This facility currently operates with Three (3) Star License that was issued on 6-17-22. The last sanitation inspection was completed on 10-2-25 with an “Superior” classification. The last approved fire inspection was conducted on 2-3-26. The last annual compliance visit was on 6-3-25. The NC Secretary of State website was reviewed today and the corporation’s name, Regina’s Kidz academy #2 Incorporated was listed as “Current/Active.” Today’s visit was conducted with you, Shontial McAllister, owner/provider. I monitored the two (2) classrooms in use occupied by children for compliance with staff/child ratios, supervision, and staff interaction/discipline. The program was also monitored for health and safety requirements both indoors and outdoors using the annual compliance monitoring checklist. Children completed daily activities with adequate supervision and engagement from the caregivers. The children were observed having free play and the older children were observed putting together puzzles, and spelling words with magnetic letters. The children also completed toileting and handwashing routines as required. The children ate meals from home, as the parents have signed the nutrition opt out form. The program requirements, along with equipment and furnishings were observed. Compliance with staff records was documented by you. There have been no new staff members hired since the last annual compliance visit. Children’s records were reviewed today. There are no children with prescribed medications, and no medication was observed on the premises today. This facility has purchased a 2015 Honda Odyssey to transport children from school. Today the license plate registration, vehicle insurance, first aid kit, no smoking signage, fire extinguisher, tire treads, and safety of the Honda Odyssey was monitored today. As well, along with the notebook that contains the children’s emergency information, log of boarding on and off the van, and photos of each child were also observed. The following violations of child care requirements were observed during today’s visit and must be corrected immediately. Technical assistance on how to maintain compliance was provided. Violation Number Comment Rule 1044 Prior to the expiration date of the qualification letter, the child care provider did not complete and submit required forms to complete a criminal background check. A qualification letter is valid for a maximum of five years for the date of issuance). A qualification letter for one child care provider expired on 4-16-26. G.S. 110-90.2(b) & .2703(n)&(o) 1123 All vehicles used to transport children were not free of hazards. A long crack was observed in the windshield of the 2015 Honda Odyssey used for transporting children. 10A NCAC 09 .1002(a) 1124 Emergency and identifying information, including the child's name, photograph, emergency contact information and/or a copy of the emergency medical care information form was not in the vehicle for each child being transported. 10A NCAC 09 .1003(d) The above violations must be corrected immediately and a letter must be sent to me no later than 6-12-26 stating how the items were corrected and a plan to maintain compliance with the requirements. A photo of each child being transported is one small but vital piece of the emergency preparedness plan. In an accident, medical emergency, or evacuation, time is critical. A photo helps quickly identify the child if they are injured, missing, or separated from the staff. This can prevent confusion, reduce the risk of misidentification, and speed up reunification. As well, first responders may need to coordinate quickly during an incident. A recognizable photo allows them to match the child to their records without relying solely on verbal descriptions, which can be unreliable under stress. The last approved fire inspection was conducted on 2-3-26. The last annual compliance visit was on 6-3-25. The NC Secretary of State website was reviewed today and the corporation’s name, Regina’s Kidz academy #2 Incorporated was listed as “Current/Active.” Today’s visit was conducted with you, Shontial McAllister, owner/provider. I monitored the two (2) classrooms in use occupied by children for compliance with staff/child ratios, supervision, and staff interaction/discipline. The program was also monitored for health and safety requirements both indoors and outdoors using the annual compliance monitoring checklist. Children completed daily activities with adequate supervision and engagement from the caregivers. The children were observed having free play and the older children were observed putting together puzzles, and spelling words with magnetic letters. The children also completed toileting and handwashing routines as required. The children ate meals from home, as the parents have signed the nutrition opt out form. The program requirements, along with equipment and furnishings were observed. Compliance with staff records was documented by you. There have been no new staff members hired since the last annual compliance visit. Children’s records were reviewed today. There are no children with prescribed medications, and no medication was observed on the premises today. This facility has purchased a 2015 Honda Odyssey to transport children from school. Today the license plate registration, vehicle insurance, first aid kit, no smoking signage, fire extinguisher, tire treads, and safety of the Honda Odyssey was monitored today. As well, along with the notebook that contains the children’s emergency information, log of boarding on and off the van, and photos of each child were also observed. The following violations of child care requirements were observed during today’s visit and must be corrected immediately. Technical assistance on how to maintain compliance was provided. Violation Number Comment Rule 1044 Prior to the expiration date of the qualification letter, the child care provider did not complete and submit required forms to complete a criminal background check. A qualification letter is valid for a maximum of five years for the date of issuance). A qualification letter for one child care provider expired on 4-16-26. A photo helps quickly identify the child if they are injured, missing, or separated from the staff. This can prevent confusion, reduce the risk of misidentification, and speed up reunification. As well, first responders may need to coordinate quickly during an incident. A recognizable photo allows them to match the child to their records without relying solely on verbal descriptions, which can be unreliable under stress. It is strongly suggested to keep the photos up to date and staple them to each child’s emergency information that is maintained in the vehicle. Staff having this training allows them to respond quickly and effectively to emergencies like choking, falls, allergic reactions, or cardiac arrest, potentially saving a child's life by providing immediate care until professional medical help arrives. It was suggested to mark on your calendar and set an alarm on your phone one (1) year and ten (10) months from the date that you last completed CPR and First Aid training. This reminder will be to contact a trainer so the CPR and First Aid training can be completed within the two (2) year renewal timeframe. Equipment: Outdoor play can have many benefits for children’s healthy development, but spending time outside also comes with risks of illness and injury. You must help keep children safe while they learn and stay active outside. Having outdoor equipment free of hazards such as rust is important because it ensures children's safety while they play and learn outside. Rust and other hazards can lead to injuries or illness. To prevent injuries or illness, regular inspections and maintenance of outdoor equipment helps prevent accidents. Daily you should inspect equipment and the outdoor area for hazards and remove them prior to the children going outdoors. • You completed your Clean Classrooms for Carolina Kids lead in water testing on 3-30-25. • A suggestion was made to save the most recent child care requirements in Chapter 9 effective January 2025, to your desktop computer for easy access. Compliance with capacity, supervision, discipline, attendance, program requirements, storage of hazardous substances, CPR & First Aid, permit restrictions, the required postings, and your staff record documentation was monitored as a partial assessment of applicable child care requirements. You stated that no medications are administered or are on the premises and that transportation is not provided. Currently no children are enrolled for second shift care. During today’s visit, a partial assessment of applicable child care requirements was conducted. The following violation was cited and corrected during the visit. Today we discussed that role modeling plays a vital role in a child's development, and child care staff can help children learn healthy behaviors through modeling. Children learn by observing and copying the adults in their lives. Place your beverage in a container where children are not aware of the contents. If the drinking vessel is clear, then the only beverages that should be in the classroom are beverages that the children can consume. It was suggested for staff to use drinking tumblers that are not see through. • Complete the Annual Immunization Report by 1-15-25. The reporting period for child care immunizations is now open. Reporting for 2024-2025 has been extended due to Hurricane Helene. Children have a grace period from requirements until 11-30-24, and reports will be accepted until 1-15-25. This legislation allows facilities to remain at their current star level without a reassessment until June 30, 2024 and when reassessed it reduces the education evaluation requirements for lead teachers from 75% to 50% until June of 2026. To prepare for the reassessment process, DCDEE has developed a cohort system. There are three cohorts, and each include a preparation year and a reassessment year. This facility currently operates with a Three (3) Star License that was issued on 6-17-22. The last sanitation inspection was completed on 10-26-23 with an “Superior” classification. The last approved fire inspection was conducted on 1-30-24. The NC Secretary of State website was reviewed today and this facility’s corporation’s name, Regina’s Kidz Academy #2, Incorporated was listed as “Current-Active.” Today’s visit was conducted with you, Shontial McAllister. I monitored the two (2) classrooms in use for compliance with staff/child ratios, supervision, and staff interaction/discipline. The program was also monitored for health and safety requirements both indoors and outdoors using the annual compliance monitoring checklist. Children completed daily activities with adequate supervision. The infant was observed sleeping in a pack-n-play, and the preschool-aged child was observed reading books with the caregiver. Afterwards the preschool-aged child went outside for gross motor play. Once inside the child washed their hands and ate lunch brought from home. The program requirements, along with equipment and furnishings were observed. Compliance with staff records, along with children’s records were reviewed today. You stated no medications are administered and no medications were observed on the premises. You added that this facility does not provide transportation. The following violation of child care requirements was cited during today’s visit and must be corrected immediately. Technical assistance on how to maintain compliance was provided. It is an important part of licensure. It was suggested to set an alarm weekly to remind you and your staff to complete ongoing training. Ongoing training can be completed for free through DCDEE Moodle, the Resource and Referral Child Care Services Association https://www.childcareservices.org/providers/, and other entities that provide approved ongoing training. • A suggestion was made to save the most recent child care requirements in Chapter 9 effective January 2024, to your desktop computer for easy access. • We discussed rotating toys and providing developmentally appropriate toys and equipment for the age groups in care. Today I shared some age appropriate manipulatives, blocks, cloth food/pretend play items for your infant and toddler classroom. • Mr. Gary was observed on the premises during today’s visit. Upon arrival to the facility he was outside working in the flower bed in the front yard of the residence. You stated that Mr. Gary was the lawn maintenance guy and that he is not employed as a caregiver/staff member and that he does not have any caregiving responsibilities. • Please link you and your staff asap.
