A checklist was used to note the requirements I monitored today. Today’s visit was conducted with you, Pamela Thorpe, owner/operator. Your FCCH operates with a 4-STAR license. Upon arrival, there were seven (7) children present, five preschool children and two school-age children. They were observed eating lunch and preparing for a nap. The primary care area of the FCCH is a renovated/addition at the back of the FCCH. The entrance is at the back of the FCCH. You stated, in addition to the primary care area, the children have access to the bathroom located in the hallway. The children play in the fenced backyard. There are no pets at the FCCH. You stated you did not administer medications at your FCCH. You are approved for transportation and drive a 2008 Ford E350 van. I monitored children’s files, program files and provider files. Willie, the provider’s husband, is the substitute and his file was in compliance. **VERIFICATION OF RECORDS: *EPR Training completed on=8-3-17 and your EPR plan was updated on 1-28-25. *Your Lead Water test was completed on 4-16-24 & will expire 4-16-2027. One violation was observed, and a corrective action letter was given to me during the visit; no additional information is required. Violation Number Comment Rule 1948 Sleeping infants, ages 12 months and younger were not visually checked every 15 minutes. One infants safe sleep chart was not completed for August 11, 2025. .1724(a)(7) *****TECHNICAL ASSISTANCE/REMINDERS: ***** SAFE SLEEP CHART: Today we discussed the importance of always documenting that infants are placed on their back when being placed in their crib for nap time, unless there is a Written Waiver on file, signed by the child’s parent/legal guardian. This includes if the infant rolls onto their tummy or side when being placed down for nap. By documenting tummy or side when the infant is being placed in the crib for the first time, the care giver is stating they purposefully placed the infant on their tummy or side when placing them in the crib for nap time. Sleep checks should tell a clear story of how the infant was initially placed in the crib when going down for nap and the time the infant went down for nap, each position the infant was observed in during each sleep check conducted by the infant room staff, and when the infant woke up. I suggest that if an infant does not nap on any given day, that you document on their safe sleep chart, NO NAP, on that specific day or if an infant is absent on any given day that you record absent on the day. We also discussed setting a timer for 15 minutes, so you will not forget to complete the safe sleep chart. *ABCMS PORTAL Linking Criminal Background Checks for FCCH’s – FCCH Providers and household members/substitutes/etc. need to log into the Criminal Background Check Portal on the DCDEE website and to link the facility license ID number that’s listed on your license to their qualifying letter. It is important to do this so DCDEE can contact the facility if a random criminal history check uncovers them with a criminal charge or conviction working in a childcare facility. It is also a way for staff at the CBC Unit to keep you updated on the status of out-of-state checks if a new household member/substitute, etc. has a Provisional Qualification. Instructions on how to link to the facility have been provided by newsletters and emails and at visits. In the future, new household members/substitutes, etc. need to link within five days of hire or transfer to your facility. Review Criminal History Record Check Requirements for Child Care Centers in the NC Child Care Requirements 10A NCAC 09 .2703. The last SIP/lockdown drill was on 2-3-25. *Your Lead Water test was completed on 4-16-24 & will expire 4-16-2027. One violation was observed and corrected during the visit; no corrective action letter is required. Violation Number Comment Rule 702 Potentially hazardous items, including but not limited to, power tools, nails, chemicals, propane stoves, lawn mowers, and gasoline or kerosene, whether or not intended for use by children were not stored in locked areas, removed from the premises, or otherwise inaccessible to children. The provider's white storage building was unlocked. .1719 (a) (1) *****TECHNICAL ASSISTANCE/REMINDERS: ***** *Mrs. Thorpe stated her husband was cleaning the yard and shut the door to the storage building, and left it unlocked. During the visit, this violation was corrected. *We discussed you have some pending work on the ceiling in the hallway and in the primary care room that RTI identified. We discussed that you must correct this work and log it into their website. The last SIP/lockdown drill was on 12/4/2023. *Health & Safety Trainings will be due again March 2027. To maintain compliance with NC laws and rules regarding childcare operation, the following violations must be corrected immediately.
