Your last annual compliance visit was on 5-12-25. My supervisor, Michele Remington, accompanied me on this visit. A checklist was used to note the requirements monitored. Today’s visit was conducted with you, Mary Overby, Provider. Your FCCH operates with a 3-STAR rated license, with a capacity of 8 children per shifts for children ages one to twelve years old. You were given a copy of the children’s enrollment forms, and you took pictures of the provider verification form, operator’s statement of responsibility form and the visit summary cover page. These were also mailed to you. The primary care area of the FCCH has two (2) converted bedrooms; one at the end of the hall, and one in the first room on the left side of the hallway. The home is a ranch style home. You accompanied us on the walk-thru indoors and outside. We stated to you that you did not need to change your routine due to the visit, that you could continue with the daily activities. You stated that you needed to be with us so when we had questions or needed something you could respond. She played with a doll and other items while indoors and played outside when we conducted our walk-though outdoors and checked the vehicle used for transportation. **VERIFICATION OF RECORDS and REMINDERS: *Send me proof of your on-going training hours on Thursday evening, 4-23-26. You stated your last virtual class was on 4-23-26. *EPR Training completed on 8-25-18. The EPR was updated 5-12-25 and must be updated before 5-12-26. Failure to log onto the website may result in losing your entire EPR plan. The EPR can be accessed using the website: https://rmp.nc.gov/ccf/. If you need assistance with your NCID password/username, call 919-814-6326. Refer to your previous visit summaries for additional technical assistance. *You have completed the Asbestos, Lead Paint and a Lead Water test. Your lead water test was completed on 1-17-25. *You completed ABCMS (criminal record portal), keep this portal updated. For example (new household members and/or you have a substitute. Should you need assistance please contact the Criminal Background Check Unit at (919) 814-8401 and someone will assist you. *I reviewed the NC Secretary of State’s website and observed the owner of the facility (Prince’s and Princess’s Day Care, Inc.) is listed as current-active. To maintain compliance with NC laws and rules regarding childcare operation, the following violations must be corrected immediately. Remember to address each violation that’s not marked “corrected during the visit”. Failure to correct any documented violations within the established time, the Division of Child Development & Early Education may deny, suspend, terminate, or revoke any permit to operate (10A NCAC 09 .2000). Violation Number Comment Rule 714 Monthly check for hazards on the outdoor play area was not completed using a form supplied by the Division. The last outdoor inspection was January 2026. No outdoor safety inspections have been completed in February and March. 10A NCAC .1721(e)(5)(A-F) 904 Documentation of the operator’s emergency medical care plan to follow in the event of a child medical emergency, was not on file and available for review. The emergency medical care plan was not completed or posted. .1713 921 Operator did not maintain accurate daily attendance records including documentation of arrival and departure for all children in care, including the operator's own preschool children. Daily attendances showing the names, arrival and departure times were not completed on four children that were at home earlier this morning. No children had been signed in or out at the time of our arrival. .1721(e)(6) 1103 For each child being transported, the operator or other transportation provider did not have identifying information, including the child's name, photograph, emergency contact information, and a copy of the emergency medical care information form required by Rule .1721(a )(3) in the vehicle whenever children were transported. The provider stated she transported four (4) children earlier today and photo IDs were not on file for each child. .1723(13) 1833 A statement acknowledging the parental decision to opt out of the supplemental food provided by the operator was not signed by the parent and/or maintained on file at the home. The provider stated the children provide all their meals and snacks. There were no opt-out forms on file for any of the children. .1706( c) 1853 The operator did not conduct a monthly fire drill. No fire drill was conducted in February and March. The last fire drill was on 1-7-26. .1719(a)(15) & .1721( e)(2) 1854 The operator did not conduct a quarterly lockdown or shelter-in-place drill and or the drill record was incomplete. No SIP or lockdown drill was conducted in March 2026. The last emergency drill was on 12-7-25. .1719(a )(16) & .1721(e )(7) 1920 Indoor and outdoor area was not checked daily for debris, vandalism, broken equipment, and animal waste. one tricycle was missing the rubber handles on the end of the handlebars. exposing the holes in the handlebars where children’s fingers could get stuck. .1719(a)(17) 1962 Activity plans did not include activities intended to stimulate the developmental domains, in accordance with NC Foundations for Early Learning and Development. There was no activity plan posted and no activities were observed during the visit. 10A NCAC 09 .1718(a)(8)(A)(i-iv) 2055 An individual sleeping space for resting such as a bed, crib, play pen, cot, mat, or sleeping bag with individual linens was not provided for each pre-school age child who was in care for four hours or more, or for all children if overnight care is provided. Children that were receiving overnight care did not have individual sleep bed/mat. 10A NCAC 09 .1718(a)(2) ***** SUBSTITUTE/ ADDITIONAL CAREGIVER. Rule .1729 ******** *We discussed that children are not allowed to go with you to doctor’s appointments, funerals, etc. You stated you do not have a substitute caregiver. You stated in the past that your granddaughter, Andrea or your sister-in-law, Mary W., accompanied you to appointments and cared for the children while you were with the doctor. We discussed this situation in depth; when you are not supervising the children (when you are in an appointment) the other person is considered the caregiver in your absence. *Before a substitute can be alone with the children, they must have a completed file at your FCCH. A checklist is on DCDEE’s website under “Provider Documents”. Today, we reviewed the substitute/additional caregiver form, and you stated, you will close if you have a doctor’s appointment, funeral, etc. to attend, since there were too many requirements. Information was previously shared with you on 5-12-25 regarding substitute’s/additional caregivers. ****OVERNIGHT CARE/3rd shift Care. ****** 2055 You stated six (6) children are enrolled for 3rd shift care. You explained that in the bedroom at the end of the hall, two sisters sleep on the daybed, one child sleeps on the couch and three sleep on the floor in sleeping bags. Each child must have an individual sleeping area. There should be at least 18 inches between each child, to help with the spread of germs. **********RECORDS********* *Record keeping is one of the most time-consuming tasks associated with administering a childcare program. It is an important part of licensure. *Proof of records must be available for DCDEE staff to verify they’ve been completed. If you need additional assistance, contact me. *We discussed using the children’s file checklist on DCDEE’s website to make sure all the required documents are on file and to maintain compliance. *To receive credit for any on-going/annual training courses, remember to attach your training certificates to the checklist (as indicated at the bottom of the form). *******CHILDREN’S RECORDS: ***************** *The following was reviewed with you, while children’s records were monitored. *#1895-The health section on the children’s application must be completed. Parents should put “n/a” vs. leaving blanks on the forms. *#919-the top part of each child’s medical report must be filled out by the parents and the bottom part filled out by the doctor. Each section requires different information. *#1712-You stated you transport children to school on a daily basis; this is considered a “routine task” it’s part of your daily routine. All three sections of the Written Plan of Care form must be completed. In section two (2) answer the questions explaining how you perform these routine tasks and parents must sign part (3) three. *#1833-Ms. Overby stated that meals/snacks are provided by the parents and she will provide meals/snacks if needed. ************ATTENDANCE RECORDS********** *#921-Attendance records were reviewed for two (2) weeks. **********ACTIVITY PLANS: ************* *#1962-You stated you purchased a curriculum called Horizons Preschool Curriculum, and it was observed in the unopened box. You stated you will use the curriculum for the children in care. I showed you where the sample fillable form is on DCDEE’s website that you can use and we discussed how to complete the form. We discussed activity plans must be accessible to parents, dated and completed weekly. Activity plans for all children must meet the following minimum licensing requirements: *Include daily activities to stimulate the following 5 developmental domains, as listed in your NC Foundations for Early Learning and Development book: (social &emotional development, health & physical development, approaches to play and learning, language development/communication, and cognitive development). *Include a daily gross motor activity which may occur indoors or outdoors. *Reflect at least 4 activities daily (one of which is gross motor), one of which is offered outdoors, weather permitting. The remaining 3 activities should occur in the following areas (art/creative play, children’s books, blocks and block building, manipulatives and family living/dramatic play). ************* 1st AID KIT in your vehicle ************** *#1996-The First Aid kit in your vehicles contained napkins. You stated you had used all your band-aids. You went inside your home and added gauze and band-aids to your First aid kit. I suggest you add additional items such as tweezers, bandages, disposable gloves, face mask, liquid soap, cold pack for treatment of minor injuries or stabilization of major injuries. Refer to your rule book under Definitions #23 for items required in a first aid kit. ******************UNAPPROVED SPACE*************************** The provider stated that she does not use the prayer shed because it is unapproved space and cannot be used during operating hours by you or any enrolled children. *********************Fire Drills*********************** We discussed when monthly fire drills are conducted, you must press the test button on your smoke detector vs. calling to the children that it’s a fire drill. Children need to hear what the alarm sounds like, so they’ll know what to do in case of an actual fire. We discussed having two means of egress (exits) accessible during operating hours. You have a clear pathway from the rooms used for child care to the kitchen door, the 2nd door in the kitchen/dining area leads to the laundry area/backyard and was blocked with chairs/other items during today’s visit. You stated you use the front door as your alternate exit. *******************SPACE FOR CHILDREN & SANITATION*************** Overall space for children and Sanitation: in the bathroom used by children there was a large cup on the sink with multiple toothbrushes. Children was not observed using toothbrushes. There were three area for children to play and eat. You are planning to choose Pathway 2, Instructional Quality (classroom and instructional quality). I will conduct a visit to your home (after July 2026) to issue a new license to you. Your last annual compliance visit was on 6-4-2024. A checklist was used to note the requirements monitored. Today’s visit was conducted with you, Mary Overby, Provider. Your FCCH operates with a 3-STAR rated license, with a capacity of 8 children for three (3) shifts for children ages one to twelve years old. Your license is in a hold-harmless state. There was one child present and observed playing with manipulatives. Mary’s uncle, John, was visiting the home and was inside upon arrival and remained outside most of the visit. The primary care area of the FCCH is two (2) converted bedrooms; the older children are cared for in the room at the end of the hallway and the younger children are cared for in the first room on the left side of the hallway. The home is a ranch style home. The kitchen and dining room are used by children when meals are served. You stated the children are not allowed in the living room, your bedroom and the hallway bathroom. You stated the children play in the front and side yard; you live alone, you do not have any pets, and do not administer any medications and no incident reports/logs have been needed. I monitored posted documents, transportation, discipline, capacity, safety issues, program and provider records, etc. Fifteen (15) children are currently enrolled, and I spot checked five (5) children’s records. **VERIFICATION OF RECORDS and REMINDERS: *EPR Training completed on=8-25-18. The EPR was updated during today’s visit and will need to be updated again by 5-12-26. *You have completed the Asbestos, Lead Paint and a Lead Water test. Your lead water test was completed on 1-17-25. Keep a copy of the report in your records. *Your CPR & 1st Aid will expire December 30, 2025. Remember these training hours do not count toward your on-going training hrs. *You stated you only transport one (1) child (JM) and he was present today. AN aerosol can of Lysol was on the kitchen counter top. During the visit, the provider put this can on top of the refrigerator. .1719 (a)(7) 1103 For each child being transported, the operator or other transportation provider did not have identifying information, including the child's name, photograph, emergency contact information, and a copy of the emergency medical care information form required by Rule .1721(a )(3) in the vehicle whenever children were transported. 1. One child (JM) that was present and is transported did not have a photo ID attached to his emergency information form. 2. Five (5) children's emergency information form has not been updated within the last year. .1723(13) 1889 Products that are labeled "keep out of reach of children" without any other warnings, were not stored on a shelf or in an unlocked cabinet that is five feet above the finished floor. Products such as lotions, hair products and make-up were stored below five feet in the unlocked bathroom located in the hall, making them accessible to children. .1719(a)(7) **Technical Assistance was given for the three violations cited on TRANSPORTATION, CHILDREN’S RECORDS & SAFETY HAZARDS** *Item # 1103-Children’s emergency information/application must be updated every year, and the emergency information form and photo ID of the child must be with you each time you transport the child. *Item #706 & 1889-Storage of Hazardous Items-Intentional planning of the environment ensures a safe environment has been created which prevents and reduces injuries to young children. Items that require locked storage were improperly stored in the unlocked bathroom and on the kitchen counter. It is extremely important that hazardous items such as any aerosol cans and beauty products such as make-up, lotions, hair products, etc. be in a locked storage or five feet up from the finished floor. Anything that says “keep out of reach of children” must be locked or out of reach of children. This is a repeat violation from 11-19-24. ***** SUBSTITUTE/ ADDITIONAL CAREGIVER. Rule .1729 ******** *Before a substitute can be alone with the children, they must have a completed file. A checklist is on DCDEE’s website under “Provider Documents” the documents are in alphabetical order. ****OVERNIGHT CARE/3rd shift Care. ****** *Remember, since you provide overnight care, each child should have at least a 4” thick waterproof mattress with a fitted sheet. Refer to rule .1728 for more information. *****RECORDS**** *Proof of records must be available for DCDEE staff to verify they’ve been completed. We discussed placing them in your file folders that we created together on past visits. If you need additional assistance, contact me. *We discussed using the children’s file checklist on DCDEE’s website to make sure all the required documents are on file and to maintain compliance. *To receive credit for any on-going training courses, remember to attach all training certificates to the checklist (as indicated at the bottom of the form). ****EPR PLAN **** *Rule .1719-Remember to update your Emergency Preparedness and Response (EPR) Plan every year and print out the cover page showing the revised date. Rule .1719 (a) (2) *The QRIS (new rated license process) is being discussed by the childcare commission and updates are on DCDEE’s website. You can attend the virtual commission meetings and express your comments/opinions on childcare changes. *ABCMS-Criminal Background Portal UPDATE: The process of notifying the Division has changed and is now captured in ABCMS. This change has been in effect since February 2024 and DCDEE has been sending you emails. Effective immediately, you will need to obtain a Business NCID and complete Provider Portal training in Moodle at https://www.dcdee.moodle.nc.gov/course/view.php?id=119. Once the training has been completed and the access code has been given, you must verify your current household members and/or substitutes are noted on the roster. This information should be updated in ABCMS on an ongoing basis as household members move in and when you have a substitute of any kind. DCDEE staff will verify that you have completed this process, and it will be monitored at each visit. Should you need assistance please contact the Criminal Background Check Unit at (919) 814-8401 and someone will assist you. *****This is stated in (childcare law) G.S. 110-90.2 & .2703(r) **** Childcare operators are to notify the Division of any new child care providers working who were hired or move into you Family Child Care Home within five business days by logging into the ABCMS portal. All household members, substitutes and/or additional caregivers/emergency substitutes must have a current criminal record and be linked to your facility ID in the ABCMS portal. If you want your granddaughter, Andrea Overby to be a substitute she must complete this process along with you. This is a two-step process. Today, I showed you an email I sent to you in November 2024, explaining this process. From 6pm to 8pm on May 6, 2025, I provided a 2-hour virtual training course for you. You stated you logged on but through you did this already and did not attend the workshop. a special host that reviewed the ABCMS process in detail. Today, we discussed that the ABCMS is an additional step to getting a criminal record qualifying letter. This must be completed immediately. I’ve emailed you detailed instructions with screen shots as early as 2024 and as late as last week. Staff in the Criminal Record Department are available to assist you. Today’s visit was conducted with you Mary Overby, Provider. Mary’s adult granddaughter, Andrea Overby was present and interacting with the children in a room to the left down the hallway, which is one of the primary care rooms. Your FCCH currently operates with a 3-star license that’s in the hold harmless stage. I conducted a partial assessment monitoring for safety issues, supervision, discipline, criminal records, program records, posted documents, and permit restrictions. Upon arrival one child was present and observed playing with a puzzle. Shortly after my visit, another child arrived. ***Verification of Records & Training*** *I reviewed the NC Secretary of State’s website and observed the owner of the facility (Prince’s and Princess’s Day Care, Inc.) listed as current-active. *EPR plan was updated on 1-8-24 and must be updated on or by 1-8-25. *Annual Professional Developmental Plan was completed on 1-22-24 and must be updated on or by 1-22-25. *You had an older EMC form posted. Remember to print, complete and post the newest emergency medical care plan. The form is located on DCDEE’s website. Today, I gave you and your granddaughter step-by-step instructions how to locate all FCCH provider forms on our website. *The last fire drill was on 11-4-24, the last lockdown drill was on 10-3-24 and the last outdoor inspection was on 11-3-24. One violation was cited and corrected during the visit concerning storage of hazardous items/safety. Technical assistance was given on how to maintain compliance. Storage of Hazardous Items- Intentional planning of the environment ensures a safe environment has been created which prevents and reduces injuries to young children. Items that require locked storage were improperly stored in the unlocked hall bathroom in an area around the handwashing sink accessible to children. It is extremely important that items that have “keep out of reach of children” are locked or five feet up from the finished floor. DCDEE email blasts have been sent out to all providers. The process of notifying the Division has changed and is now captured in ABCMS. This change has been in effect since February 2024. Effective immediately, you will need to obtain a Business NCID and complete Provider Portal training in Moodle at https://www.dcdee.moodle.nc.gov/course/view.php?id=119. No action is needed on your part if you have completed the reference training and are currently using the ABCMS Provider Portal to update information regarding new hires or residents. Once the training has been completed and access has been given, you must verify your facility roster to ensure current staff are noted on the roster. This information should be updated in ABCMS in an ongoing basis as staff members are hired and when their employment is terminated. This satisfies the requirement to notify the Division of new child care providers working who were hired or moved into the child care facility within five business days. A checklist was used to note the requirements I monitored today. Today’s visit was conducted with, you, Mary Overby, Provider. Your FCCH operates with a 3-STAR rated license that will be due for reassessment in COHORT #3. There were no children present during my visit. The primary care area of the FCCH is two converted bedrooms at the back of the one-story home. The younger children are cared for in a room across from the bathroom and the older children are cared for in the room at the end of the hall. The bathroom used by children is in the room at the end of the hallway. The kitchen and dining room are used by children when meals are served. You stated the children play in the front and side yard and you have no pets and do not administer any medications. Today, I gave you technical assistance on record keeping. We discussed how to use DCDEE’s checklists and how to label your hanging files/file folders. I gave you hard copies of the checklists and put post-it notes on your files as reminders and wrote in due dates and highlighted sections on the documents to help you. I gave you an opportunity to ask questions. We discussed the importance of submitting the corrective action letter on or before the due dates to avoid additional visits and/or administrative actions. **VERIFICATION OF RECORDS: *EPR Training completed on=8-25-18 *Prevention of Shaken Baby syndrome and Abusive Head Trauma Policy=3-29-21 *I reviewed the NC Secretary of State’s website and observed the owner of the facility (Prince’s and Princess’s Day Care, Inc.) is listed as current-active. *The last fire drill was on 12/7/23. The last lockdown drill was on 12/7/23 and the last outdoor inspection was on 1/8/24. To maintain compliance with NC laws and rules regarding childcare operation, the following violation must be corrected immediately. Remember to address the violation. We discussed using a children's signature page for the parent to complete. *Health and Safety Trainings must be completed every 5 years. These trainings were due in June 2023 and are free through MOODLE’s website. Today, I gave you the health and safety checklist that’s on DCDEE’s checklist. In order to receive credit for these training courses, you must attach all training certificates to the checklist (as indicated on the form at the top). *We discussed children's emergency information which is located on the children's application should be updated when information changes and at least annually. *We discussed the Recognizing and Responding to Suspicions of Child Maltreatment is found on preventchildabusenc.org as indicated and highlighted on the checklist. *We discussed how to find the medication training you must complete in MOODLE and how to find trainings in MOODLE.
