPR training appropriate to the age of the children in care. Verification of staff completion of the CPR course from an approved training organization was not in the staff file. One staff member employed on 10/23/23 has not completed CPR training. .1102(d) 1811 Shelter-in-place or lockdown drills were not practiced every three months and/or drill record was incomplete. A lockdown drill was last practiced on 10/13/2023 and another lockdown or shelter-in-place drill has not been practiced since. .0604(u);.0302(d)(8) Child Care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. Any violation(s) documented may impact the compliance history score. The violation(s) documented must be corrected immediately. On or before 3/1/2024, I must receive a written, dated, and signed compliance letter that describes accurately and in detail how and when the violations were corrected. A full assessment of child care requirements were monitored. Today’s visit was conducted with you, Freda Dixon, Center Director and Aisha Jenkins, Assistant Director. Your program currently operates with a Five-star license, issued 4/26/2017. Your program was also monitored for compliance with implementing an approved curriculum as required for all four and five star licensed facilities where four-year-old children are enrolled. I monitored all classrooms in use for compliance with supervision, and staff interaction/discipline. Staff-child ratios and maximum group sizes required in Child Care Rule 10A NCAC 09 .3009 were also monitored and one violation related to staff/child ratio was documented. Children enrolled in playing outside, eating lunch, and resting. The facility was monitored for health and safety requirements both indoors and outdoors. The program requirements, along with equipment and furnishings were also observed. A sampling of children records and staff records were monitored and information documented on the children’s records form and the staff and training worksheet. A checklist was also used to monitor compliance. The last annual compliance visit was conducted 10/26/22. The sanitation and fire inspections were current. You explained your staff conducted the BRIGGANCE Assessment for children in that classroom, the results, and the discussion with the parents of two specific children. RATED LICENSE COHORT: Your facility is in cohort one. Beginning in July 1, 2023 to June 30, 2024 is your preparation year. I recommend you consider completing the following tasks and activities that will help prepare you for your reassessment year, which will be sometime between July 1, 2024 and June 30, 2025. A full assessment of child care requirements were monitored. Today’s visit was conducted with you, Freda Dixon, Center Director and Aisha Jenkins, Assistant Director. Your program currently operates with a Five-star license, issued 4/26/2017. Your program was also monitored for compliance with implementing an approved curriculum as required for all four and five star licensed facilities where four-year-old children are enrolled. I monitored all classrooms in use for compliance with supervision, and staff interaction/discipline. Staff-child ratios and maximum group sizes required in Child Care Rule 10A NCAC 09 .3009 were also monitored and one violation related to staff/child ratio was documented. Children enrolled in playing outside, eating lunch, and resting. The facility was monitored for health and safety requirements both indoors and outdoors. The program requirements, along with equipment and furnishings were also observed. A sampling of children records and staff records were monitored and information documented on the children’s records form and the staff and training worksheet. A checklist was also used to monitor compliance. The last annual compliance visit was conducted 10/26/22. The sanitation and fire inspections were current. Violation Number Comment Rule 318 Children between 12 and 24 months of age were grouped with children 3 years of age or older. In space two, a one-year-old child was grouped with two two-year-olds and two three-year-olds. 10A NCAC 09 .0713(a)(6) 847 Parent's medication authorization did not include required information. Authorization to administer Aquaphor and Aveeno was not on file for one child. 10A NCAC 09 .0803(4)(6-9) All violations documented today must be corrected immediately. A corrective action letter explaining how and when the violations were corrected must be sent to me no later than 10/12/2023. Be sure to include the facility’s identification number. You explained the child is a returning child from the previous school year. Authorization for Aquaphor and Aveeno expired. You will reach out to the parent to see if the medication should be returned or if an updated authorization is needed. Staff/child ratio and groupings – Today we discussed that according to Child Care Requirement .0713(a)(3) states a child two years of age and older may be placed with children under one year of age when a physician certifies that the developmental age of the child makes this placement appropriate. We also discussed Child Care Requirement .0713(a)(6) states children between the ages of 12 months and 24 months shall not be grouped with older children unless all children in the group are less than three years of age. We discussed the specific groupings for children currently enrolled in space two. You explained your staff conducted the BRIGGANCE Assessment for children in that classroom, the results, and the discussion with the parents of two specific children. RATED LICENSE COHORT: Your facility is in cohort one. Beginning in July 1, 2023 to June 30, 2024 is your preparation year. I recommend you consider completing the following tasks and activities that will help prepare you for your reassessment year, which will be sometime between July 1, 2024 and June 30, 2025. If possible, we will align the reassessment with your annual compliance visit timeframe. -Have all staff update WORKS accounts to reflect all currently completed coursework (DCDEE-WORKS) -Determine whether you wish to have Environment Rating Scale (ERS) Assessments [ITERS-R, ECERS-R, SACERS-U, FCCERS-R] conducted -Request technical assistance with your child care consultant and local partners -Participate in local CCR&R and NCPC quality related training/workshops (CCRR Information) -Reach out to your local Community College to discuss educational opportunities -Review NCRLAP website ERS resources (www.NCRLAP.org). A worksheet is available for each subscale of each rating scale. -Consider requesting an ERS assessment (free of charge) during the preparation year. Remember the scores can be used in a variety of ways …..ERS assessments can be requested during the preparation year and scores can be used as guidance to enhance program quality …..If desired, assessment scores can be used during the preparation year if the operator wants to move forward with a rated license reassessment …..Assessment scores can be saved to use during the reassessment year ..…Facilities can request another ERS assessment (free of charge) in your reassessment year for any ERS assessment scoring less than a 5.0 CONSULTATION: The Child Care Commission revised rules related to staff/child ratios for multi-age groups; cooperative arrangements; and enhanced space requirements. You may access training on these new requirements on MOODLE at ncchildcare.ncdhhs.gov. Ensure you are using the updated July 1, 2023, Child Care Rules book. If the operator fails to correct any documented violations within the established time period, the Division of Child Development and Early Education may deny, suspend, terminate, or revoke any permit to operate (10A NCAC 09 .2000).
