Upon arrival I was greeted by and completed a walk-through with the director, Ashley Winters. At the time of visit, there were a total of 25 children present. The children were observed coloring a worksheet, playing with table toys, and completing puzzles. The infants were crawling around and playing with materials. One infant was being held and drinking a bottle. LICENSE STATUS Currently this center operates with a five star license issued July 2, 2021. I reviewed the NC Secretary of State's website and observed that the owner is listed as current/active. INSPECTIONS I reviewed all required inspections including fire drills, emergency drills, sanitation, fire and outdoor inspections. MONITORING During today’s visit, a full assessment was conducted, monitoring all space occupied by the children, including the outdoor play environment. All spaces were monitored for supervision, staff/child ratios, materials, equipment and required postings. I reviewed a selection children’s and staff files. A checklist was used to note the requirements I monitored today. I left a copy of the checklist at the end of the visit. The following violations were documented: Violation Number Comment Rule 533 Human milk, formula and other bottled beverages including sippy cups, sent from child's home were not fully prepared, dated, and labeled for the appropriate child. One infant's bottles were not labeled with today's date. 15A NCAC 18A .2804(d) 847 Parent's medication authorization did not include required information. The over-the-counter permission form was completed for a prescription medication. 10A NCAC 09 .0803(4)(6-9) Please mail or email your letter of compliance by February 7, 2025. The letter must address the violations and describe how the violations were corrected. Any violations documented during visits to your program may have an impact on the total compliance history score and cause your score to drop below the mandated level. **Prior to today’s visit, the compliance history score for the center was 93%. Any violations cited during today's visit may affect this percentage. TECHNICAL ASSISTANCE >>Please remind your parents and staff that they must label the children bottles and personal items. One infant’s bottles were not labeled with today’s date. >>When a child had prescription medication, you complete the Medication Administration Permission Form. One child’s prescription ointment was completed on the Medication Administration Permission Form for Over-The-Counter Topical Medications form. Please be sure that the parents are filling out the correct forms. One printed that correct form and will give it to the parents. >>We discussed the fence panel that is leaning on the back playground. You stated that you are waiting for someone to remove and a plan to replace it. The fence cover the outside electrical panel. RATED LICENSE Hold harmless has been extended until the new QRIS (STAR rating) rules are implemented (SB 425). Providers in Cohort 1 are not required to continue their with a rated license assessment unless you want to, and providers in Cohort 2 do not need to start their preparation year assessment unless you want to. Prep year scales and assessment year scales are optional and up to your discretion at this time. Please continue to work on preparing and ensuring staff education is current in WORKS. Message from NCRLAP https://ncrlap.org/ 2024: “NC continues to use the Revised editions of each scale for official assessments. The Revised editions have spiral bindings on the side and not the top, so make sure to have the correct version when thinking about your next assessment. Beginning on February 1, 2025 the Third editions of the ECERS, ITERS, and FCCERS will be used for official assessments; these scales all have a spiral binding on the top. More information about these versions will be posted in the fall of 2024 to help programs get to know the new measures and think about future assessments”. North Carolina Child Care Providers can obtain access to the Provider Access to ABCMS after completing the Moodle training and completing the form afterward. Once providers are logged in, you can access codes to allow your staff and applicants to create a connecting application to link themselves to your facility. LEAD WATER and ASBESTOS >>Please check your lead water testing date. Per sanitation this must be completed every three (3) years and you are also required to test for asbestos and lead paint. A full assessment of child care requirements was conducted. Your program currently operates with a Five-star license. The facility was monitored for approved curriculum. Today’s visit was conducted with you, Ashley Winter, Center Director and Shaneka Sawyer Assistant Director. You visited each child care area with me. I monitored classrooms in use for compliance with staff/child ratios, supervision, capacity, group size and staff interaction/discipline. Children throughout the facility were playing outside, listening to a book being read, and playing in activity areas. Infants were resting, having floor play time, and being fed. Staff interacted with children, assisted with play, and routines. The facility was monitored for health and safety requirements both indoors and outdoors and one violation was cited. Program requirements, along with equipment and furnishings complied. All inspections and drills were current. A sampling of children’s and staff’s records was monitored and information documented on the children’s records form and staff and training worksheet. Violations related to children and staff records were documented. A checklist was also used to monitor compliance. The last annual compliance visit was conducted 3/30/23. The owner/operator is listed as current/active with the NC Secretary of State. Transportation was monitored including three vehicles used to transport children. Violation Number Comment Rule 620 All walls and ceilings including doors and windows were not kept clean and in good repair. The window sill in space two, the wall around the sink in space five, and the wall on and around a door in space ten had peeling paint. The ceiling in space ten had a large hole in the ceiling tile with the ceiling vent unsecured. 15A NCAC 18A .2825(a) 1311 Emergency medical care information was not on file in the center on the child's first day of attendance and/or was not updated as changes occurred or at least annually for each child. Emergency medical care information was not updated at least annually for three children. .0802(c) 1899 Health and safety training topics were not included as part of on-going training within five years of completing the previous health and safety training topics. One staff member did not complete the health and safety training topics and two staff members did not complete the Recognizing and Responding to suspicions of Child Maltreatment as part of on-going training within five years of the previously completed topics in 2018 and 2017. .1103(b) Child Care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. Any violation(s) documented may impact the compliance history score. The violation(s) documented must be corrected immediately. On or before 2/13/2024, I must receive a written, dated, and signed compliance letter that describes accurately and in detail how and when the violations were corrected. Please be aware any information submitted by you is legal documentation. If it is determined the information provided in the letter is not true, this may be considered falsification of information. If sufficient information is not received by the due date, a follow-up visit will be conducted. You explained the facility has been tested in the past and may be due to be tested again. Lead may have some health risks for children and adults. Children’s Records – Today you explained you will review the children’s records to make sure the required documents are in them. You may also want to visit DCDEE website regularly for any updated forms. Health and Safety Training – Today you explained you think the staff member took the training but did not provide the certificates. We discussed the training that needed to be completed every five years including. You may access the trainings on MOODLE and visit https://www.preventchildabusenc.org/ for the Recognizing and Responding to Suspicions of Child Maltreatment training. Updates/Resources: Make sure you are using the most recent child care requirements dated July 2023. You may access the newest version on DCDEE website. Today I discussed information regarding Clean Classrooms for Carolina Kids program to identify and eliminate exposure to lead and asbestos hazards in child care. You have completed the lead water testing. Due to the age of your building, you may want to take advantage of the program. Visit cleanwaterforuskids.org/Carolina to sign up and get more information. There may be money available to assist programs needing abatement. The North Carolina Health and Safety Resource Center offers a variety of training that may be conducted onsite at your facility or virtually.
